Intraarticular distal humerus fracture complicated by osteogenesis imperfecta treated with primary total elbow arthroplasty: a case report.

Intraarticular fractures of the distal humerus are challenging problems for the treating surgeon. When these fractures are complicated by severe comminution and poor bone quality, open reduction and internal fixation may lead to poor clinical outcomes when compared with those treated with primary total elbow arthroplasty. The population in which this clinical scenario most often arises is the elderly. An unusual case is presented in which a 38-year-old individual was successfully treated with primary total elbow arthroplasty for a highly comminuted, intraarticular distal humerus fracture with

Modified Suzuki frame for the treatment of difficult Rolando fractures.

Fifteen consecutive patients with severely comminuted Rolando fractures were treated by closed reduction and fixation with a modified Suzuki frame without rubber bands, followed by immediate mobilization. All the fractures healed within 5 weeks. At 3 months, no rotational deformity was observed. The Kapandji score was equal that of the contralateral thumb in eight cases. No residual pain was recorded. Grip strength was 78% and pinch strength was 78% of the contralateral hand. One patient needed the frame tension modified. One patient developed a sensory deficit in the area of the superficial b

Displaced olecranon fractures in adults. Clinical evaluation.

Displaced fractures of the olecranon in adults were analyzed for pain, function, range of motion, and roentgenographic appearance. With one exception, the 38 transverse or oblique fractures were treated by reduction and internal fixation. Results were best in the group treated by screw plus wire in combination (average rating, 17.7 points). Other ratings averaged 17.2 for the intramedullary screw, 16.8 for figure-of-eight wire, and 16.7 for Arbeitsgemeinschaft fur osteosynthesefragen (AO) tension-band wire. Factors associated with poorer results included postoperative displacement or malreduct

Intrinsic and extrinsic risk factors for nonunion after nonoperative treatment of midshaft clavicle fractures.

BACKGROUND: The optimal treatment of midshaft clavicle fractures remains controversial. Nonunion is usually considered to be an uncommon complication following a nonoperatively treated clavicle fracture. HYPOTHESIS: Not every midshaft clavicular fractures shares the same risk of developing nonunion after nonoperative treatment. The present study was performed to identify the intrinsic and extrinsic independent factors that are independently predictive of nonunion in patients with midshaft clavicular fractures after nonoperative treatment. MATERIALS AND METHODS: We performed a retrospective s

A Unique Presentation of the Glenoid, Coracoid, and Proximal Humerus Fractures.

Fractures of the proximal humerus are common injuries with a bimodal age distribution. They usually present in younger patients after high-energy trauma and in elderly patients after lower-energy trauma. Fractures of the proximal humerus are rarely associated with concomitant fractures of the glenoid, and this is a complex injury pattern that indicates the presence of significant instability. Such injuries are usually treated surgically. Even more rarely, patients may present with proximal humerus fractures and fractures of the coracoid process. A male patient presented to our emergency depart

A transcuneiform fracture-dislocation of the midfoot.

Isolated fractures of the ossa cuneiformia are rare fractures of the foot and they usually occur in the context of a direct trauma. We report about a patient with a dislocation of the midfoot and concomitant fractures of the cuneiforme bones I-III due a car traffic accident. X-rays of the foot confirmed a lateral dislocation of the midfoot with multiple fractures of all ossa cuneiformia. An anterior-longitudinal approach was performed to access the ossa cuneiformia and the bases of the metatarsalia. Comminuted fractures of all ossa cuneiformia, a dislocation of the metatarsal bones I-III and a

Two unusual cases of coronal lunate fracture.

Volar lunate fractures are rare injuries, usually seen as a result of high-energy trauma and often in association of other carpal injuries. We present 2 unusual cases of coronal volar lunate fractures. The first case involved a proximal pole scaphoid fracture in association with a volar lunate fracture. During surgical exposure, the lunate fracture was found to be rotated 180 degrees volarly. This displaced lunate fracture, although appearing small on radiographs, consisted of the entire volar half of the capitate facet of the distal lunate. Both the scaphoid and lunate fractures were anatomic

Complex Monteggia Fractures in the Adult Cohort: Injury and Management.

Monteggia fractures involve proximal ulna fracture associated with a radiocapitellar joint dislocation. The Bado classification is primarily based on the direction of the radial head dislocation. The Jupiter subtype classification of Bado II fractures further characterizes the severity of proximal ulna comminution and the involvement of the coronoid fragment. This latter classification can better prognosticate the challenges of surgical reconstruction and clinical outcomes. Surgery for all adult Monteggia fractures is required to restore the anatomic alignment of the ulna, which indirectly red

Acetabular fractures in the elderly: evaluation and management.

Acetabular fracture patterns in the elderly, with increased involvement of the anterior column, quadrilateral plate comminution, medialization of the femoral head, and marginal impaction, differ from those noted among a younger cohort. Poor prognostic factors for open reduction and internal fixation (ORIF) are posterior wall comminution, marginal impaction of the acetabulum, a femoral head impaction fracture, a so-called gull sign, and hip dislocation. The rate of conversion to total hip arthroplasty following formal ORIF has been reported to be 22% at a mean of twenty-nine months. Total hip r

Does Lumbopelvic Fixation Add Stability? A Cadaveric Biomechanical Analysis of an Unstable Pelvic Fracture Model.

OBJECTIVE: We sought to determine the role of lumbopelvic fixation (LPF) in the treatment of zone II sacral fractures with varying levels of sacral comminution combined with anterior pelvic ring (PR) instability. We also sought to determine the proximal extent of LPF necessary for adequate stabilization and the role of LPF in complex sacral fractures when only 1 transiliac-transsacral (TI-TS) screw is feasible. MATERIALS AND METHODS: Fifteen L4 to pelvis fresh-frozen cadaveric specimens were tested intact in flexion-extension (FE) and axial rotation (AR) in a bilateral stance gliding hip mode

A unique indexing splint for use in combined Le Fort and nasal injuries to avoid tracheostomy.

Midface fractures with associated displaced and/or comminuted nasal fractures can require tracheostomy for a general anesthetic airway if these fractures are to be reduced simultaneously. We describe a unique indexing splint that allows oral intubation yet also allows simultaneous reduction of midface and nasal fractures under one anesthetic. Furthermore, tracheostomy with its potential complications is avoided. The case report illustrates the technique, and alternative treatments for these injuries are discussed.

Multifragmentary tibial pilon fractures: midterm results after osteosynthesis with external fixation and multiple lag screws.

Osteosynthesis of intraarticular tibial pilon fractures is preferably achieved using locking plates via a minimally invasive technique. If combined with severe soft tissue damage there is a high risk of wound-healing deficits after plate osteosynthesis. Thus our aim was to find an alternative method of treatment for those cases with combined soft tissue injuries. We report on five cases with comminuted fractures of the joint surface combined with critical soft tissue condition that were treated with lag screws and external fixation (AO) applied across the ankle joint. All five patients were fol

Effects of input energy and impactor shape on cranial fracture patterns.

This study documents relationships between input energy, impactor shape, and the formation of fractures in human crania. Parietal impact experiments (n = 12) were performed at 67% higher input energy compared to previously reported experiments. Fracture origins, characteristics, and locations were compared at two input energy levels with three impactor shapes (focal "hammer", flat "brick", and curved "bat"). Impacts with all three impactors at both energy levels produced fractures originating at and remote to the impact site, indicating both mechanisms are typical in temporoparietal blunt forc

Repairing Facial Fractures with Interrupted Maxillary-mandibular Arches by Computer-assisted Reverse Planning Model Surgery.

BACKGROUND: Management of comminuted facial fractures with maxillary-mandibular arch interruption is difficult, resulting in inadequate bone reduction and malocclusion. Traditionally, a good quality dental splint is helpful, but difficult to obtain in acute trauma. We apply a computer-assisted design and three-dimensional printing technology to improve splint fabrication and utilization, thus facilitating restoration of dental occlusion and facial fracture. METHODS: We retrospectively reviewed patients who suffered from facial fractures with interruption of the maxillary-mandibular arches. We

A Classification of Extra-articular Distal Radius Fractures With Associated Injuries.

PURPOSE: To develop a classification system for extra-articular distal radius fractures (DRFs) integrating radiographic parameters of instability, ulnar fractures, distal radioulnar (DRU) joint disruption, and carpal involvement and to evaluate its reliability. METHODS: A multicenter study across three university-affiliated orthopedic centers identified 1,239 extra-articular DRFs from 3,784 radiographs. Radiographic features were categorized into four domains: (1) DRF instability (radial inclination <15°, radial height loss ≥5 mm, dorsal angulation >15° in dorsally displaced fractures or vola

Techniques for maxillomandibular fixation: old and new.

PURPOSE OF REVIEW: Maxillomandibular fixation (MMF) is essential for many surgeries on the bony middle and lower face. MMF techniques have multiplied in recent years, each with unique benefits and drawbacks. This review catalogs MMF trends and evidence for and against the most prevalent MMF methods. RECENT FINDINGS: Traditional Erich arch bars remain the most robust technique in establishing MMF, particularly for comminuted/complicated fractures. Drawbacks are increased operative time required, wire stick injuries, poor oral hygiene, and gingival trauma. Screw-based techniques save considerab
