Interventions for Menière's disease: an umbrella systematic review.

OBJECTIVES: To systematically review the efficacy of interventions for Menière's disease (MD) to report clinical implications of the results and to identify areas for future valuable research. METHODS: In line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Abstracts (PRISMA-A) guideline, a systematic online database search was conducted in which MEDLINE (PubMed), Embase (Ovid) and CENTRAL (Cochrane Library) were searched until May 2021 in order to search for the efficacy of treatment was analysed in a systematic review. High jugular bulb with a diverticulum and vestibular aqueduct dehiscence: an anatomical variant to be aware in patients with hearing loss.

PURPOSE: To describe an anatomical variant that should be consider in patients with hearing loss. METHODS: An 8-year-old girl underwent to temporal bone computed tomography for the evaluation of bilateral conductive hearing loss and further assessment of possible enlarged vestibular aqueduct or high jugular bulb on brain magnetic resonance imaging (MRI). RESULTS: CT of temporal bone showed a cystic cavity with bony sclerotic margins extending from the right jugular foramen to the vestibular aqueduct. Bony dehiscence of the jugular foramen with the right carotid canal was also noted. On brain

Von Hippel-Lindau disease (VHL): a need for a murine model with retinal hemangioblastoma.

Von Hippel-Lindau (VHL) disease is a highly penetrant autosomal dominant systemic malignancy that gives rise to cystic and highly vascularized tumors in a constellation of organs. Patients with VHL disease commonly present with hemangioblastomas in the central nervous system and the eye while other manifestations include pheochromocytoma, clear cell renal cell carcinoma, endolymphatic sac tumors of the middle ear, pancreatic cystadenomas, epididymal and broad ligament cystadenomas. Animal models inactivating the VHL gene product in various organ tissues have been constructed over the past 15 y

Otosyphilis mimics immune disorders of the inner ear.

Syphilis is a well established cause of hearing loss. Sensorineural hearing loss may develop in the congenital or acquired form. The clinical course of the early acquired and late congenital forms are similar: sudden or rapidly progressive bilateral sensorineural hearing loss with mild vestibular symptoms. Cochleovestibular involvement in early acquired syphilis has been related to a basilar meningitis with lymphocytic infiltration of the labyrinth and VIIIth nerve. However, neurosyphilis and inner ear syphilis are not the same disease. Prompt diagnosis and treatment with corticosteroids and p

Molecular analysis of the Pendred's syndrome gene and magnetic resonance imaging studies of the inner ear are essential for the diagnosis of true Pendred's syndrome.

Pendred's syndrome is a combination of congenital sensorineural hearing loss and iodine organification defect leading to a positive perchlorate test and goiter. Although it is the commonest form of syndromic hearing loss, the variable clinical presentation contributes to the difficulty in securing a diagnosis. The identification of the disease gene (PDS) prompts the need to reevaluate the syndrome to identify possible clues for the diagnosis. To this purpose, in three Italian families presenting with the clinical features of Pendred's syndrome, the molecular analysis was accompanied by full cl

Amphibian calcium metabolism.

Calcium is present in amphibian blood at a concentration similar to that in other vertebrates, about 1-2 mmol l-1. The fraction of free calcium in amphibians is lower than that in other tetrapod vertebrates because about 50% of the plasma Ca2+ is bound to plasma proteins and perhaps other molecules. Plasma [Ca2+] varies seasonally, increasing in spring and summer and decreasing in winter. Changes in plasma [Ca2+] also occur during larval development, as the concentration of this ion increases in larval forms as they approach metamorphosis. Calcium is exchanged at a variety of sites in animals.

Immediate restoration of basal sensorineural hearing (Mb Meniere) using a pressure chamber.

A new technique for the restoration of basal sensorineural hearing loss in Mb Meniere was described. Three cases with unilateral basal sensorineural hearing loss, fullness of the ear, and tinnitus were reported. In the acute stage of their disease the patients were treated in a pressure chamber where it was possible to increase or decrease the air pressure within the range +/- 110 cm H2O. The equilibration of middle ear pressures to surrounding air pressures was checked. Exposure to underpressure resulted in a rise of the hearing thresholds at low frequencies and relief of subjective symptoms.

Imaging of Temporal Bone Mass Lesions: A Pictorial Review.

Tumoral lesions of the temporal bone include benign or malignant tumors and congenital or inflammatory lesions. Temporal bone lesions are difficult to approach. Therefore, making a preoperative diagnosis and considering whether the lesions require treatment are necessary; if they require treatment, then the type of treatment requires consideration. These tumors cannot be observed directly and must be diagnosed based on symptoms and imaging findings. However, the differentiation of temporal bone lesions is difficult because they are rare and large in variety. In this pictorial review, we divide

The endocrine system, vertigo and balance.

Steroid, amine and peptide hormones affect the peripheral vestibular system. Vasopressin hypersensitivity of the endolymphatic sac may be implicated in the pathogenesis of Meniere's disease. Specific vasopressin antagonists will help define the role of vasopressin in Meniere's disease. The modulation of central vestibular pathways by neuroactive steroids may involve effects on gamma-aminobutyric acid-ergic and glutaminergic pathways. The vestibular nuclei also express enzymes that are important in the synthesis of steroids and the modulation of their activity. Steroids mediate both facilitator

Type B Haemophilus influenzae-induced otitis media in the mouse.

To characterize the middle and inner ear cellular inflammatory responses to otitis media using immunohistochemical methods, we inoculated type B Haemophilus influenzae into the middle ears of healthy adult BALB/c mice. Mac-1+ neutrophils and macrophages appeared in the middle ear at 3 days. Lyt-1+ T cells and Lyt-2+ T suppressor/cytotoxic cells entered the middle ear mucosa on days 7 and 14. IgG+ and IgM+ T cells were present at all time points, with IgA+ lymphocytes forming the majority of mucosal immunoglobulin-bearing cells at 2 weeks. The cochlear scala tympani contained Lyt-1+ and Mac-1+

[Feasibility of high resolution CT in the demonstration of the vestibular aqueduct (Ménière) and otospongiosis of the labyrinth].

Ever since the introduction of temporal bone imaging by means of high-resolution CT, it appears that the combination of high spatial resolution, high density resolution and the freedom of patient positioning for scanning of optimal otological planes may play a unique role in the diagnosis and follow-up of a number of otological disorders. Two examples are described here. The first is the possibility of determining whether the vestibular aqueduct in idiopathic Ménière's disease is obliterated or not, and if so, whether it is a bony or a fibrous obliteration. Although the results are preliminary

Mondini dysplasia; a clinical and pathological study.

Mondini dysplasia is characterized by bony and membranous anomalies of the inner ear exhibiting a wide range of morphological and functional abnormality. Typically the cochlea is flat, the cochlear duct is short, the auditory and vestibular sense organs and nerves are immature, the vestibule is large, the semicircular canals are wide, small or missing and the endolymphatic sac is bulbous. Incomplete expressions of the disorder may occur with little or no loss of auditory or vestibular dysfunction, while severe forms show no hearing or vestibular response. The disorder may occur in association

Operations on the saccus endolymphaticus and vestibular nerve: have they fulfilled their promise?

The rationale for operations on the saccus and vestibular nerve in patients incapacitated by Meniere's disease as a replacement for total labyrinthectomy, has been the expectancy that less radical procedures will protect these patients from total auditory incapacity. Aware of postoperative hearing losses in some patients after 3 years, we have studied the results in 21 Shambaugh saccus decompression (SD) and 29 middle fossa vestibular nerve section (VNS) operations, in order to assess the claims made for these procedures. We found that while control of vertigo was similar to that reported else

Apical lesions of the cochlea in idiopathic endolymphatic hydrops and other disorders: pathophysiological implications.

Apical lesions of the cochlea may be caused by direct trauma, impairment of blood supply, contamination of perilymphatic fluid and endolymphatic hydrops. It is proposed that endolymphatic hydrops causes apical lesions when endolymph or its components leak into the perilymphatic fluid and pass through the helicotrema to degrade the tissue fluid environment of the sensory and neural structures in this area.

The use of the underpressure chamber in the treatment of patients with Menière's disease.

Thirty-four Menière patients (43 ears) were treated in an underpressure chamber. Békésy audiometry and impedance audiometry were performed immediately before and after the exposure to underpressure. A hearing gain of more than 10 dB. was found in 19 per cent of the ears (8/43). In five ears this improvement lasted for a few days. Only one patient (2 per cent) reported a relief of tinnitus. In 9 out of the 30 patients (30 per cent) with recent vertiginous attacks a positive subjective effect on the vertigo was obtained.

Vestibular nerve section and saccus decompression: an evaluation of long-term results.

The 1972 AAOO committee (Alford, 1972) guidelines brought some uniformity into the evaluation of therapy for Menière's Disease. We have adhered to its recommendations in this long-term follow-up report of 21 saccus decompressions and 29 vestibular nerve sections performed on 46 patients between 1968 and 1977. Comparisons between these and other groups have been possible with regard to: control of vertigo; hearing; tinnitus; and development of hydrops in the contralateral ear. All the vestibular nerve section group have enjoyed sustained relief from vertigo. Class D results (recurrent vertigo)

Delayed endolymphatic hydrops.

Delayed endolymphatic hydrops is a disease entity that can be differentiated from Ménière's disease. Typically it occurs in patients who have sustained a profound hearing loss in one ear, usually from infection or trauma, and then after a prolonged period of time develop either episodic vertigo from the same ear (ipsilateral delayed endolymphatic hydrops) or fluctuating hearing loss, also sometimes with episodic vertigo, in the opposite ear (contralateral delayed endolymphatic hydrops). The ipsilateral form of the disease may be treated by labyrinthectomy but no satisfactory therapy is availab
