Dexfenfluramine for weight gain secondary to psychotropics.

We describe the use of dexfenfluramine (Redux) in six patients with mood disorders (bipolar disorder = 3; major depression = 3) who developed significant weight gain as a result of their psychotropics. All patients were females with an age range of 27-58 years. The duration of the trial varied from 3 to 20 weeks, with the dexfenfluramine dosage being 15 mg twice daily. Weight loss ranged from 2 to 23 pounds. None of the patients had any evidence of primary pulmonary hypertension or serotonin syndrome, despite being on serotonergic agents such as fluoxetine, paroxetine, and sertraline.

[Effects of chronic administration of benfluorex on the pharmacokinetics of iodine 123 labelled insulin in Zucker obese rats (fa/fa)].

Zucker obese rats (fa/fa), aged 8 to 10 weeks, were treated orally by benfluorex (2 x 25 mg.kg-1, day-1) for 2 weeks and were compared with a control group of the same age treated with placebo. Benfluorex induced a break in the weight curve, a significant fall in serum triglycerides, blood glucose and plasma insulin and in the insulin content of the pancreas. Following intrajugular injection of iodine 123 labelled insulin, the liver of the treated animals bound 25 percent more insulin than the liver of control animals. Conversely, the renal clearance of insulin of the treated animals was reduc

The effects of benfluramate on carbohydrate metabolism in obesity.

Glucose turnover rate, glucose oxidation and lactate production from glucose were investigated in 5 obese subjects using the primed infusion technique, before and four weeks after a course on benfluramate therapy. All subjects lost weight on the drug. Mean glucose turnover rate, but not CO2 and lactate production increased after treatment. The changes in metabolism appear to reflect the combined effects of weight loss and drug action.

Drugs induced pulmonary arterial hypertension.

Pulmonary arterial hypertension (PAH) is a rare disorder characterized by progressive obliteration of the pulmonary microvasculature, resulting in elevated pulmonary vascular resistance and premature death. According to the current classification, PAH can be associated with exposure to certain drugs or toxins, particularly appetite suppressant drugs, such as aminorex, fenfluramine derivatives and benfluorex. These drugs have been confirmed to be risk factors for PAH and were withdrawn from the market. The supposed mechanism is an increase in serotonin levels, which was demonstrated to act as a…

Study on detection of drugs in slimming health foods using GC-MS/MS.

The determination of five drugs, fenfluramine (FEN), N-nitrosofenfluramine (NFE), sibutramine (SIB), mazindol (MAZ) and phenolphthalein (PHP), was studied in slimming health foods using GC-MS/MS. These drugs have been detected at high rates, especially in slimming health foods. Prolonged or excessive consumption of non-approved or unauthorized pharmaceuticals may cause serious adverse health consequences. In this study, samples were extracted with methanol and ultrasonication. Analyses were performed by GC-MS/MS, using established MS/MS parameters in the electron ionization (EI) mode and chemi…

Benfluorex and unexplained valvular heart disease: a case-control study.

BACKGROUND: Recent case reports suggest that benfluorex, a fenfluramine derivative used in the management of overweight diabetic patients and dyslipidemia, is associated with cardiac valve regurgitation. METHODS: We conducted a case-control study. Eligible patients were those admitted in the cardiology or the cardiac surgery units of our hospital between January, 1(st) 2003 and June 30(th) 2009, with mitral insufficiency diagnostic codes (ICD-10 I340 and I051). Patients with either a primary cause (degenerative, known rheumatic heart disease, infectious endocarditis, congenital, radiation-ind…

Adulteration of over-the-counter slimming products with pharmaceutical analogue--an emerging threat.

OBJECTIVES: To review pharmaceutical analogue adulteration of non-prescription slimming products. DESIGN: Retrospective study. SETTING: Tertiary referral centre for toxicology analysis, Hong Kong. PATIENTS: All patients known to have been hospitalised after taking slimming products adulterated with pharmaceutical analogues from September 2004 to December 2006. MAIN OUTCOME MEASURES: Age, reasons for hospital admission, major biochemical findings, and toxicology analysis results of the offending slimming products. RESULTS: N-nitrosofenfluramine, an analogue of fenfluramine with hepatotoxic…

Role of nitrergic system in behavioral and neurotoxic effects of amphetamine analogs.

Several amphetamine analogs are potent psychostimulants and major drugs of abuse. In animal models, the psychomotor and reinforcing effects of amphetamine, methamphetamine (METH), 3,4-methylenedioxymethamphetamine (MDMA; Ecstasy), and methylphenidate (MPD; Ritalin) are thought to be dependent on increased extracellular levels of dopamine (DA) in mesocorticolimbic and mesostriatal pathways. However, amphetamine analogs that increase primarily serotonergic transmission, such as p-chloroamphetamine (PCA) and fenfluramine (FEN), have no potential for abuse. High doses of METH, MDMA, PCA, and FEN p…

Endocardial myxomatous change in Harlan Sprague-Dawley rats (Hsd:S-D) and CD-1 mice: its microscopic resemblance to drug-induced valvulopathy in humans.

A full assessment of all heart valves in rats and mice is often impractical and is usually not performed in routine toxicity studies, largel … ved. These microscopic features were similar to those seen in valvular disease in humans induced by fenfluramine-phentermine (fen-phen), ergot alkaloids (ergotamine, methysergide), and carcinoid syndrome. The mitral valve in rats and pulmonary valve in mice were most frequently affected. An association between murine progressive cardiomyopathy (MPC) and EMC wa …

[Drug treatment of anorexia nervosa and bulimia nervosa. A review].

With the apparent increase in prevalence of anorexic and bulimic eating disorders, the search for effective treatments for these disorders h … strated for tricyclic antidepressants (imipramine, desipramine), serotonergic agents (fluoxetine, d-fenfluramine), non-selective monoamine-oxydase-inhibitors (isocarboxazide, phenelzine) and trazodone. The antibulimic effect appears not to be associated with the antidepressant effect. Theoretical, methodological and practical issues concerning pharmacological …

Neuropharmacology of drugs affecting food intake.

The importance of the central monoamines NE, DA and 5-HT in ingestive behavior has inevitably resulted in considerable effort being expended in attempting to implicate these monoamines in the mechanism of action of anorectic drugs. The statements that amphetamine-induced anorexia is unlikely to be due to central serotoninergic systems and that central noradrenergic and dopaminergic systems are not implicated in the appetite suppressant effect of fenfluramine are in all probability correct. However, to attribute the ability of drugs to decrease food intake unequivocally to a specific effect on …
