Acute distal radioulnar joint instability.

The distal radioulnar joint (DRUJ) is critical for pronosupination of the forearm and for upper extremity function in general. Because of its critical role in tool handling, some authors place the human DRUJ on par with the opposable thumb in the evolution of man. This seemingly simple cylindrical joint is stabilized by a complex network of soft tissue constraints, primarily components of the triangular fibrocartilage complex. Chief among these are the dorsal and palmar radioulnar ligaments. Any injury that disrupts these stabilizers, their balanced tension, or the bony congruity of the joint …

[Stage oriented surgical cartilage therapy. Current situation].

Chondral or osteochondral lesions are typical injuries in orthopaedics and traumatology. Since there is no regeneration of damaged articular … and autologous chondrocyte transplantation normally achieve a regeneration of the defect with only fibrocartilage tissue, but both can achieve good medium-term clinical results. Each of these therapeutic principles has certain major indications. In order to select an adequate therapy, the classification of chondral or osteochondral lesion is needed. From a mult …

Calcium pyrophosphate dihydrate crystal deposition disease.

Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease is characterized by the accumulation of pyrophosphate dihydrate crystals in articular and periarticular tissues. Various terms have been utilized to describe this arthropathy, which has led to some confusion. CPPD crystal deposition disease is among many conditions that may result in crystal deposition within cartilage. Chondrocalcinosis is a pathologic and radiographic term denoting calcification of cartilage within joints including both hyaline articular cartilage and fibrocartilage. Pseudogout is a clinical term applied to an…

Analysis of cartilage-derived retinoic-acid-sensitive protein (CD-RAP) in synovial fluid from patients with osteoarthritis and rheumatoid arthritis.

We have measured the concentration of cartilage-derived retinoic-acid-sensitive protein (CD-RAP) in synovial fluid (SF) from the knees of 49 … ohistochemical studies showed expression of CD-RAP in the cytoplasm of chondrocytes in newly-formed fibrocartilage. Since CD-RAP is mainly produced in young and proliferating chondrocytes, our results suggest that the level of CD-RAP in synovial fluid reflects remodelling of articular cartilage and may be used as a marker to estimate objectively the restorative …

Chronic wrist pain.

Nonarticular chronic wrist pain continues to be a challenging subject in which interest and knowledge are increasing. Although many more excellent articles could have been included in this review, an attempt is made to discuss papers that have impact on the recognition, diagnosis, and treatment of various wrist problems. Developments in diagnostic algorithms and refinements in the use of imaging and arthroscopic tools are reviewed. Arthroscopy provides exciting direct visual information and therapeutic opportunities for triangular fibrocartilage complex disruptions and lunatotriquetral ligamen…

The gross and histologic anatomy of the scapholunate interosseous ligament.

The scapholunate interosseous ligament, which connects the carpal scaphoid and lunate bones, was evaluated by gross dissection and serial histologic sections in transverse, sagittal, and coronal planes in 21 fresh and 16 fixed adult cadaver wrists. The scapholunate interosseous ligament is consistently divisible into three anatomic regions: dorsal, proximal, and palmar. The dorsal region is thick and composed of short, transversely oriented collagen fibers. The proximal region is principally composed of fibrocartilage, with a few superficial, longitudinally oriented collagen fibers. The proxim…

Single stage excision of preauricular sinus.

A total of 44 patients with preauricular sinus underwent one-stage surgical treatment. There were 32 unilateral and 12 bilateral cases giving a total of 56 sinuses. Of these 16 were infected at the time of presentation. Based on the observation that preauricular sinus represents the duct of a preauricular gland which is closely adherent to the fibrocartilage of the ear, the authors propose a method for the complete cure of this lesion by excising the whole gland and duct together. This technique was used in all cases including 16 patients with infected sinuses. There were three recurrences, tw…

Acromial enthesopathy and rotator cuff tear. A radiologic and histologic postmortem investigation of the coracoacromial arch.

Changes of the coracoacromial ligament (CAL) at its insertion into the undersurface of the acromion were studied radiologically and histologically in 76 autopsy specimens. Two changes were noted: (1) a downward, bony projection of the acromion, an anatomic variant limited to the area covered by the CAL, which might reduce the height of the subacromial compartment, and (2) a thickened layer of fibrocartilage, constituting a potential cause for narrowing of the subacromial space. The former might act as a predisposing factor for the impingement syndrome, whereas the latter could develop in respo…

Ligamentous sports injuries of the hand and wrist.

Athletic injuries of the hand and wrist continue to increase as athletes get bigger, faster, and the weekend warrior continues to be active throughout his or her lifespan. The management of hand and wrist injuries in these athletes depends on the sport, position, timing during the season the injury occurs, and how many years the athlete has left to play in competition. In the past, these injuries were thought to be trivial; but if not ideally managed, complications can occur which may keep the skilled athlete or the weekend warrior out for a prolonged period of time. This article concentrates

MRI morphometric hip comparison analysis of anterior acetabular labral tears.

OBJECTIVE: Anterior (3 o'clock) acetabular labral tears (AALTs) have been reported to be associated with iliopsoas impingement (IPI). However, no study has examined the association between anatomical bony variables of the hip joint and AALTs. The purpose of this study was to evaluate the association between AALTs, femoroacetabular impingement (FAI) and other bony variables of the hip. MATERIAL AND METHODS: Seventy-six out of 274 hip MRI records met the inclusion criteria. Two independent blinded investigators evaluated the location of acetabular labral tears (ALTs), edema at the musculotendin

Diagnosis and management of superior labral anterior posterior tears in throwing athletes.

Injury to the superior glenoid labrum is increasingly recognized as a significant source of shoulder pain and dysfunction in the throwing athlete. Several theories have been proposed to explain the pathogenesis of superior labral anterior posterior (SLAP) tears. The clinical examination of the superior labrum-biceps tendon complex remains challenging because of a high association of other shoulder injuries in overhead athletes. Many physical examination findings have high sensitivity and low specificity. Advances in soft tissue imaging such as magnetic resonance arthrography allow for improved

Fibrocartilaginous embolism: a cause of acute ischemic myelopathy.

STUDY DESIGN: A case report of fibrocartilaginous embolism (FCE) presenting as acute myelopathy. OBJECTIVES: To illustrate the clinical presentation and magnetic resonance imaging features of FCE. SETTING: Johns Hopkins Transverse Myelitis Center. CASE REPORT: A 16-year-old boy was diagnosed with ischemic myelopathy secondary to FCE 2 years after symptom onset. Diagnosis was delayed because the clinical and radiological characteristics were not recognized initially. After rehabilitation, the patient made a modest recovery. CONCLUSIONS: Diagnosis of FCE can be made by recognition of the cha

Chondrocalcinosis of the wrist.

Calcium Pyrophosphate Dihydrate Deposition (CPDD) disease has characteristic radiographic features including soft tissue calcification, joint space narrowing, bone sclerosis, subchondral cyst formation without osteophyte formation, and large intraosseous geodes. Triangular fibrocartilage calcification is frequently found and isolated scapho-trapezio-trapezoid (STT) arthritis is specific for CPDD. Distal radio-ulnar (DRUJ), isolated midcarpal joint and piso-triquetral joint involvement also occur. 127 patients were reviewed. Seventy-eight had symptomatic STT joint arthritis, for which 36 underw…

Biomechanical study of the extensor carpi ulnaris as a dynamic wrist stabilizer.

PURPOSE: To investigate the dynamic stabilizing effect of the extensor carpi ulnaris (ECU) on the distal radioulnar joint (DRUJ) and the ulnocarpal joint in a simulated model of triangular fibrocartilage complex (TFCC) injury. METHODS: Using 8 fresh-frozen cadavers, we fixed the humerus and ulna at 90° of elbow flexion, and the radius and hand were allowed to rotate freely around the ulna. Passive mobility was tested by translating the radiocarpal unit relative to the ulna along dorsal-palmar directions. Unit displacement was measured by an electromagnetic tracking device in different forearm…
