Treatment of Clubfoot Using the Ponseti Method.

The Ponseti method consists of a specific technique of manipulation of the clubfoot deformity, followed by the application of a plaster cast with the foot in the corrected position. A percutaneous tenotomy of the Achilles tendon is done prior to the final cast to gain complete correction in most patients. Bracing with a foot abduction orthosis is necessary to minimize relapse of the deformity. The method begins with the Ponseti manipulation and consists of the following steps: (1) Identify the head of the talus by palpation. (2) Supinate the forefoot to eliminate the cavus deformity and create

[The foot in hereditary motor and sensory neuropathies in children].

PURPOSE: Although Charcot-Marie-Tooth disease (CMT) is known to be the most common neuromuscular cause of pes cavovarus, other paralytic deformities of the foot may be present with hereditary motor and sensory neuropathies (HMSN). The purpose of our review is to analyze these foot deformities and to assess the results of the different therapeutic methods used. MATERIAL AND METHODS: We evaluated 66 patients who had HMSN and had a total of 127 foot deformities. Fifty three patients had CMT, 6 patients Déjerine Sottas disease (DS), and 7 patients had an unclassified HMSN. The average age at diag…

Lower extremity torsional deformities in children: a prospective comparison of two treatment modalities.

A prospective randomized analysis was conducted on children with lower extremity torsional deformities. Its purpose was to define the natural history of these anomalies and to determine the efficacy of treatment with a Denis-Browne splint. The natural history of pediatric lower extremity torsional deformities is to correct toward the mean of the population. The Denis-Browne splint did not alter the natural history of lower extremity torsional deformities as defined by the foot progression angle.

Use of the lapidus bunionectomy in first ray insufficiency.

Surgical management of first ray insufficiency in HAV or hallux limitus is crucial to long-term success. Although technically demanding, the Lapidus fusion serves to address the deformity at its apex and restores normal weight bearing to the foot. This arthrodesis offers predictable, durable results in addressing various first ray deformities and occupies an important place in the foot and ankle surgeon's armamentarium. The authors' technique is described herein for the interest of and evaluation by surgeons performing this procedure. As clinicians recognize the importance of addressing proxim

Footwear characteristics and foot problems in older people.

BACKGROUND: Foot problems are common in older people, however the contribution of incorrectly fitting footwear and heel elevation to the development of foot pain and deformity has not been fully evaluated. OBJECTIVES: To examine the relationship between footwear characteristics and the prevalence of common forefoot problems in older people. METHODS: Presence of foot pain and deformity were identified in 176 people (56 men and 120 women) aged 62-96 years (mean 80.09, SD 6.42) using a questionnaire and clinical assessment. Shoe fit was determined by comparing length, width and area measurement

[Familical cleft foot--a clinical study over 4 generations (author's transl)].

Within this paper a family is being described in which foot deformities are shown over four generations. During two generations cleft foot is found as well as a syndaktylism. The cases are demonstrated clinically, by photographs and also by X-ray. The question of inheritance is being discussed in comparison with the existing literature, also taking into account the coexistence of cleft foot and syndaktylism.

[Split tibialis posterior tendon transfer on peroneus brevis for equinovarus foot in CP children].

Equinovarus deformity of the foot is a result of the muscles imbalance in which inventors of the foot, mostly posterior and anterior tibialis muscle, overpower evertors. In children with cerebral palsy untreated spastic equinovarus deformity may cause severe fixed foot deformity and painful callosities under metatarsal heads and on the lateral side of the foot. The gait pattern becomes less effective and needs more energy. The study group consisted of 154 children with cerebral palsy treated in our Clinic by the multilevel soft tissue surgery. For foot problems 136 children needed surgical int

Talectomy for equinovarus deformity in myelodysplasia.

Nine patients with myelomeningocele (seventeen involved feet) had talectomy for the correction of equinovarus deformity. The age at surgery ranged from one year and eight months to seven years and four months old. The length of follow-up averaged seven years and four months and ranged from twenty-two months to twelve years. Fifteen feet had a good and two had a poor correction of the deformity of the hind part of the foot, the result being directly related to the intraoperative correction of the equinus deformity. The correction of the fore part of the foot was rated as good in eight, fair in

Involvement of foot in patients with spondyloarthritis: Prevalence and clinical features.

BACKGROUND: The purpose of this study was to evaluate the foot involvement in a group of patients with spondyloarthritis in regard to sympto … tion (33%), erosion and reconstruction (50%). Forefoot involvement was found in 18/31 patients. Forefoot deformities were found in 9 patients. Two patients had sausage toe and feet skin abnormalities were observed in 12 patients. At podoscopic examination, 23 patients had abnormal footprints. Foot involvement was more frequent in peripheral spondyloarthritis (p=0. …

Longitudinal change in foot posture in children with cerebral palsy.

PURPOSE: Foot deformities are common in children with cerebral palsy (CP), yet the evolution of such deformities is not well documented. We aimed to observe and analyse changes in foot posture during growth in children with CP. Methods We followed 51 children (16 unilateral, 35 bilateral; 37 Gross Motor Function Classification Scale (GMFCS) I/II, 14 III/IV) aged two to 12 years in this level II, IRB-approved prospective longitudinal study. Data after bony foot corrections were excluded. Outcome measures included coronal plane pressure index (CPPI) and pressure impulses from the heel, medial mi…

Analysis of Insole Geometry and Deformity by Using a Three-Dimensional Image Processing Technique: A Preliminary Study.

BACKGROUND: Accurate representation of the insole geometry is crucial for the development and performance evaluation of foot orthoses designed to redistribute plantar pressure, especially for diabetic patients. METHODS: Considering the limitations in the type of equipment and space available in clinical practices, this study adopted a simple portable three-dimensional (3-D) desktop scanner to evaluate the 3-D geometry of an orthotic insole and the corresponding deformities after the insole has been worn. The shape of the insole structure along horizontal cross sections is defined with 3-D sca

Surgery for equinovarus deformity in Friedreich's ataxia improves mobility and independence.

UNLABELLED: Friedreich's ataxia results in morbidity because of many factors; progressive equinovarus deformity is one of these. We studied … rgery was assessed using subscales of the Barthel index and functional independence measure. Severe foot deformities in which either surgery or botulinum toxin injection was recommended correlated with current age, years since disease onset, and years that the patient required a wheelchair for mobility, but not with the GAA repeat size or age at disease onset. Fun …

Epidemiology of the Charcot foot.

The Charcot foot (osteoarthropathy) is a significant lower extremity complication of diabetes mellitus that can result in significant deformity, ulceration, and subsequent limb loss. A result of even unrecognized trauma to an insensitive foot, continued weight bearing on the injured foot promotes the evolution of the disorder that is often diagnosed only after significant deformity has occurred. Although this entity is considered a rare complication of the diabetic population, it has profound implications for those persons affected. Greater awareness of the frequency, distribution, determinant

Essentials of Deformity Planning.

Learning the essentials of deformity planning is the basis for the treatment of simple and complex deformities. Understanding the planes of deformity, radiographic correlation, and clinical correlations allows the surgeon to treat the condition with deeper knowledge of deformity, leading to improved deformity correction.
