Influence of fear of falling on gait and balance in Parkinson's disease.

PURPOSE: To study the relationship of fear of falling (FoF) with gait characteristics and balance in individuals with Parkinson's disease (PD). METHOD: Seventy-nine non-demented individuals (62 males) with PD were studied. Their mean age was 69.22 ± 8.93 years. The average time since diagnosis was 8.27 ± 5.31 years. FoF was assessed by the Activities-specific Balance Confidence (ABC) Scale in which high scores indicate less FoF. Gait was measured using a computerized walkway. Balance was measured by timed tests including the 5-step test, 360 degree turn, timed sideways walk, and timed up and …

Effects of treadmill inclination on the gait of children with Down syndrome.

The goal of this study was to analyze the effects of upward treadmill inclination on the gait of children with Down syndrome (DS). Sixteen children with a mean age 8.43 ± 2.25 years, classified at level I of the Gross Motor Function Classification System (GMFCS) and able to walk without personal assistance and/or assistive devices/orthosis were evaluated. Spatial-temporal variables were observed as well as the angular variation of hip, knee and ankle in the sagittal plane, while children walked on the treadmill carried out on 0% and 10% upward inclination. The results showed that children with…

Adult osteomalacia: a treatable cause of "fear of falling" gait.

A 65-year-old man was hospitalized with a gait disorder, obliging him to shuffle laterally(1) (video on the Neurology Web site at www.neurology.org) because of pain and proximal limb weakness. He had a gastrectomy for cancer 7 years previously, with severe vitamin D deficiency; parathormone and alkaline phosphatase were increased, with reduced serum and urine calcium and phosphate. There was reduced bone density (figure). He was mildly hypothyroid and pancytopenic. B12 and folate levels were normal. Investigation for an endocrine neoplasm (CT scan, Octreoscan) was negative. EMG of proximal mus…

Delayed recovery of gait function in a patient with intracerebral haemorrhage.

OBJECTIVE: We report here on a patient with intracerebral haemorrhage (ICH), who showed unusually delayed motor recovery of the leg, which started at 6 months after ICH onset. CASE REPORT: A 53-year-old man underwent craniotomy and drainage for a right putaminal ICH. The patient presented with complete paralysis of the left extremities at ICH onset. Despite having undergone rehabilitation starting at 7 weeks after onset, the patient did not show significant motor recovery of the left extremities until 6 months after onset, when the affected left leg began to show motor recovery to the point t

Effects of preoperative gait analysis on costs and amount of surgery.

BACKGROUND: The purpose of this study was to determine the effects of clinical gait analysis (GA) on the costs of care in ambulatory children with cerebral palsy (CP) and the amount of surgery these children undergo. METHODS: A retrospective review identified all ambulatory patients with CP who had undergone lower extremity orthopaedic surgery at our hospital from 1991 to 2005 with at least a 6-month follow-up. The patients were grouped into those who had undergone GA before their index surgery (GA group, N=313) and those who had not (NGA group, N=149). The groups were compared in terms of th…

Gait and muscle activation changes in men with knee osteoarthritis.

The aim was to examine the biomechanics of level- and stair-walking in men with knee osteoarthritis (OA) at different pre-determined gait speeds and to compare the results with those obtained from healthy control subjects. Special emphasis was placed on the estimation of joint loading. Fifty-four men with knee OA (50-69 years) and 53 healthy age- and sex-matched controls were enrolled in the study. The participants walked barefoot in the laboratory (1.2 m/s+/-5%), corridor (1.2; 1.5 and 1.7 m/s+/-5%), and climbing and coming down stairs (0.5 and 0.8 m/s+/-5%) separately. Joint loading was asse…

Role of attentional resources on gait performance in Huntington's disease.

Patients with Huntington's disease (HD) suffer from cognitive deficits with impaired executive functions, including limited attentional resources. We sought to use a dual-task paradigm to evaluate attentional demands and the ability of patients with HD to concentrate on two tasks simultaneously. We analyzed the interference effects of cognitive and motor tasks on walking in HD and the contribution of clinical symptoms to gait disturbances. Patients and controls were asked to perform either a motor task (carrying a tray with four glasses), a cognitive task (counting backwards), or no task at al…

Spectrum of gait impairments in presymptomatic and symptomatic Huntington's disease.

The purpose of this study was to quantify gait impairments in presymptomatic and symptomatic Huntington's disease (HD) subjects, and examine sensitivity of gait measures. Our sample (n = 65) included presymptomatic mutation carriers (PMC) (n = 15), symptomatic HD subjects (SHD) (n = 30) and healthy controls (n = 20). Participants were requested to walk at their preferred speed on a computerized walkway that recorded spatiotemporal variables. We administered the Unified HD Rating Scale (UHDRS) for PMC and SHD. PMC demonstrated decreased gait velocity (P < 0.01), stride length (P < 0.008), and i…

The influence of focal cerebellar lesions on the control and adaptation of gait.

Cerebellar ataxic gait is influenced greatly by balance disorders, most likely caused by lesions of the medial zone of the cerebellum. The contributions of the intermediate and lateral zone to the control of limb dynamics for gait and the adaptation of locomotor patterns are less well understood. In this study, we analysed locomotion and goal-directed leg movements in 12 patients with chronic focal lesions after resection of benign cerebellar tumours. The extent of the cortical lesion and possible involvement of the cerebellar nuclei was determined by 3D-MR imaging. The subjects (age range 13-…

[Quantitative indices of gait in patients with psychogenic and organic dysbasia].

The study aimed at comparison of quantitative indices of gait in patients with hysteria and organic dysbasia. Two groups of patients were studied - 28 patients with psychogenic dysbasia and 22 with organic gait disturbances (cerebellar ataxia in multiple sclerosis and post stroke hemiparesis). A control group included 10 healthy subjects. Space and temporal gait characteristics were investigated using an impregnation method (footprint), i.e. analysis of gait parameters by footprints. Step width (base) and length, step frequency and gait speed were assessed. All parameters were recorded both at…

Orthostatic myoclonus: a contributor to gait decline in selected elderly.

BACKGROUND: We encountered 15 patients with a newly recognized clinical phenomenon that we term orthostatic myoclonus. We conducted a retros … or observed leg jerking in 13 of 15 patients during upright posture. An insidious deterioration of gait that was often described as "apraxia" or "gait initiation difficulty" accompanied the myoclonus in 13 of 15 patients. Clinicians frequently suspected normal pressure hydrocephalus or orthostatic tremor syndrome. Surface EMG studies revealed short du …

No evidence of an association between MMR vaccine and gait disturbance.

BACKGROUND: MMR vaccine has been reported to cause gait disturbance, and this possible association has been claimed to support the MMR-causes-autism theory. AIMS: To determine whether any association between gait disturbance and MMR vaccination exceeds the age related background rate of gait disturbance, using record linkage and self control case series analyses. METHODS: MMR vaccination records were linked to hospital admission and general practitioner attendance data. An increased rate of gait problems with onset in various intervals in the 60 day period after MMR vaccination was looked fo…

Balance and gait deficits in schizophrenia compounded by the comorbidity of alcoholism.

OBJECTIVE: Alcoholism carries a liability of balance and gait instability that persists with sobriety. Such deficits are less well documented in schizophrenia and may be compounded by comorbidity with alcoholism, which is prevalent in schizophrenia. METHOD: The authors administered quantitative ataxia tests to 10 patients comorbid for schizophrenia and alcohol dependence/abuse, 10 nonalcoholic patients with schizophrenia, 24 nonschizophrenic patients with alcohol dependence, and 27 age-matched comparison men. RESULTS: All three patient groups were impaired relative to the comparison subjects…

Knock knee and the gait of six-year-old children.

AIM: Knock knee (genu valgum) interferes with the locomotive and supporting function of the lower limb. In static conditions the load-bearing axis of the valgus limb is displaced laterally in relation to the middle of the joint, causing the knee joint, the ankle joint, and the foot as a whole to be weighted in the wrong way. The purpose of this work is to examine the influence of knock knee on gait kinematics. METHODS: The gait of twenty-two 6-year-old children of both sexes in whom knock knee had been medically diagnosed was compared with the gait of 33 children of the same age whose knee jo…

A descriptive study of 'kangaroo gait' in sheep in northern Britain.

Two questionnaires were undertaken to obtain improved knowledge of the signalment, incidence and risk factors of the condition termed 'kangaroo gait' (KG; a forelimb locomotor disorder of adult female sheep). The first survey was mailed to 221 veterinary surgeons in practices in northern Britain which provided veterinary care for sheep, and the second was mailed to 438 sheep-flock owners and managers located in the Scottish Borders, who were recruited via their veterinary surgeons. Responses of 84.7 and 30.7% were achieved, respectively. In total, 29.7% of veterinary practices and 24.1% of flo…
