Infections of the retroperitoneal space may present with insidiously vague symptoms and non-specific clinical signs. We report a case of a retroperitoneal abscess presenting as necrotizing fasciitis of the thigh from direct spread over the iliac crest. In cases of necrotizing fasciitis of the thigh with no obvious source, an intra-abdominal nidus of infection may be considered.

BACKGROUND: Necrotizing fasciitis (NF) is a severe life-threatening soft tissue infection characterized by rapidly spreading necrosis of the fascia and the subcutaneous tissue. Its incidence owing to invasive Streptococcus pyogenes has significantly increased in children recently. Our experience with NF in children to describe diagnostic and therapeutic aspects is hence presented herein.
METHODS: Records of children who were treated for NF in our unit from 1999 to 2006, inclusive, were reviewed retrospectively. Information recorded for each patient included medical history, clinical character…

AIM: Here we present the clinical symptomatology of and therapy for necrotizing fasciitis.
METHOD: The case of a 35-year-old female patient with sustaining fractures of the 5 (th) and 10 (th) thoracic vertebrae and a pulmonary contusion and without any skin lesions is presented.
RESULTS: Conventional x-rays and computed tomography revealed stable spine fractures not necessitating surgical intervention. Fifteen days after the accident the patient developed septic conditions. An interdisciplinary search (surgical, neurological, urological, internal medicine) for the septic focus first remained…

Necrotising fasciitis is a rare, life-threatening, soft-tissue infection characterised by rapidly spreading inflammation and subsequent necrosis of the muscle, fascia and surrounding tissues. We report a case of necrotising fasciitis originating from the shoulder in a 59-year-old female patient. Necrotising fasciitis of the shoulder is very rare and has a poor prognosis because of its potential to spread to the chest wall. It can occur in otherwise healthy people without any predisposing conditions and could present as muscle strain. Disproportionate pain is the hallmark of this condition. Ult…

BACKGROUND: The term necrotizing fasciitis (NF) is now used in a generic sense to include all diffuse necrotizing soft-tissue infections except gas gangrene. It is a synergistic, polymicrobial soft-tissue infection associated with rapid progression, extensive necrosis, profound systemic toxemia, considerable morbidity and a high mortality rate. Although the disease is no respecter of age and affects a wide age group, adults are known to be more commonly affected than children.
AIMS: To highlight the differences and similarities in the modes of presentation and results of intervention of NF in…

Necrotizing soft tissue infections encompass a wide variety of clinical syndromes resulting from introduction of various pathogens into inju … tions in the upper extremity, focusing on gas gangrene, or clostridial myonecrosis, and necrotizing fasciitis, to facilitate early diagnosis and optimal management of these lethal diseases.

BACKGROUND: Necrotizing fasciitis is an uncommon but serious complication of chickenpox infection in young children. Because many of these infections affect the musculoskeletal tissues, orthopedic surgeons are often the first caregivers to be involved in diagnosis and treatment. Our objective was to review the diagnostic features of necrotizing fasciitis and analyze treatment methods to control and eradicate the musculoskeletal infection.
DESIGN: A review.
SETTING: The Children's Hospital of Eastern Ontario, Ottawa, a major Canadian pediatric trauma and referral centre.
PATIENTS: Five child…

The authors report on a neonate who presented with acute appendicitis and who went on to have fatal necrotizing fasciitis. Although recognized in adults, this rare combination of conditions has not been reported previously in the neonate and requires rapid recognition with aggressive medical and surgical intervention.

Historically maggots have been used to clean necrotic war wounds. With the ready availability of sterile maggots, they are being used increasingly in surgical practice. Although maggots have most frequently been employed in the management of necrotic wounds and ulcers involving the lower limb, we have identified a particularly useful application in the head and neck and describe a case in which maggots played a significant part in the successful treatment of a florid necrotizing cervical fasciitis in a patient, who was unfit for repeated surgical debridement.

We reviewed eight patients with necrotizing fasciitis of odontogenic origin. There were three women and five men, mean age 58 (range 46-72), and none had any associated medical condition such as diabetes. All cases had symptoms of toothache for a mean duration of 34 days (range 26-42) before they sought treatment. Infection originated in the molar teeth region, and initially presented as an odontogenic or periodontal abscess. The clinical features of necrotizing fasciitis became apparent only after the superficial fascia had been invaded. The transient unusually reddish hue for a dark skin mig…

We report the case of a one-day-old newborn infant, female, birth weight 1900 g, gestational age 36 weeks presenting with necrotizing fasciitis caused by E. coli and Morganella morganii. The newborn was allowed to fall into the toilet bowl during a domestic delivery. The initial lesion was observed at 24 hours of life on the left leg at the site of the venipuncture for the administration of hypertonic glucose solution. Despite early treatment, a rapid progression occurred resulting in a fatal outcome. We call attention to the risk presented by this serious complication in newborns with a conta…

INTRODUCTION: Necrotizing fasciitis is an uncommon infectious entity that poses difficult diagnostic and therapeutic management decisions.
PURPOSE: This paper addresses the presentation, evaluation and management of the patient with a necrotizing soft tissue infection.
CASE REPORT: A 54-year-old man presented to his physician with pain and swelling of the left anterior chest wall following a presumed insect bite several days prior. He was treated with oral antibiotics but returned to the office three days later with increased swelling, pain, and erythema in the axilla. Necrotizing fasciitis …

This article describes a case of necrotizing fasciitis in a 52-year-old man with previously undiagnosed diabetes. Treatment involved massive debridement and subsequent dressing of the open surgical wound. A modern hydrofibre dressing (Aquacel) was inserted intraoperatively and subsequently continued postoperatively. Wound healing occurred over 3 months and the patient was discharged with no disability. The dressing managed the exudate level and kept the wound moist. It also was well tolerated by the patient, was comfortable and easy for the nurses to remove and apply.

Group A streptococci are common colonizers of the skin and upper respiratory tract. Serious infections of the respiratory tract as well as the skin and soft tissue are common. Highly virulent Group A streptococci are not infrequently the cause of invasive, life-threatening infections. Necrotizing fasciitis is uncommon and rarely the result of Group A streptococci. Necrotizing fasciitis of the psoas from Group A streptococci has been reported as a complication in patients with colon cancer perforation or peritonitis. We report the first case of Group A streptococcal necrotizing fasciitis of the…

Necrotizing fasciits (NF) is a rapidly progressive disease characterized by extensive necrosis of the fascia, skin, and subcutaneous tissue, with characteristical sparing of the underlying muscle. It is considered to be a rare entity and is associated with a high mortality rate since early recognition and treatment are not provided. Extensive surgical debridement of all necrotic tissues and appropriate antimicrobial treatment are the cornerstones for a successful treatment. We present a case of NF in a 36-year-old man and discuss it's pathophysiology, clinical manifestations and the best thera
