Gallbladder adenomyomatosis presenting as fever of unknown origin: a case report.

Gallbladder adenomyomatosis is a rare disorder, characterized by benign hyperplasia of the gallbladder mucosa creating invaginations through the muscular layer, known as Rokitansky-Aschoff sinuses. It is considered an acquired disease, with pathophysiology similar to that of the diverticular disease of the colon. Diagnosis is often achieved by ultrasound, but a significant percentage is misdiagnosed as chronic cholecystitis, whereas the diagnosis is finally achieved histologically. We describe a case of gallbladder adenomyomatosis presenting as fever of unknown origin. The patient was a 17-yea

Computed tomography of the nonvisualizing gallbladder.

A prospective computed tomography (CT) study of a group of patients suspected of having gallbladder disease was performed. Ten patients were evaluated who had gallbladder nonvisualization on two consecutive oral contrast medium administrations. Evaluation of the gallbladder and common bile duct by CT disclosed specific abnormalities including gallstones, unsuspected common bile duct obstruction, and acute and chronic cholecystitis. Additional information on the physiologic processes involved in gallbladder nonvisualization is offered.

Gallbladder volvulus misdiagnosed as acute acalculous cholecystitis: A case report.

INTRODUCTION AND IMPORTANCE: Gallbladder volvulus is a rare surgical disease with clinical manifestations similar to acute acalculous cholecystitis. Diagnosing gallbladder volvulus is critical as delayed surgical intervention in gallbladder volvulus is associated with high morbidity and mortality. CASE PRESENTATION: A 62-year-old male patient presented to our outpatient department for right upper quadrant pain of one-month duration. Taking into consideration the patient's clinical symptoms, laboratory results, and imaging findings, we diagnosed the patient with acute acalculous cholecystitis

Mast cell infiltration and activation in the gallbladder wall: Implications for the pathogenesis of functional gallbladder disorder in adult patients.

BACKGROUND: Functional gallbladder disorder (FGD) is characterized by recurrent biliary colic with a decreased gallbladder ejection fraction on cholescintigraphy but absence of visible gallbladder abnormalities on ultrasonography. FGD is generally regarded as a primary gallbladder motility disturbance, however, the underlying pathophysiology remains largely unknown. In this study, we investigated the potential role of mast cells in the pathogenesis of FGD by examining mast cell density and activation in the gallbladder wall. DESIGN: Twenty adult patients with FGD undergoing cholecystectomy we

Risk factors for poor outcomes of children with acute acalculous cholecystitis.

BACKGROUND: Acute acalculous cholecystitis (AAC) is generally considered to be a mild disease in children; however, if left untreated or treated without caution, AAC can lead to severe outcomes, such as death. The objectives of this study were to present the clinical features and identify the predictors of mortality in pediatric AAC. METHODS: Patients diagnosed with AAC between 2005 and 2012 were enrolled. AAC was defined by the presence of fever and an echo-proven thickened gallbladder wall exceeding 4 mm. A poor health outcome was defined as death. Further information related to the demogra

Pancreaticobiliary diseases associated with anomalous pancreaticobiliary ductal union.

BACKGROUND: Anomalous pancreaticobiliary ductal union (APBDU) is associated with different pancreaticobiliary diseases. The aim of this study is to clarify the frequency with which it occurs and its several subtypes in various pancreaticobiliary diseases. METHODS: Radiograms of 1752 subjects who underwent endoscopic retrograde cholangiopancreatography were reviewed independently by two endoscopists. APBDU was diagnosed using recently accepted criteria and divided into B-P and P-B subtypes according to the insertion of biliary and pancreatic ducts. RESULTS: Fifty-nine (8.7%) of 680 subjects w

[Functional sequelae and clinical importance of atrophic duodenitis].

As compared to healthy persons and patients with non-atrophic duodenitis, patients with atrophic duodenitis manifest a decrease of gallbladder contractility as shown by the ultrasonography data. In chronic pancreatitis with concomitant atrophic duodenitis, the degree of a decrease in pancreatic enzymic secretion is more considerable than in patients suffering from chronic pancreatitis without duodenitis or from non-atrophic duodenitis. Comparison of the tests for enhancement of the incretion of endogenous cholecystokinin-pancreozymin and with intravenous injection of the hormone formed the bas

Lymphangioma of the gallbladder in childhood: a case report and review of the literature.

Lymphangiomas are malformations of the lymphatic system, which is a common disease in children, usually superficial in body, but seldom seen in visceral organs. We present the case of a child with lymphangioma of the gallbladder. Also we reviewed the literatures. A 2-year-6-month-old boy complained of progressing difficulty in walking for 6 months. Cerebral MRI showed abnormal signals in the white matter suggesting leukodystrophy. Ultrasound of abdomen showed a mixed-echoic mass in the site of gallbladder. CT scan showed an enlarged gallbladder with increased density. Bloodcounting, liver func

As many as 30 patients with uncomplicated ulcer disease and 8 essentially healthy individuals were studied for the tone and contractile function of the gallbladder under basal conditions with regard to the phases of duodenal interdigestive motility. In normals, the gallbladder functions according to phases of duodenal interdigestive motility while the clinical picture in those patients with ulcer disease is distinguished by predominance of hypermotor dyskinesia of the gallbladder and bile tract, which fact is manifested by reduction of its capacity as well as hypertone and discordant work of L

Epidemiology and molecular pathology of gallbladder cancer.

Gallbladder cancer is usually associated with gallstone disease, late diagnosis, unsatisfactory treatment, and poor prognosis. We report here the worldwide geographical distribution of gallbladder cancer, review the main etiologic hypotheses, and provide some comments on perspectives for prevention. The highest incidence rate of gallbladder cancer is found among populations of the Andean area, North American Indians, and Mexican Americans. Gallbladder cancer is up to three times higher among women than men in all populations. The highest incidence rates in Europe are found in Poland, the Czech

Atlas for Cholangioscopy and Cholecystoscopy: A Primer for Diagnostic and Therapeutic Endoscopy in the Biliary Tree and Gallbladder.

Percutaneous endoscopy of the biliary system (cholangioscopy) and gallbladder (cholecystoscopy) has significantly impacted diagnostic and therapeutic approaches to many diseases in interventional radiology, overcoming previous challenges related to scope size and rigidity. The current endoscopes offer enhanced maneuverability within narrow tubular structures such as bile ducts. Before endoscopy, reliance on 2D imaging modalities limited real-time visualization during percutaneous procedures. Percutaneous endoscopy provides 3D perspectives, enabling a better appreciation of normal structures, t

Updates in Hepatobiliary Surgery: New Data on Portosystemic Shunts and Cholecystectomy in Dogs and Cats.

Advances regarding surgical treatment of hepatobiliary diseases over the past 5 years have resulted in improved outcomes and decreased mortality in veterinary patients. Options for minimally invasive treatment of intrahepatic portosystemic shunts and gallbladder disease have led to decreased morbidity with similar outcomes to open procedures. For extrahepatic portosystemic shunts, studies evaluating long-term outcome between surgical attenuation and medical management, as well as between options for surgical attenuation, are being used to direct evidence-based treatment of this congenital anom

A Case of Tubular Adenoma of Gallbladder Diagnosed Using Contrast-Enhanced Ultrasonography.

Contrast-enhanced ultrasound (CEUS) has been used to diagnose gallbladder (GB) diseases for recent years because it is sensitive to visualize vascularity. Herein, we report a case who had a 1.7 cm × 1.2 cm polypoid lesion located in the gallbladder fundus with a feeding artery located in the stalk. On CEUS, the lesion showed early arterial phase enhancement (time to peak enhancement 18 s), persisting throughout the venous and delay phases. This enhancing pattern suggested that the lesion was a GB adenoma rather than a GB carcinoma. Cholecystectomy was performed, and pathology of the tissue rev

Abnormalities in gallbladder morphology and function in patients with cholelithiasis.

Thirty-seven symptomatic cholelithiasis patients who had cholecystectomy were studied to determine the relationships between clinical manifestations, histologic findings and gallbladder absorptive capability. A clinical score was calculated from clinical data which we thought might be predictive of abnormal gallbladder histology. Histologic parameters indicative of gallbladder disease were used to calculate a histologic score. Shortcircuit current measurements, which reflect gallbladder sodium absorption, were used to assess absorptive function. Patients with very high clinical scores, indicati

Torsion of the gallbladder: two case reports.

Torsion of the gallbladder is a rare disease. It is simply defined as a rotation of the gallbladder on its mesentery along the axis of the cystic duct and cystic artery. The aetiology is unknown. However, several factors are postulated as playing causative roles. The disease symptoms mimic acute cholecystisis. The current value of radiological imaging is limited, but clinically, volvulus of the gallbladder is an acute surgical situation. It may be treated and diagnosed by a laparoscopic approach.

Imaging of the gallbladder, 1987.

Gallbladder disease, and the patients with symptoms suggestive of gallbladder disease, remains a widespread clinical problem that requires investigation by imaging techniques. Sonography, and to a lesser extent cholescintigraphy, has facilitated the evaluation of these patients and allows the diagnosis of cholelithiasis, and its principal complication of cholecystitis, with a high degree of reliability and accuracy. Interventional techniques are being developed that may provide nonsurgical approaches to improved therapy of gallbladder calculi and their complications.

Lack of endogenous cholecystokinin promotes cholelithogenesis in mice.

BACKGROUND: Celiac disease is an autoimmune enteropathy caused by a permanent intolerance to dietary gluten in genetically predisposed individuals. Cholecystokinin (CCK) release from the proximal small intestine and gallbladder emptying in response to a fatty meal are greatly reduced in celiac patients before they start the gluten-free diet, showing a genetic predisposition to gallstones. METHODS: To elucidate the complex pathophysiological mechanisms determining the biliary characteristic of celiac disease, we investigated the effect of the absence of endogenous CCK on cholesterol crystalliz

A Case of Pediatric Gallbladder Torsion Diagnosed by Vortex Ultrasound Sign.

Gallbladder is twisted in gallbladder torsion and abdominal symptoms rapidly progress. Timely and accurate diagnosis and treatment is essential. This disease is frequently misdiagnosed as acute cholecystitis and rarely occurs in children. Whirlpool sign and absent blood flow in the cystic artery were identified in a 12-year-old boy presenting with suspected acute cholecystitis. Computed tomography revealed simple cholecystitis, whereas ultrasound suggested gallbladder torsion. Emergency laparoscopic surgery confirmed distended gallbladder torsion, leading to cholecystectomy. Ultrasonic whirlpo

[Gallbladder torsion: report of a case].

A case of an 81 year old woman who had fever, abdominal pain and a palpable mass in epigastrium and right hypochondriac region is presented. She was diagnosed as having acute cholecystitis and hydrops of the gallbladder. The surgical operation was performed and the findings were: dilated gallbladder with necrotic aspect, free floating with torsion of the cystic duct (greater than 180 degrees) wrapped in the mentum. There was no hepatic bed of the gallbladder. After correcting the torsion, the gallbladder was extirpated, with good clinical evolution. The etiopathogenia is discussed and the lite
