Proximal Femoral Bone Cyst After Single Screw Fixation for Slipped Capital Femoral Epiphysis: A Case Report.

CASE: An 11-year-old female patient underwent bilateral in situ fixation for slipped capital femoral epiphyses using single, cannulated, stainless steel screws. She presented 12 years later with a large osteolytic lesion of the proximal femur, which only involved 1 side. Histological evaluation showed a foreign-body reaction and synovial lining. Infection was ruled out and dynamic hip screw stabilization, and bone grafting were performed. The bone graft healed, and the hardware was removed to prevent a similar reaction. In retrospect, postoperative radiographs at 7 weeks showed subtle osteolys…

Silicone Breast Implants: A Rare Cause of Pleural Effusion.

Pleural effusions are one of the rarest complications reported in patients with silicone gel filled breast implants. The silicone implants have potential to provoke chronic inflammation of pleura and subsequent pulmonary complications such as pleural effusion. Herein, we report a 44-year-old female who presented with left sided pleural effusion, six weeks after a silicone breast implantation surgery. The most common infectious, inflammatory, and malignant causes of pleural effusion were excluded with pleural fluid cytology and cultures. With recurrent effusion in the setting of recent surgery,

Focal uptake of Tc-99m MDP in a gossypiboma.

A 51-year-old Korean woman with a history of breast cancer underwent screening bone scintigraphy, which revealed minimal soft tissue uptake of Tc-99m methylene diphosphonate (MDP) in the right lower quadrant. CT and plain films confirmed that the uptake was due to a retained surgical sponge or gossypiboma. The incidence of gossypibomas has been reported at high as 1 in 1000 to 15,000 intra-abdominal operations. The natural progression of an aseptic gossypiboma is a foreign body reaction and granuloma formation. This inflammatory granulomatous reaction is the most likely cause of the extraosseo

Postoperative inflammatory reaction developing focal but severe brain edema. A possible complication of topical application of Biobond-soaked oxycellulose.

A 66-year-old female underwent uneventful removal of parasagittal meningioma. At surgery, a piece of Biobond-soaked oxycellulose was applied to the lateral wall of the superior sagittal sinus for hemostasis. Her early postoperative course was complicated by focal but severe brain edema, which was adjacent to the hemostatic agent. Unlike foreign body granuloma previously reported, this complication was considered to be attributed to inflammatory reaction of Biobond, because of the early onset and fulminant edema despite of small volume of the mass lesion. Although there have been no previous re

Effects of lymph stasis on healing of rat intestinal anastomosis.

The effects of lymphatic drainage on ileal anastomotic healing using interrupted polyglycolic acid sutures were studied in rats after division and obstruction of celiac-mesenteric lymphatics, and the data compared with sham-operated controls. In 4 of 20 rats with lymph stasis, but not sham-controls, anastomotic leakage was associated with generalized peritonitis and death. Histologic examination of the anastomotic site at 7 and 14 days revealed prolonged exudation with acute inflammatory reaction and less prominent granuloma formation where lymphatics were interrupted. Whereas foreign body gia

Calcium oxalate deposition in growing bone: anatomical and radiological study in a case of primary oxalosis.

A combined radiological, histological, and crystallographical study of bone, primarily the inferior femur, taken at autopsy from a 9 year old female with primary oxalosis, is reported. The essential feature is the deposition of calcium oxalate monohydrate (whewellite) crystals, inducing a foreign body reaction and bone remodelling. Synovial and cartilage surfaces are not involved. The changes occur primarily at sites of previous or present growth and seem to be related to the reaction of tissue to the deposition of blood-borne crystals. The radiological changes may be explained in part by the

Pseudofolliculitis barbae with keloids.

Pseudofolliculitis barbae is a common cutaneous infection occurring in the bearded area of the face of black men. The infection is caused by ingrown hairs that produce an inflammatory foreign body reaction characterized by papules and pustules at the point of hair penetration that may result in permanent scarring, usually in grooved patterns, and occasionally in keloids. The purpose of this article is to describe the successful treatment of a patient with pseudofolliculitis with keloids that was based on the pathophysiology of the disease and the biology of wound repair.

Poly-ε-Caprolactone-Hydroxyapatite-Alumina (PCL-HA-α-Al2O3) Electrospun Nanofibers in Wistar Rats.

Biodegradable polymers of natural origin are ideal for the development of processes in tissue engineering due to their immunogenic potential and ability to interact with living tissues. However, some synthetic polymers have been developed in recent years for use in tissue engineering, such as Poly-ε-caprolactone. The nanotechnology and the electrospinning process are perceived to produce biomaterials in the form of nanofibers with diverse unique properties. Biocompatibility tests of poly-ε-caprolactone nanofibers embedded with hydroxyapatite and alumina nanoparticles manufactured by means of t

Cutaneous Granulomatosis: a Comprehensive Review.

Cutaneous granulomatosis is a heterogeneous group of diseases, characterized by a skin inflammatory reaction triggered by a wide variety of stimuli, including infections, foreign bodies, malignancy, metabolites, and chemicals. From a pathogenic point of view, they are divided into non-infectious and infectious granulomas. Pathophysiological mechanisms are still poorly understood. Non-infectious granulomatous skin diseases include granuloma annulare, necrobiosis lipoidica, rheumatic nodules, foreign body granulomas, cutaneous sarcoidosis, and interstitial granulomatous dermatitis. Necrobiosis l

Soft Pillows, Hard Consequences: A Case Report Unveiling a Down Feather Neck Granuloma in a Pediatric Patient.

Down pillows, fitted with soft feathers, are commonly used among parents for their children due to their comfort, breathability, and warmth. In this case report, we present a 17-month-old boy who presented with a chief complaint of fever and progressive left neck swelling warranting a surgical excision after a failed course of antibiotic therapy. Histopathologic examination of the lesion demonstrated down feathers with associated granuloma with foreign-body type giant cell reaction, attributed to the use of the down pillow.

Endoscopic Removal of Huge Cholesterol Granuloma in the Maxillary Sinus Confused With Odontogenic Keratocyst.

Cholesterol granuloma is a foreign body reaction to the deposition of cholesterol crystals, usually found in association to chronic middle ear diseases, being highly uncommon in the paranasal sinuses. Furthermore, a huge and aggressive cholesterol granuloma involving the maxillary sinus, hard palate, buccal space, and maxillary alveolus is extremely rare and has not been reported previously. This article reports a case of huge cholesterol granuloma in the maxillary sinus confused with an expansile odontogenic keratocyst, which was treated successfully via transnasal endoscopic approach.

Synthetic fibre granuloma of the conjunctiva.

Synthetic fibre granuloma of the conjunctiva, sometimes known as 'teddy bear granuloma', results from granulomatous foreign body reaction of the conjunctiva to synthetic fibres. It is often an incidental finding, most commonly found in children, is unilateral, and occurs in the lower eyelid. We present here, what we believe is the first reported case of synthetic fibre conjunctival granuloma in Hong Kong, together with a review of the condition. An awareness of this clinical entity allows early and accurate diagnosis and early treatment.

Bee sting of the cornea: case report.

Three cases of corneal bee sting were treated. Each eye reacted differently from the others: case 1 with severe pain, conjunctival chemosis and hyperemia, central corneal edema, and reduced visual acuity; case 2 with severe pain, conjunctival hyperemia, and a white corneal infiltrate; and case 3 with only a minimal foreign-body sensation. The reaction to the venom can be toxic or immunologic. The behavior of the toxins-melittin, apamin, and formic acid-and of the principal allergen (probably phospholipase A) are described.

Cerebral coenurosis. Report of a case with arteritis.

A case of the unusual clinical condition of cerebral coenurosis, the unique parasitic disease that is localized only in the CNS, is reported. The patient had repeated attacks of transient hemiparesis due to intracranial arteritis which was demonstrated by several angiographic studies. The CSF showed a discreet lymphocytosis and increased immunoglobulins. This inflammatory reaction which preceeded the manifestations of the intracranial mass can be explained by the parasitic foreign bodies, the liberation of somatic and metabolic parasitic toxins and the immunological reaction of the organism. Pseudofolliculitis barbae: review and update on new treatment modalities.

Pseudofolliculits barbae, PFB, is a common cutaneous disease encountered frequently in medical practice. PFB represents a chronic inflammatory condition of the hair follicle caused by ingrown hairs producing an inflammatory foreign body reaction. The pathogenesis of PFB is multifactorial. Factors such as hair type and direction of hair growth play a role in the initial inflammatory reaction. In the armed forces, PFB represents a real challenge for both the physician and the patient. The combat environment, with the recent threat of biological and chemical weapons, requires the servicemen to be

Giant papillary conjunctivitis associated with elevated corneal deposits.

A patient presented with central corneal scarring and neovascularization associated with elevated deposits that were shown to be keratin and calcium. Giant papillary conjunctivitis (GPC) was noted at a corresponding location in the palpebral conjunctiva. The lid reaction resolved after the elevated corneal deposits were debrided. A rigid gas-permeable contact lens was then fitted for visual rehabilitation. Either foreign bodies or elevated corneal deposits may cause GPC.

Silicone Granulomas at the Peripheral Intravenous Cannula Puncture Site.

Different adverse reactions to silicone present in cosmetic fillers, implants, or medical devices have been reported. The granulomatous reaction on the injection site due to catheters' silicone coating is an infrequent complication, which may be clinically difficult to suspect. Although the definitive diagnosis requires identification of the silicone with techniques such as X-ray spectroscopy, the possibility of involvement of silicone, as well as the histopathologic demonstration of granulomas with refractile, nonpolarizable, and nonstainable material strongly suggests this diagnosis. Creation of a rich vascular subcutaneous space for cell transplantation via injectable biological hydrogels.

The subcutaneous space offers an attractive and accessible site for cell transplantation. However, its clinical utility is often hindered by insufficient vascularization. This study evaluated the vascularization potential of three hydrogels; human collagen type I, human fibrin, and alginate implanted subcutaneously in Sprague-Dawley rats. Polylactide-co-glycolide (PLG) scaffolds used as positive controls for foreign body response. Sequential injections were performed at 1, 2, and 4 weeks, and tissues were retrieved for histopathological examination. Human collagen type I and fibrin induced rob

Ibuprofen-loaded bilayer electrospun mesh modulates host response toward promoting full-thickness abdominal wall defect repair.

Pro-inflammatory response impairs the constructive repair of abdominal wall defects after mesh implantation. Electrospinning-aid functionalization has the potential to improve the highly orchestrated response by attenuating the over-activation of foreign body reactions. Herein, we combined poly(L-lactic acid-co-caprolactone) (PLLA-CL) with gelatin proportionally via electrospinning, with Ibuprofen (IBU) incorporation to fabricate a bilayer mesh for the repair improvement. The PLLA-CL/gelatin/IBU (PGI) mesh was characterized in vitro and implanted into the rat model with a full-thickness defect
