Expression of inducible nitric oxide-synthase in the vestibular system of hydropic guinea pigs.

Immunohistochemical investigations of the guinea pig vestibular system, using a specific antibody to the inducible isoform of NO-synthase (iNOS/NOS II), have been performed 3 weeks after surgical closure of the right endolymphatic duct (n = 7). Endolymphatic hydrops (ELH) of the right temporal bone became evident by excavation of the Reissner's membrane in all seven animals. Those animals revealed iNOS-expression in ganglion cells, in the wall of blood vessels and in nerve fibers of the right vestibular system, while the corresponding left temporal bones and temporal bones of non-operated cont…

Labyrinthine Abnormalities on MRI in Untreated Otosclerosis: Prevalence and Clinical Relevance.

OBJECTIVE: To determine the prevalence and clinical relevance of labyrinthine abnormalities detected by delayed 3D FLAIR MRI in untreated ot … stapedial region (57/58, 98%). Morphological anomalies of the endolymph were rare, with one case of endolymphatic hydrops (1/58, 2%). In contrast, BLB disruption was identified in 34/58 ears (59%), predominantly in the basal cochlear turn (34/58, 59%) and peri-saccular region (32/58, 55%). BLB disruption was strongly associated with endosteal involvement (76%) and roun …

Vestibular type of Mondini anomalies with BPPV and Meniere's disease-like symptoms.

An extremely rare case with labyrinthine anomaly with normal hearing was demonstrated. This case firstly showed transient positioning vertig … generation of Meniere's disease-like vertigo: neuro-otologic findings with furosemide test implied endolymphatic hydrops on the left side. Meniere's disease-like episodic vertigo could be due to subsequently generated endolymphatic hydrops.

Electrocochleography and low-frequency harmonic acceleration in Meniere's disease.

We evaluated a series of 31 consecutive cases of Meniere's disease using electrocochleography (ECochG) and low-frequency harmonic acceleration (HA). The ECochG was positive for endolymphatic hydrops in 74% of the cases. A positive ECochG was strongly related to the degree of hearing loss. The results of harmonic acceleration were consistent with peripheral vestibular disorder in 80% of the cases. Five illustrative cases are also presented.

Polyarteritis nodosa and deafness. A human temporal bone study.

Temporal bone changes were described in a 66 year old woman with polyarteritis nodosa who became deaf 7 months before death. Polyarteritis nodosa of the left internal auditory artery was demonstrated with fibrosis and bone formation involving the cochlea and vestibular system. Endolymphatic hydrops of the basal turn of the cochlea was also present, as well as a chronic perforation of the free wall of the saccule.

Changes in utricular function during artificial endolymph injections in guinea pigs.

Various theories suggest endolymphatic hydrops may cause a rupture of the membranous labyrinth or may force open the utriculo-saccular duct, resulting in a sudden change in inner ear function. Here, we have used slow injections of artificial endolymph into either scala media or the utricle of anaesthetised guinea pigs to investigate the effects of hydrops. Vestibular function was continuously monitored in addition to the measurements of cochlear function developed in our laboratory (Brown et al. Hear Res, 2013). Scala media injection induced consistent functional changes, which occurred in two…

Low plasma renin concentration in patients with Meniere's disease (preliminary report).

Endolymphatic hydrops is the histopathologic hallmark of Meniere's disease. The cause of excess inner ear fluid is unknown, but the possibility of a systemic abnormality of fluid-solute regulation has been considered. One indicator of systemic fluid and electrolyte balance is the enzyme renin. Plasma renin levels were measured in patients with a clinical diagnosis of Meniere's disease and compared to a group of patients who did not have Meniere's disease. The mean plasma renin concentration of patients with Meniere's disease was statistically lower than that of the other subjects. The study's …

Contemporary role of endolymphatic mastoid shunt surgery in the era of transtympanic perfusion strategies.

OBJECTIVES: Although there exist undisputed methods to permanently silence the aberrant end organ, controversy surrounds the durable efficacy of non-ablative interventions. This study provides a contemporary review of our institution's clinical experience in performing endolymphatic mastoid shunt surgery (EMSS) in patients with medically refractory endolymphatic hydrops, or Meniere's disease. METHODS: Between 1984 and 2002, 1,612 patients were referred to our institution with a diagnosis of Meniere's disease. Of these referrals, 1,172 patients met the criteria for Meniere's disease. Although …

[Ménière's disease].

Ménière's disease is based upon a clinical definition: the traditional Ménière's triad, a physiopathological definition, and the presence of hydrops. If all cases of Ménière's disease suffer from hydrops, not all cases of hydrops suffer from Ménière's disease. Recent progress thanks to electrocochleography, determination of the summating potential and its possible reversability under the effect of glycerol, enables a more reliable diagnostic approach, which furthermore includes the usual clinical and para-clinical examinations. Medical treatment has several objectives: the action of water on t

Case Report: Positive Pressure Therapy Combined With Endolymphatic sac Surgery in a Patient With Ménière's Disease.

Positive pressure therapy (PPT) is applied for medically-intractable vertigo in Ménière's disease (MD); however, it remains unknown whether PPT affects in vivo endolymphatic hydrops (EH). In this case report, we describe a 5-year course of MD in a patient in which EH was repeatedly observed. As the patient experienced recurrent vertigo attacks after endolymphatic sac surgery, he began to use the PPT device additionally and vertiginous episodes decreased in accordance with a decrease in the EH volume. The mechanism of PPT is suggested that the pressure increase in the middle ear inhibits EH dev…

Aquaporins as potential drug targets for Meniere's disease and its related diseases.

The homeostasis of water in the inner ear is essential for maintaining function of hearing and equilibrium. Since the discovery of aquaporin … egulated in part via the arginine vasopressin-AQP2 system in the same fashion as in the kidney, and endolymphatic hydrops, a morphological characteristic of Meniere's disease, is thought to be caused by mal-regulation of this system. Therefore, aquaporins appear to be important for the development of novel drug therapies for Meniere's disease and related disorders.

Vascular mechanisms in Meniere's disease.

Normal venous drainage of the vestibular organs through the vein of the paravestibular canaliculus (PVC) may be crucial to inner ear fluid mechanics. It is proposed that increased venous pressure, with resultant venous insufficiency of the vestibular organs, may result in endolymphatic hydrops unless collateral venous circulation develops. Certain variations in pattern of venous drainage where the vestibular organs drain predominantly through the PVC vein may be a predisposing factor. In patients with Meniere's disease, different mechanisms can cause venous insufficiency. One suggested mechani…

[Fluctuating vestibular deficit--a new Ménière disease related internal ear disorder].

A third inner ear disorder similar to Menière's disease and fluctuant hearing loss seems to be due to endolymphatic hydrops. This disorder is characterised by attacks of vertigo without any cochlear symptoms. During the course of the disease, which lasts up to 8 years, the vestibular organ loses its function in most cases. Clinical and electronystagmographic findings are presented, and the possible pathophysiological mechanisms are discussed.

The endolymphatic sac and inner ear homeostasis. II: Effect of glycerol on the sensory end organs with or without colchicine pretreatment.

The effects of glycerol and colchicine on the sensory end organs of the inner ear were investigated in mice. Glycerol alone induced a wideni … cells as well as dissociation of Golgi complexes in the sensory cells. The cochlea showed moderate endolymphatic hydrops. These findings indicate that colchicine affects the inner ear fluid regulating mechanisms which may lead to severe functional derangement after additional glycerol treatment. It is conceivable that the present experiment may serve as a useful model …

Evidence of gadolinium distribution from the endolymphatic sac to the endolymphatic compartments of the human inner ear.

To verify whether injection of substances into the endolymphatic sac (ES) diffuses into the endolymphatic compartments of the human inner ear and in particular to the endolymphatic space of the scala media (ESp-SM), as demonstrated in animals, an exploratory investigation with magnetic resonance imaging (MRI) and intraoperative electrocochleographic recordings (ECoG) was conducted in patients with Ménière's disease (MD) treated with ES decompression. A mixture of dexamethasone and gadolinium (GD) in solution was injected into the ES of 4 patients. The results of the ES injection procedure were
