Proximal interphalangeal joint arthroplasty: current trends and evidence-based practice.

Arthroplasty of the proximal interphalangeal joint (PIPJ) has evolved since its inception over 60 years ago. This review examines the indications for surgery, highlights the differences in current arthroplasty designs, variances in surgical techniques, clinical controversies, current implant outcomes data and salvage options for the failed implant. Overall, PIPJ implant arthroplasty is a good and reliable option for symptomatic PIPJ degenerative, post-traumatic or inflammatory arthritis given the proper clinical setting. If current techniques for implantation and rehabilitation are followed, p

External fixation of open comminuted fractures of the proximal phalanx.

External fixation is an accepted method of treatment for many injuries, however, only a limited number of reports concern its use in open, comminuted fractures of the hand. Ten patients with eleven fractures of the proximal phalanx were treated using the AO/ASIF small external fixator. Five fractures involved the interphalangeal joint (IPJ), two the metacarpophalangeal joint (MPJ), and four the shaft. Nine fractures were caused by small caliber gunshot wounds, one was due to a power saw injury, and one occurred during a three wheeler accident. The patients with interphalangeal joint involvemen

The mechanism of dorsal fracture dislocation of the fifth carpometacarpal joint.

Fracture dislocation at the base of the fifth metacarpal is a rare injury. Fewer than 25 cases have been published in the English-language literature. A force acting along the longitudinal axis of the fifth metacarpal causes a fracture dislocation at the base of the bone in a dorsal-ulnar direction, because of the anatomy of the joint and the mechanical properties of the bone and the intermetacarpal ligament. The mechanism of other types of injury to this joint, namely dorsal-radial or palmar-radial dislocation of the base of the fifth metacarpal is different. A case is presented which demonst

Locking of the metacarpophalangeal joint as a result of the shape of the metacarpal head in achondroplasia.

A 21 year-old woman with achondroplasia exhibited locking of the metacarpophalangeal joint of the right index finger which required surgery to release the joint. Locking was confirmed to be due to entrapment of the fan-like part of the radial collateral ligament on a metacarpal head prominence arising from epiphysial thickening.

Elbow Motion Induces Greater Median Nerve Excursion and Lower Shear Strain Than Wrist or Finger Motion in Healthy Volunteers.

INTRODUCTION/AIMS: Previous ultrasound (US)-based assessments of median nerve (MN) displacement within the carpal tunnel have shown inconsistent results due to methodological variability. Quantitative data on how different upper-limb movements affect MN displacement and shear strain at the wrist remain scarce. This study aimed to quantify MN longitudinal displacement and shear strain during finger, wrist, and elbow movements in healthy individuals to establish normative patterns of nerve gliding and deformation. METHODS: Twenty healthy subjects (13 females; mean age: 31.9 years, range: 27-36

Progressive Pseudorheumatoid Dysplasia or JIA?

Progressive pseudorheumatoid dysplasia (PPD) or spondyloepiphyseal dysplasia tarda with progressive arthropathy (SEDT-PA) is a rare arthropathy of childhood involving the axial skeleton as well as small peripheral joints. A 10-year-old boy was referred by a general practitioner with pain and deformity in the fingers of hands and limping gait. There was no joint synovitis although the finger joints were bulky on examination with mild flexion deformity. Patient had exaggerated kyphosis and lumbar lordosis with pigeon chest and restricted hip joint movements. Anteroposterior X-rays of the hip joi…

Central slip defect reconstruction utilizing partial ulnar side of flexor digitorum superficial tendon for chronic boutonniere deformity: A case report.

BACKGROUND: Post-traumatic central slip injuries, resulting in boutonniere deformity, are a complex issue that can significantly impact hand function and quality of life. The deformity should be easily reducible in the acute phase, but if left untreated, it shortens the oblique retinacular ligament, leading to chronic contracture. This is a challenging issue in hand surgery, as chronic central slip defects cannot be sutured like other tendon. Various methods for reconstructing central slips have been discussed, but the best method for subacute or chronic injuries remains unclear. This case pre

Extra-articular subcutaneous "inverted king post-truss" ligament reconstruction for severe swan neck deformity (snapping finger).

A swan neck deformity (SND) can be well tolerated for a long time, until the appearance of a disabling "snapping finger". In its most advanced condition, the other hand is needed to initiate finger flexion. We propose a technique of extra-articular, subcutaneous ligament reconstruction with an "inverted king post-truss" configuration use in roofs and to reinforce railway bridges. An artificial ligament (MaxBraid™ polyethylene surgical suture, 5 metric, Biomet) makes a figure of eight between transosseous tunnels in the proximal and middle phalanges, crossing over top of the A3 pulley. Salter-Harris type III dorsal fracture-dislocation of the second and third proximal interphalangeal joints: a case report.

BACKGROUND: Paediatric fracture-dislocation of the second (2nd) or third (3rd) proximal interphalangeal (PIP) joint is a rare injury and often associated with growth plate injury. There are few case reports regarding this uncommon injury, and no study has reported on Salter-Harris (SH) -type III injuries. CASE PRESENTATION: We present the case of a 6-year-old softball player who suffered from irreducible SH-type III fracture-dislocations of the 2nd and 3rd PIP joints at different time points, which were treated by open reduction without internal fixation. The injury mechanism for the fracture

Hemi-hamate Arthroplasty for Base of Middle Phalanx Fracture: A Systematic Review.

Intra-articular fracture dislocations of the base of the middle phalanx are complex and debilitating injuries that present a management conundrum when nonreconstructable. Hemi-hamate arthroplasty (HHA) is a treatment modality of particular use in the setting of highly comminuted fractures. This systematic review aims to summarize the reported outcomes of HHA in this context. A literature search was conducted using MEDLINE, Embase, and PubMed, yielding 22 studies with 235 patients for inclusion. The weighted mean postoperative range of movement at the proximal interphalangeal joint was 74.3° (r

Results of proximal interphalangeal arthroplasty: border digits versus middle digits.

This investigation assessed 106 consecutive primary proximal interphalangeal joint arthroplasties performed on border digits: 73 index or 33 little fingers. This was compared with 193 arthroplasties performed in non-border digits: 121 middle or 72 ring fingers. There were 20 proximal interphalangeal joint arthroplasties in the border digits that required revision surgery for pain and stiffness (10 digits), dislocation (six digits), implant fracture (one digit), and infection (three digits). Risk of revision surgery was not associated with border digit. The 5-year implant survival rate for the

Reconstruction of Traumatic Central Slip Injuries: Technique Using a Slip of Flexor Digitorum Superficialis.

Multiple reconstruction techniques have been described for correction of boutonniere deformities including direct repair, central slip reconstruction, lateral band reconstruction, transverse retinacular ligament reconstruction, staged reconstruction, and extensor tenotomy. Each technique has been reported to have variable results with complications including capsular contracture, loss of proximal interphalangeal flexion, and residual deformity. We describe a surgical technique for central slip reconstruction using a slip of the flexor digitorum superficialis tendon through a bone tunnel.

Finger flexor motor control patterns during active flexion: an in vivo tendon force study.

An in vivo tendon force measurement system was used to evaluate index finger flexor motor control patterns during active finger flexion. During open carpal tunnel release surgery (N=12) the flexor digitorum profundus (FDP) and flexor digitorum superficilias (FDS) tendons were instrumented with buckle force transducers and participants performed finger flexion at two different wrist angles (0 degrees or 30 degrees ). During finger flexion, there was concurrent change of metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joint angles, but the FDP and FDS tendon force changes were not c

The role of arthroplasty and arthrodesis following trauma to the upper extremity.

Intra-articular fractures in the hand and wrist are common. Prompt treatment with adequate fixation to allow early motion provides the best chance for a successful outcome. Arthrodesis and arthroplasty are the two main options for treatment of residual pain, deformity, or instability in the hand. Management of late wrist symptoms may include proximal row carpectomy, limited wrist fusion, or complete wrist fusion.
