[The impossible compromise. Apropos of a paper on ballistic injuries by Messrs. Cooper and Ryan].

This paper analyzes a publication of wound ballistic from MM. Cooper and Ryan published in British Journal of Surgery 1990, vol. 77 June pp. 606-610 which call "Interaction of penetrating missiles with tissues: some common misapprehensions and implications for wound management". MM. Cooper and Ryan describe interaction of penetrating missiles with tissues, they denounce that they call common misapprehensions in wound ballistic and they try to reconcile engineers works and clinical observations. As a result of this synthesis, a management of war injuries is recommended. Authors of this present

[Prevalence of symptoms of posttraumatic stress disease in police officers in relation to job-specific requirements].

The professional activity of policemen is connected to psychological and physical stress. The aim of this study was to identify the work specific requirements and stress as well as to deter-mine the exact circumstances of the experience of traumatic events and their coping. Additionally the occurrence of potentially traumatising situations during operations and the prevalence of PTSD symptoms were investigated in detail using questionnaires. As traumatising events severe traffic accidents, use of firearms, but also non-specific operations resulting in death, e. g. the killing of a child, were …

[Management of penetrating abdominal trauma: what we need to know?].

Penetrating traumas are rare in France and mainly due to stabbing. Knives are less lethal than firearms. The initial clinical assessment is the cornerstone of hospital care. It remains a priority and can quickly lead to a surgical treatment first. Urgent surgical indications are hemorrhagic shock, evisceration and peritonitis. Dying patients should be immediately taken to the operating room for rescue laparotomy or thoracotomy. Ultrasonography and chest radiography are performed before damage control surgery for hemodynamic unstable critical patients. Stable patients are scanned by CT and in s…

Gunshot wounds of the temporal bone: a rational approach to evaluation and management.

Gunshot injuries of the temporal bone are uncommon but are being seen more frequently with the increasing incidence of violent crimes and use of firearms. A retrospective analysis of 43 consecutive patients treated at our institution with this injury was undertaken to determine the clinical symptoms, associated injuries, and treatment outcome. This is the largest series from a single institution treated by a single group during a brief period of time. Most of these patients had injury to various structures within and around the temporal bone, including hearing loss (29), intracranial injury (2…

The geographical pattern of methods of suicide in Belgium: implications for prevention.

The geographical variation of reported suicide among the 43 Belgian districts was analysed for each method of suicide in both sexes and in t … outhern districts: suicidal poisoning by solid and liquid substances (in both sexes) and the use of firearms (in males). Drowning was much more concentrated in the West of the country and jumping was especially frequent in Brussels. Possible explanations and the relevance of these findings for prevention are discussed.

Buckshot colic: utilizing holmium:yag laser for ureteroscopic removal of a bullet fragment within the proximal ureter.

Buckshot colic is a rare phenomenon, presenting as firearm-induced urinary tract obstruction. We present a case of gunshot-induced ureteral obstruction in a 49-year-old male, treated endoscopically with the holmium:YAG (holmium) laser. CT revealed a 1 cm bullet fragment within the left proximal ureter. A percutaneous nephrolithotomy was performed utilizing the holmium laser to fragment the metal into basket-retrievable pieces. At 4 month follow up the patient is without evidence of stricture. To our knowledge, this is the first reported utilization of the holmium laser to treat "buckshot colic

Gunshot injuries to the temporal bone.

The current incidence of missile injury to the temporal bone (MITB) is very low in the United Kingdom. However, the increasing frequency of firearm violence in Britain suggests a greater risk of occurrence. This, along with the devastating potential sequelae of MITB (facial palsy, dead ear, intracranial damage, major vascular injury and cosmetic disfigurement), requires otolaryngologists to be conversant with all aspects of their management. The risk of major complications is much higher with MITB than with temporal bone injury following blunt trauma, and surgical management is, therefore, muc

Suicides in children and early adolescents: a retrospective review from a large metropolitan area.

Suicide in individuals younger than 14 years remains an underrecognized and understudied phenomenon in forensic pathology, despite increasing incidence and substantial medicolegal implications. Although children are often presumed incapable of understanding death or engaging in intentional self-harm, psychiatric evidence indicates that by age 8 many possess a conceptual understanding of death and suicidal behavior. This retrospective study examines suicidal deaths in subjects under 14 years of age investigated at a large urban medical examiner's office, emphasizing autopsy findings and medicol

Paediatric ballistic fracture patients: who has poor follow-up and why?

PURPOSE: Firearm-related injuries in children and adolescents have increased over the past decade. The standard of care for ballistic fractures in children is complex, resulting in a burden of healthcare follow-up that many families find challenging. Consistent follow-up is crucial, especially in orthopaedic trauma and firearm cases, to prevent complications. This study aims to identify demographic and clinical variables associated with loss to follow-up (LTFU) in paediatric patients with ballistic fractures. METHODS: This is a retrospective registry study at a Level I trauma centre for patie

Penetrating Brain Trauma due to Air Gun Shot - a Case Report.

Cases of severe injuries caused by air guns are really uncommon, but they can lead to a fatal outcome. Usually, these injuries occur in children due to their not fully developed skull bones or in adults through less resistant and thin regions of the cranium. Most of them are accidental events, followed by low percentage of suicidal attempts. In this paper, we present the case of a 68-year-old man suffering from severe depression, with self-inflicted air gun injury to the head. The patient was admitted to the hospital in a coma with a Glasgow Coma Scale (GCS) score of 8, with severe penetrating

School shootings and mental health outcomes: a scoping review.

OBJECTIVE: Recent data indicate a rising frequency of school shootings over the past two decades. Although these events are relatively rare within the broader context of firearm violence, more than 357 000 students have been exposed to firearm violence in primary and secondary schools since 1999. However, the psychological impacts remain poorly understood. This scoping review synthesises existing evidence and highlights gaps for further research. METHODS: A systematic search was conducted across PubMed, PsycINFO, Web of Science and Criminal Justice Abstracts to identify empirical studies on s

Penetrating dural venous sinus injury: a scoping review of clinical presentation, operative management, and prognosis.

Penetrating injury to the dural venous sinuses (DVS) commonly occurs at the superior sagittal sinus (SSS), transverse sinus (TS), and torcular confluence. While rare, DVS penetrating injuries carry high risk of intracranial hemorrhage, air embolism, and venous infarction. The current literature pertaining to penetrating DVS injury consists primarily of isolated case reports and small series. This scoping review consolidates published cases to characterize mechanisms, management strategies, and clinical outcomes pertaining to penetrating DVS injury.
