Botulinum toxin is a safe and effective treatment for idiopatic focal axillary and palmar hyperhidrosis, but very few data are reported in the literature on its effect on plantar idiopatic hyperhidrosis. The isolation of Scytalidium hyalinum from the toe nail of a patient from Melbourne is reported. This is the first record of the isolation of this fungus from a clinical site in Australia. A brief history is given of the occurrence of Scytalidium hyalinum and the related fungus, Hendersonula toruloidea, in tinea pedis and tinea unguium in immigrants to the United Kingdom from tropical countries. Attention is drawn to the possible presence of these dermatophyte-like infections in patients in Australia.

The existence of arthritis in cutaneous panarteritis nodosa (CPAN) is controversial. We report a 52-year-old male with chronic destructive arthritis of both knees and palpable purpura in the feet, where the underlying histological finding was necrotizing arteritis. Systemic involvement was not demonstrated. AntiHBc and antiHBs antibodies were positive. Four of the 11 cases of CPAN with arthritis that we have found reported in the literature were well documented, and only one developed erosions of the joints. Chemotherapy for chronic fungal infections is often ineffective, and the associated delay in surgical treatment may result in radical procedures being performed to effect a cure. The author describes four patients with chronic fungal lesions, three involving a foot and one the abdominal wall, all of which failed to respond to antifungal chemotherapy. A deep sclerotic process developed on the shins of a 58-year-old man, and eosinophilic fasciitis or morphea profunda was suspected clinically.
