Nicotine-induced dystonic arousal complex in a mouse line harboring a human autosomal-dominant nocturnal frontal lobe epilepsy mutation.

We generated a mouse line harboring an autosomal-dominant nocturnal frontal lobe epilepsy (ADNFLE) mutation: the alpha4 nicotinic receptor S248F knock-in strain. In this mouse, modest nicotine doses (1-2 mg/kg) elicit a novel behavior termed the dystonic arousal complex (DAC). The DAC includes stereotypical head movements, body jerking, and forelimb dystonia; these behaviors resemble some core features of ADNFLE. A marked Straub tail is an additional component of the DAC. Similar to attacks in ADNFLE, the DAC can be partially suppressed by the sodium channel blocker carbamazepine or by pre-exp…

Frontal Lobe and Subregional Volumetric Alterations Across Alzheimer's Disease, Amnestic Mild Cognitive Impairment, and Vascular Dementia: An MRI Volumetry Study.

BACKGROUND: Frontal lobe involvement represents an important but heterogeneously expressed feature across neurodegenerative and vascular cognitive disorders. While frontal atrophy has been described in Alzheimer's disease (AD), detailed volumetric assessment of frontal subregions across Alzheimer's disease, amnestic mild cognitive impairment (aMCI), and vascular dementia (VaD) remains insufficiently characterized. The aim of this study was to evaluate frontal lobe and frontal subregional volumetric alterations across these diagnostic groups using automated MRI-based volumetry. METHODS: This c…

Functional reorganization of contralesional networks varies according to isocitrate dehydrogenase 1 mutation status in patients with left frontal lobe glioma.

PURPOSE: The study aimed to assess how isocitrate dehydrogenase 1 (IDH1) mutation status in patients with glioma may alter functional connectivity (FC) in the default mode network (DMN) and fronto-parietal network (FPN). METHODS: Using resting-state functional magnetic resonance imaging, a seed-based FC analysis was employed to investigate connectivity within and between networks in seventeen patients with IDH1-mutant glioma (IDH1-M), eleven patients with IDH1-wildtype glioma (IDH1-WT), and nineteen healthy controls (HC). RESULTS: For FC within the DMN, compared to HC, both IDH1-M and IDH1-W

Frontal lobe hemodynamics detected by functional near-infrared spectroscopy during head-up tilt table tests in patients with electrical burns.

SIGNIFICANCE: Electrical burns can cause severe damage to the nervous system, resulting in autonomic dysfunction with reduced cerebral perfusion. However, few studies have investigated these consequences. AIM: To elucidate changes in prefrontal cerebral hemodynamics using functional near-infrared spectroscopy (fNIRS) during the head-up tilt table test (HUT) for patients with electrical burns. APPROACH: We recruited 17 patients with acute electrical burns within 1 week after their accidents and 10 healthy volunteers. The NIRS parameters acquired using an fNIRS device attached to the forehead

Cortical light scattering during interictal epileptic spikes in frontal lobe epilepsy in children: A fast optical signal and electroencephalographic study.

OBJECTIVE: Interictal epileptic spikes (IES) represent a signature of the transient synchronous and excessive discharge of a large ensemble … ES. These results suggest a relationship between (de)synchronization and neuronal volume changes in frontal lobe epilepsy during IES. SIGNIFICANCE: These changes in the neuronal environment around IES in frontal lobe epilepsy observed in children, as they have been in rats, raise new questions about the synaptic/nonsynaptic mechanisms that propel the neurons …

Schizophrenia, psychotic illness and other psychiatric symptoms in families with autosomal dominant nocturnal frontal lobe epilepsy caused by different mutations.

OBJECTIVES: Autosomal dominant nocturnal frontal lobe epilepsy (ADNFLE) is characterized by a strong family history of epileptic seizures, which predominantly occur during sleep. ADNFLE has been associated with mutations in two genes coding for the nicotinic acetylcholine receptor (CHRNA4 and CHRNB2). Thus far, three different mutations have been detected in the CHRNA4 gene, and two in the CHRNB2 gene. The aim of this study was to compare the frequency of psychiatric disorders in two ADNFLE families with different CHRNA4 mutations (776ins3 and Ser248Phe). METHODS: Information was gathered fro…

Responsive neurostimulation of the frontal lobe for the detection and treatment of seizures in intractable epilepsy due to tuberous sclerosis complex: illustrative case.

BACKGROUND: Responsive neurostimulation (RNS) is often considered to be a palliative therapy for drug-resistant epilepsy (DRE) and is generally not considered to be a treatment for patients with tuberous sclerosis complex (TSC). Here, the authors present the case of a 24-year-old male with TSC who obtained seizure freedom following RNS device implantation. OBSERVATIONS: Prior to RNS device implantation, the patient underwent tuberectomies, subependymal giant cell astrocytoma resection, vagus nerve stimulator placement, and left frontal lobe resection but continued to have frequent seizures. A…

Measurement of glucose metabolism in the occipital lobe and frontal cortex after oral administration of [1-13C]glucose at 9.4 T.

For the first time, labeling effects after oral intake of [1-13C]glucose are observed in the human brain with pure 1H detection at 9.4 T. Spectral time series were acquired using a short-TE 1H MRS MC-semiLASER (Metabolite Cycling semi Localization by Adiabatic SElective Refocusing) sequence in two voxels of 5.4 mL in the frontal cortex and the occipital lobe. High-quality time-courses of [4-13C]glutamate, [4-13C]glutamine, [3-13C]glutamate + glutamine, [2-13C] glutamate+glutamine and [3-13C]aspartate for individual volunteers and additionally, group-averaged time-courses of labeled and non-lab

A Case of Non-Germinomatous Germ Cell Tumors of the Frontal Lobe Arising From the Lateral Ventricle With a Synchronous Pineal Lesion.

Non-germinomatous germ cell tumours (NGGCT) are rare intracranial tumours that account for 1% to 3% of cases. They are usually seen in the pineal and suprasellar regions. NGGCT of the frontal lobe arising from the lateral ventricle with a synchronous pineal lesion is uncommon. We present a case of NGGCT with multifocal lesions in the pineal gland, frontal lobe, and pons treated with chemotherapy followed by craniospinal irradiation (CSI).

Additional observation of a de novo pathogenic variant in KCNT2 leading to epileptic encephalopathy with clinical features of frontal lobe epilepsy.

INTRODUCTION: KCNT2 was recently recognized as a gene associated with neurodevelopmental disorder and epilepsy. CASE REPORT: We present an additional observation of a 16-year-old male patient with a novel de novo KCNT2 likely pathogenic variant and review the five previously reported cases of de novo variants in this gene. DISCUSSION: Whole exome sequencing identified the missense variant c.725C > A p.(Thr242Asn), which was confirmed by Sanger sequencing. Our patient has a refractory stereotyped and monomorphic type of hyperkinetic focal motor seizure, similar to what is seen in frontal lobe…

An unusual association of headache, epilepsy, and late-onset Kleist's pseudodepression syndrome in frontal lobe cavernoma of the cerebral left hemisphere.

Cerebral cavernous angioma or cavernoma is a benign vascular malformation, usually asymptomatic. It is infrequent and often its discovery is incidental, a so-called incidentaloma. However, these lesions can be symptomatic, causing headaches, epilepsy, cerebral hemorrhage and other neurological signs depending on the brain area involved. Frontal localization is responsible for psychiatric disorders, particularly the prefrontal region, leading to prefrontal syndrome, a condition common in all frontal lobe tumors. Psychopathological syndrome can be depression-type, pseudodepression syndrome or ma…

Autosomal dominant nocturnal frontal lobe epilepsy. A video-polysomnographic and genetic appraisal of 40 patients and delineation of the epileptic syndrome.

A number of clinical and aetiological studies have been performed, during the last 30 years, on patients with abnormal nocturnal motor and behavioural phenomena. The aetiological conclusions of these studies were often conflicting, suggesting either an epileptic or a non-epileptic origin. Among the clinical characteristics of these patients, the familial clustering was one thoroughly accepted. A nocturnal familial form of frontal lobe epilepsy (autosomal dominant nocturnal frontal lobe epilepsy, ADNFLE), often misdiagnosed as parasomnia, has been recently described in some families. In one lar…

Evaluation of fetal frontal lobe development and Evans index in pregnancies complicated by gestational diabetes mellitus: prospective comparative observational study.

OBJECTIVE: To assess whether third-trimester fetal frontal lobe biometry and ventricular-cranial proportionality differ between pregnancies with gestational diabetes mellitus (GDM) and normoglycemic controls, and whether these markers vary by treatment (diet vs. insulin) or composite adverse perinatal outcome (CAPO). METHODS: In this single-center prospective observational study, 73 singleton GDM pregnancies and 73 low-risk controls underwent ultrasound at comparable gestational ages (30-38 weeks). On the transventricular plane we measured frontal anteroposterior diameter (FAPD) and occipitof…

Encoding-related hippocampus connectivity for scenes, faces, and words: Healthy people compared to people with temporal and frontal lobe epilepsy.

Interactions of the hippocampus with other brain structures are supposed to support memory formation but knowledge is limited regarding hippocampal task-based functional connectivity (FC) during encoding in both healthy people and people with epilepsy, who frequently have impaired memory. We compared absolute [FC(encoding)] and relative FC (isolating task-specific FC [FC(encoding)-FC(baseline)]) of the anterior hippocampus in 30 controls, 56 mesial temporal (mTLE, 26 right) and 24 frontal lobe epilepsy (FLE) patients using a memory fMRI-task of encoding scenes, faces and words. In controls, ab…

Individual Differences in Older Adult Frontal Lobe Function Relate to Memory and Neural Activity for Self-Relevant and Emotional Content.

OBJECTIVES: Older adults show memory benefits for self-relevant and emotional content, but there are individual differences in this effect. It has been debated whether processing of self-relevant and emotional information relies on similar processes to one another. We examined whether variation in frontal lobe (FL) function among older adults related similarly to the processing of self-relevant information as it did to emotional information, or whether these relations diverged. METHODS: While undergoing fMRI, participants (ages 60-88) viewed positive, negative, and neutral objects, and imagin…

Unraveling the Pathogenesis of Post-Stroke Depression in a Hemorrhagic Mouse Model through Frontal Lobe Circuitry and JAK-STAT Signaling.

Post-stroke depression is a common complication that imposes significant burdens and challenges on patients. The occurrence of depression is often associated with frontal lobe hemorrhage, however, current understanding of the underlying mechanisms remains limited. Here, the pathogenic mechanisms associated with the circuitry connectivity, electrophysiological alterations, and molecular characteristics are investigated related to the frontal lobe in adult male mice following unilateral injection of blood in the medial prefrontal cortex (mPFC). It is demonstrated that depression is a specific ne…
