The effects of a new designed forearm orthosis in treatment of lateral epicondylitis.

PURPOSE: This paper reports on the design and testing of a new designed forearm orthosis and explores its efficacious in comparison to the standard counterforce orthosis in patients with lateral epicondylitis. METHOD: Twenty-four patients were enrolled in this assessor-blinded clinical trial and randomly assigned to two parallel treatment groups. The measures of pain and function, the pain threshold and grip strength were compared using patient rated tennis elbow evaluation (PRTEE) form, algometer and dynamometer respectively at baseline and 4 weeks after treatment. Paired and independent t-t…

Eleven years experience in the operative management of pediatric forearm fractures.

BACKGROUND: There has been a trend toward operative management of pediatric diaphyseal forearm fractures (DFFx). We studied our experience with surgical management of these injuries to assess indications, frequency, outcome, and complications. METHODS: One hundred forty-four consecutive children had surgical management of 149 DFFx over 11 years at our Level 1 pediatric trauma center. A chart/radiographic review established perioperative events, intraoperative findings, time-to-union, range-of-motion, and complications. We developed the Children's Hospital of Philadelphia Forearm Fracture Fixa…

Reduction of bowing forearm fractures in the ED under propofol sedation.

Bowing fractures of the long bones result from a deforming force that bends the bone similar to an archer's bow. In some of the earlier descriptions of bowing fractures, the recommended treatment for these was closed reduction under general anesthesia in the operating room. The aim of this study was to describe 2 pediatric patients with bowing fractures of the forearm that were successfully reduced in the emergency department (ED) under deep sedation with propofol. Two cases of bowing fractures of the forearm reduced in the ED under propofol sedation were described. Some bowing fractures of th…

Liposarcoma of the forearm in a man with type 1 neurofibromatosis: a case report.

INTRODUCTION: The combination of neurofibromatosis and liposarcoma is very rare. We present a case of a dedifferentiated liposarcoma in the forearm, as a complication in a patient with neurofibromatosis type 1. CASE PRESENTATION: A Caucasian man with neurofibromatosis type 1 presented at our clinic complaining of a slow growing swelling on his left forearm over a period of one and a half years. Clinical examination and history pointed to malignancy. Radiological examination inclusive of magnetic resonance imaging and positron emission tomography confirmed our suspicion. A final diagnosis of d…

[Ultrasound diagnosis of forearm fractures in children: a prospective multicenter study].

BACKGROUND: Metaphyseal forearm fractures are a common pathology in any emergency department. The standard diagnostic procedure involves X-rays of the forearm and wrist. As former studies have shown that these fractures can be visualized using ultrasound, we compared the accuracy of sonographic and X-ray diagnostics. METHODS: From January 2007 to May 2008, a total of 93 patients aged between 0 and 12 years with forearm trauma were initially examined using a 7.5-Mhz linear transducer. After diagnosis, axis deviation and treatment were fixed and standard X-rays were taken. The results of the tw…

[Assessment of surgical and conservative treatment of forearm fractures: results in juveniles].

BACKGROUND: We aimed to assess treatment outcomes of forearm fractures in patients aged between 10-18 years, for whom strict guidelines for surgical and conservative treatment are not yet clear. METHODS: From a total of 41 patients between the ages of 10-18 years, 34 patients with a minimum of one-year follow-up in our clinic with the diagnosis of forearm fracture were evaluated retrospectively. All the patients had open epiphyses. Twenty-three of these patients were treated conservatively and 11 surgically. Patients were evaluated according to time of union, function and cosmesis. Average fo…

Comparative analysis of electromyographic pattern in the forearm muscles of hemiplegic patients.

It was purposed the use of electromyography (EMG) to evaluate the activation of the agonists and antagonists muscles of spastic patients, to test the viability in the development of an instrument that given quantitative data of the patient spasticity. 30 hemiplegic and 15 normal volunteers had been submitted to the EMG of flexor and extensor carpi ulnaris muscles during the flexion and extension movements of the wrist. The individuals with less severe spasticity (mAS (modified Ashworth Scale) ringing 0 to 3 degree), had presented deficit in the activation of the flexor muscles in plegic side i

[Dislocation fractures of the forearm. Galeazzi, Monteggia, and Essex-Lopresti injuries].

Combined forearm fractures are identified according to their location as Galeazzi, Monteggia, or Essex-Lopresti injuries. The feature common to these three forms is the combination of a forearm fracture with instability of the distal or proximal radioulnar joint. Appropriate management of the injury at an early stage is indispensable to achieve good functional results. Galeazzi fractures should initially be treated by open reduction and correct anatomy restored by plate osteosynthesis. Fixation of the distal radioulnar joint with Kirschner wires should be performed in cases of persistent dislo…

Reconstruction of the brachial artery in supracondylar humerus fractures and forearm dislocations.

Presented herein is an analysis of the short- and long-term results of the treatment of brachial artery injuries in 25 patients with extension supracondylar humerus fractures and anterior forearm dislocations. The cause of arterial injury was a blunt trauma in 11 patients and contused-crushed wounds in 12 cases. In two patients, iatrogenic injury of the brachial artery occurred after reposition of humerus fractures in other medical institutions. Fourteen patients were admitted to the Department for the first 6 hours from the time of injury, 5 after up to 12 hours, and 6 patients after 12 hours…

[The reverse forearm flap pedicled with the ulnar branch of the ulnar artery].

OBJECTIVE: To introduce a new flap for the repair of soft tissue defects in the hand. METHODS: The distally pedicled ulnar flap is based on the ascending branch of the upper-wrist cutaneous branch of the ulnar artery. The retrograde flow is ensured by the dorsal carpal arch via the descending branch. RESULTS: From 2000 to 2003, the flap was used to treat 8 cases of hand defects, including 3 dorsal defects, 2 palmar defects and 3 the first web defects. All flaps survived and the appearance was satisfactory. CONCLUSIONS: The reverse forearm flap pedicled with the ulnar branch of the ulnar art…

Nonbridging external fixation in the treatment of unstable fractures of the distal forearm.

INTRODUCTION: Unstable fractures of the distal forearm often require surgical treatment to restore the normal anatomy and function. We have used a relatively new technique, nonbridging external fixation, in the treatment of these fractures in our hospital during the past few years. Our results are presented here. MATERIALS AND METHODS: Fifty-two patients (41 female, 11 male) with an unstable fracture of the distal forearm were treated using nonbridging external fixation at Oulu University Hospital during 1996-1999. The patients' mean age was 57 years. There were 45 Colles-type fractures, and …

The indications and outcomes in the use of osteocutaneous radial forearm free flap.

INTRODUCTION: Whether secondary to cancer surgery ablation or trauma, surgeons are faced with defects of the mandible or maxilla that would be best reconstructed with a thin, pliable soft tissue component and vascularized bone. A subset of these challenging wounds do not require the bicortical bone necessary to reestablish structural integrity or to retain a dental prosthesis, because the soft tissue needs are more critical than the bony needs. It is this niche that the radial forearm osteofaciocutaneous free flap (RFOFF) fulfills well. In the past, potential and real donor site morbidity has …

Evaluation of the forearm in untreated adult subjects with multiple hereditary osteochondromatosis.

BACKGROUND: Limb-length discrepancy or angular deformities as a result of altered bone growth may lead to a decreased range of motion and impaired function as well as premature osteoarthritis in patients with multiple hereditary osteochondromatosis. The purpose of this study was to describe the function of the forearm in untreated patients in order to facilitate comparison with studies of the results of treatment of this condition. METHODS: The medical records of fifty-one pediatric patients were identified and served as the basis for identifying a cohort of adult relatives with the disorder.…

Secondary osteoporosis and the risk of distal forearm fractures in men and women.

Secondary osteoporosis plays an important role in the pathogenesis of hip and spine fractures, but relatively little is known about the potential impact of secondary osteoporosis and fall-related disorders on the risk of distal forearm fractures. To address this issue, we conducted a population-based, nested case-control study comparing 496 Rochester, Minnesota, residents with an initial distal forearm fracture to an equal number of age- and gender-matched controls. Potential risk factors were assessed by review of each subject's complete (inpatient and outpatient) medical records in the commu…

[Defect coverage of the hand and forearm with a free scapula-parascapula flap].

BACKGROUND: Complex defects of the forearm and hand are associated with the loss of important structures. Single-stage reconstruction of these defects requires composite tissue transplantations. The subscapular region offers various components for the coverage of complex defects. The scapular and the parascapular flaps can be used each as cutaneous, fasciocutaneous and osteocutaneous or as a combined flap as well. The purpose of this study was to present our experience with the combined scapular-parascapular free flap for defect coverage of the forearm and hand in 13 patients. PATIENTS AND ME…

Enhanced cutaneous lymphatic network in the forearms of women with postmastectomy oedema.

Postmastectomy oedema (PMO) of the arm is a common aftermath of axillary lymphatic damage during treatment for breast cancer. The aim of the present study was to quantify the forearm dermal lymphatic capillaries in order to determine whether they exhibit adaptive responses to PMO. Both forearms were examined by fluorescence microlymphography in 16 patients with oedema following treatment for breast cancer (mean swelling 25 +/- 4%) and 19 patients treated for breast cancer but without oedema. Delineated lymphatic networks were analysed stereologically. The main findings were: (1) lymphatic dens…

The effect of alendronate on bone mass after distal forearm fracture.

Fracture and immobilization of an extremity lead to bone loss at the fracture and at adjacent sites. We conducted a 1-year, single-center, prospective, randomized, double-blind study to determine whether bone loss would occur in the distal radius after a Colles' fracture and whether this loss could be prevented using an antiresorptive drug (alendronate). Thirty-seven women with a recent fracture of the distal forearm and low bone mineral density (BMD) of the lumbar spine were randomized to receive either 10 mg alendronate daily or placebo. BMD of both forearms was measured at baseline and afte…
