Cortical somatosensory potentials evoked by magnetic stimulation of thoracic and lumbar roots.

We applied twin coil magnetic stimulation to thoracic and lumbar roots to evaluate the posterior column function in 50 normal subjects and 34 patients with neurologic disorders. In all nine patients with cervical myelopathy, there were abnormal somatosensory evoked potentials (SEPs) with stimulation of upper thoracic levels. In 14 patients with thoracic or lumbar myelopathy, there were normal SEPs with stimulation of spinal roots above the lesions and abnormal SEPs with stimulation below the lesions. In a patient with girdle sensation between T-3 and T-6, the peak latencies of P2 were signific…

Myoelectric evoked potentials versus locomotor recovery in chronic spinal cord injured rats.

The purpose of this study was to determine the utility of descending evoked potentials in evaluating functional recovery in rats after spinal cord contusion injury. Rats received thoracic contusions at T9 using a controlled-displacement impactor. They were evaluated for 5 weeks postinjury using auditory startle responses (ASR) while alert, or by cerebellar motor evoked potentials (CMEP) while anesthetized. ASR and CMEP were recorded electromyographically from forelimb and hindlimb muscles. Open field locomotor performance was also assessed and recovered to almost normal levels by 3 weeks posti…

[Visual evoked potentials and face recognition. Influence of celebrity and emotional expression].

Visual evoked potentials (VEP) were recorded in the right and left parietal and occipital regions of 40 right-handed controls in a facial recognition task. VEP were studied first according to the renown of the faces, then according to their emotional expression. Asymmetry was noted between the hemispheres: P100 was of greater amplitude and longer latency in the left occipital region. Later components (P400 and P600) were of greater amplitude and longer latency in the right parietal region in all situations. P100 latency on the left side was shorter for renowned faces than for non-renowned face…

Prognostic value of early somatosensory evoked potentials in neonates with cystic leukomalacia.

Somatosensory evoked potentials (SEPs) were performed between 31 and 49 weeks postmenstrual age on 33 neonates with extensive cystic leukomalacia. 27 had periventricular leukomalacia (PVL), while six had deep white matter lesions. All but two of the 27 infants with PVL had a reproducible potential at discharge, being delayed in 11 and within the normal range in 14. No potentials could be obtained in any of the infants with cysts in the deep white matter. All surviving infants developed severe neurological sequelae, irrespective of the N1 being delayed or within normal range. These data suggest…

Somatosensory evoked potentials in the neurological sequelae of treated vitamin B12 deficiency.

10 patients with chronic sequelae from vitamin B12 deficiency showed a characteristic profile of abnormalities of median and peroneal somatosensory evoked potentials: mild to moderately slow peripheral conduction to Erb's point, normal conduction or mild slowing across the cervical portion of the median somatosensory evoked potentials, and absence or severe slowing of impulse propagation along the spinal cord with the peroneal somatosensory evoked potentials. These findings are compared with those of other published series of somatosensory evoked potentials in vitamin B12 deficiency. These res…

The value of visual evoked potentials in optic neuropathy of Graves' disease.

Visual evoked potentials (VEP) were tested in patients with ophthalmopathy (OP) of Graves' disease (GD) before treatment and half-a-year and 5-7 yr after orbital decompression (7 patients) or retrobulbar irradiation (3 patients). Five patients not requiring treatment for OP and 16 healthy subjects served as controls. Treatment of OP was given on clinical grounds including decreased visual acuity (VA) indicating optic neuropathy. Before treatment VA was decreased in 5 out of 19 eyes whereas VEP were abnormal in all. The mean latency at N60 both in the operated (83.1 +/- 21.6 ms) and in the irra…

Lesions of the sensorimotor region: somatosensory evoked potentials and ultrasound guided surgery.

In 10 patients with lesions of the sensorimotor cortex cortical SEP were registered to identify the postcentral gyrus, and intra-operative ultrasound sonography served to locate the lesion. The combination of both techniques helped to find the optimal approach to the lesion. Postoperative results were considered favourable, as only one patient suffered transient postoperative deterioration, six were unchanged and in three patients the pre-operative motor deficits were improved. The combination of intra-operative ultrasound and neurophysiological identification of the sensorimotor cortex is con

Asymmetries in topographic brain maps of auditory evoked potentials in the elderly.

We compared topographic brain maps of the middle response (MR) and late response (LR) auditory evoked potentials in two groups of elderly males. Subjects were categorized on the basis of the performance of the poorer hearing ear on the Synthetic Sentence Identification (SSI) test. Evoked potentials, obtained in response to 1000-Hz tone pips and tone bursts, were recorded under three conditions of sound stimulation:binaural (BIN), left ear only (LE), and right ear only (RE). Comparisons of the topographic brain maps generated by the two groups revealed significant asymmetry in the MR of the exp…

Medical cure of a brainstem abscess and serial brainstem auditory evoked potentials.

The brainstem abscess of a nine-year-old girl with tetralogy of Fallot was cured after six weeks of parenteral antibiotic therapy, without surgical intervention. Serial studies of brainstem auditory evoked potentials were undertaken until the patient was clinically normal. To the authors' knowledge, this is only the second medically cured case reported in the literature, and it is the first case studied with serial brainstem auditory evoked potentials. If the clinical status allows, medical treatment of a brainstem abscess with appropriate antibiotics could be tried before surgical interventio…

[Brainstem lesion in multiple sclerosis, blink reflex, and brainstem auditory evoked potentials].

Blink reflex (BR) and brainstem auditory evoked potentials (BAEP) were recorded from 168 patients with several diagnostic categories of multiple sclerosis, from which 98 complained brainstem symptoms (BSS+). From the whole group, the BR showed a higher degree of abnormality (45.75%) than BAEP (27.95). This range increased in the group BSS+ (52.15% for the BR and 34.9% for the BAEP) and even more when sings or symptoms of brainstem were present at the time of exploration (BSS+P) 60.4% for the BR and 41.3% for BAEP. Among patients who never complained brainstem symptoms, the BR disclosed a subcl…

Somatosensory and magnetic evoked potentials in a postoperative paraparetic patient: case report.

Somatosensory evoked potential (SSEP) testing is increasingly being used to test for spinal cord injuries and to monitor spinal surgery to reduce the risk of paraplegia. It is a sensory test, but it is assumed that any process severe enough to affect the motor tracts will also affect the sensory tracts and, therefore, be identified. Increasingly, however, isolated motor-tract involvement has been reported. A new technique, magnetic coil stimulation of the cortex, directly monitors the motor tracts. We report a case where the SSEP was normal although the magnetic motor-evoked potential was abno

[Visual evoked potentials in chessboard pattern reversion in children with optic neuritis].

Visual evoked potentials (VEP) to chess pattern reversion were studied in 17 children with optic neurites during the acute stage and in the course of convalescence 1-2 months and 1-3 years after the disease onset. Coarse VEP changes were detected in the patients during the acute stage of optic neuritis. A positive time course of VEP was followed up in late periods after the acute attack of the disease. Despite normalization of vision acuity, no complete restoration of VEP to small-cell chess patterns was seen. The findings evidence that studies of VEP to chess pattern reversion should be carri…

Visual evoked potentials in optic nerve injury. Does it merit a mention?

The value of Visual Evoked Potentials (VEP) in the management of indirect optic nerve injury was prospectively studied in 128 patients. In fifty patients who were either unconscious, uncooperative or had massive black eye, preventing the assessment of vision, a light Emitting Diode (LED) was used to record visual evoked potentials. The initial VEPs were normal in 17, abnormal in 49 and absent in 62. All 17 patients with normal VEP showed visual recovery. Amongst the 49 patients with abnormal VEP, 43 (88%) showed improvement. In 62 patients, initial VEPs showed no response but, in 12 subsequent…

Postnatal development of flash visual evoked potentials in the jaundiced Gunn rat.

In this study, the postnatal development of flash visual evoked potential (VEP) has been monitored in the jaundiced (jj) Gunn rat model of neonatal hyperbilirubinemia to determine whether this evoked response is affected by bilirubin-induced neurotoxicity. VEP could first be recorded at 16 d of age. The jj rats exhibited prolonged wave latencies and lower wave amplitudes during the 3rd wk of postnatal life, when compared with their nonjaundiced littermates. There was no correlation at 21 d of age between VEP parameters and either bilirubin levels or body weight. About one third of the jj anima

Somatosensory evoked potentials of the medial and lateral plantar and calcaneal nerves.

The ideal electrodiagnostic procedure to assess possible plantar neuropathies continues to elude investigators. Motor studies are rarely abnormal, pure sensory studies may be difficult to obtain, needle electromyography can demonstrate membrane instability in normal feet. Mixed nerve plantar studies may be more diagnostically valuable than the other techniques but they also have shortcomings. In this report, a technique utilizing somatosensory evoked potentials to assess the medial and lateral plantar and calcaneal nerves is demonstrated. Normative data with respect to latencies, amplitudes, a…

Amplitude and latency characteristics of spinal cord motor-evoked potentials in dogs.

The motor-evoked potential can be reliably recorded in anesthetized dogs by use of percutaneous placement of active recording electrodes near the dorsal lamina of the vertebral column. Two types of responses were observed in this study; short (less than 5.5 ms at T9-10)- and long (greater than 5.8 ms at T9-10)-latency waves. Short-latency waves are larger in amplitude and appear with higher stimulus intensities than do long-latency waves. Short-latency waves are conducted at greater than 80 m/s and may not reflect pyramidal tract activation. The safety of using higher intensity stimuli to gene

Urologic function after experimental cauda equina compression. Cystometrograms versus cortical-evoked potentials.

Twenty female beagle dogs underwent an L6-7 laminectomy and six dogs each had 25, 50 or 75% constriction of the cauda equina and 2 control dogs had laminectomy only. Cystometrograms were performed pre- and post-operatively and three months after constriction. Cortical evoked potentials were monitored pre- and post-operatively and monthly for three months. After three months of constriction, the cauda equina of these dogs in each group was examined histologically and vascular circulation was examined by latex and India ink injection (Spalteholz technique). The control dogs had normal CMGs and C…

Do visual evoked potentials detect neural damage in children treated for cancer?

Abnormal visual evoked potentials (VEPs) have been reported in children treated for acute lymphoblastic leukaemia (ALL), which suggests that VEPs may be useful in screening for toxicity. The authors investigated this by recording flash and pattern VEPs in a control group of 34 siblings of patients, a group of six children studied longitudinally during the early stages of treatment for ALL, and three other follow-up groups. In only three follow-up patients were VEP results outside the normal range and the six ALL patients did not develop new abnormalities during early treatment. Although differ…

Trigeminal somatosensory evoked potentials: technical parameters, reliability and potential in clinical dentistry.

Various parameters of the short-latency trigeminal somatosensory evoked potential (TSEP) waveform were investigated in 12 healthy volunteers. The waveform was found to comprise two principal components at 23 (N1) and 39 (P1) ms and to show a significant contralateral dominance. Varying the stimulus strength significantly affected the amplitudes but not the peak latencies. The waveform pattern was maintained irrespective of whether the reference site was Fpz or Cv7. The peak latencies of N1 and P1 were found to be very reproducible. To a lesser extent, N1-P1 amplitude was also reproducible. The

Evoked potentials suggest cranial nerves and CNS involvement in chronic relapsing polyradiculoneuropathy.

Brainstem auditory and visual potentials were recorded from 6 patients with chronic idiopathic polyradiculoneuropathy. Evoked potentials were either completely absent or their latency was prolonged suggesting slow conduction in visual or auditory pathways. The results of our study provide further evidence suggesting subclinical involvement of cranial nerves and central nervous system sensory pathways in chronic inflammatory demyelinating polyradiculoneuropathy.
