BACKGROUND: Both tibial plateau fractures and extensor apparatus injuries are serious injuries to the knee joint that generally do not occur in the same patient. We report a rare case of open tibial plateau fracture combined with quadriceps tendon rupture and complete displacement of the patella into the tibial plateau fracture.

PURPOSE OF THE STUDY: Bifocal proximal and distal avulsion of the patellar tendon is a rare event in adults with, to our knowledge, no previous report in the literature. Exceptional bifocal avulsion of the patellar tendon is generally related to an underlying bone or systemic disease. We report the case of a 90-year-old diabetic patient who developed simultaneous avulsions of the patellar and tibial insertions.

Optic nerve head avulsion is a rare condition. It consists in a rupture of the nerve fibers at the level of the lamina cribrosa followed by a retraction of these fibers into the intact optic sheath. We report a case of optic nerve head avulsion following globe contusion. A four-year-old girl presents at the emergency department with left elbow pain after a fall. X-ray demonstrated an epiphysiolysis of the capitellum humeri, without a bony fragment, the so-called Salter Harris Type 1 fracture. The fracture was treated by open reduction and internal fixation. Low-energy trauma such as sprains account for 75% of injuries to the ankle; therefore, ankle pain is one of the most common problems encountered by emergency physicians. A close inspection of the radiographs is often required for diagnosis and this depends on the knowledge of high-risk areas since proper treatment relies on the accuracy of the initial diagnosis. In this article, we will present a template that has helped to increase the accuracy for detecting subtle avulsion fractures, chip fractures, and osteochondral fractures of the hind foot in adults sustaining low-energy ankle injuries.

Types and causes of renal osteodystrophy in chronic (3-12 years, hemodialysed patients are presented. Tissue weakening due to osteodystrophy resulted in 6 extensor mechanism lesions in 4 patients. Two quadriceps tendon avulsions, 1 patellar fracture, 2 patellar tendon avulsions involving the tip of patella and 1 patellar tendon rupture occurred. Pathomorphology and course of surgical treatment has been discussed. Good functional results have been achieved.

A 19-year-old man with a fracture of the distal humerus avulsed the anterior interosseous nerve. Avulsion of this nerve may be more common than previously believed. As shown by a cadaver study, the mechanism of injury is related to traction on the median nerve. Complete clinical assessment requires an accurate neurological examination; specifically, the integrity of the anterior interosseous nerve is tested, and a characteristic disturbance of pinch is identified. By treatment with a free nerve graft it is possible to achieve full functional recovery.

We report on an 8-year-old child with avulsions of the left L3, L4 and L5 nerve roots and traumatic meningoceles that were not associated with lumbar spine or pelvic girdle fractures. The patient had a history of a road traffic accident. Plain radiographs of the pelvis revealed left hip dysplasia. The magnetic resonance imaging findings of the lumbar spine are illustrated. An avulsion fracture of the lesser trochanter in the absence of known or appropriate antecedent trauma was the initial manifestation of unrecognized metastatic malignant disease in the subtrochanteric area of four patients.
