Guideline for the Evaluation of Cholestatic Jaundice in Infants: Joint Recommendations of the North American Society for Pediatric Gastroenterology, Hepatology, and Nutrition and the European Society for Pediatric Gastroenterology, Hepatology, and Nutrition.

Cholestatic jaundice in infancy affects approximately 1 in every 2500 term infants and is infrequently recognized by primary providers in the setting of physiologic jaundice. Cholestatic jaundice is always pathologic and indicates hepatobiliary dysfunction. Early detection by the primary care physician and timely referrals to the pediatric gastroenterologist/hepatologist are important contributors to optimal treatment and prognosis. The most common causes of cholestatic jaundice in the first months of life are biliary atresia (25%-40%) followed by an expanding list of monogenic disorders (25%)

Identification of patients at increased risk for colorectal cancer in an open access endoscopy center.

GOALS: To determine whether patients referred for open access endoscopy (OAE) are being appropriately identified as "increased risk" or "average risk" for colorectal cancer (CRC) by referring physicians. BACKGROUND: OAE allows nongastroenterologists to schedule elective endoscopies without prior consultation with a gastroenterologist. It is unknown how accurately referring physicians identify CRC risk of such patients. METHODS: We retrospectively reviewed the records of outpatients referred to a single OAE center for screening or surveillance colonoscopy from July 1, 2001 to November 8, 2002…

Obscure gastrointestinal bleeding.

Obscure GI bleeding is a relatively common problem facing internists, gastroenterologists, and surgeons in a typical clinical practice. The etiology is occasionally suggested by the patient's age, history, and medications. Management is complicated and typically requires a team-oriented approach, with input from the internist, gastroenterologist, radiologist, and surgeon alike. SBFT and enteroclysis seem to have a limited role, unless there is a high suspicion of a small bowel mass lesion or Crohn's disease. Scintigraphy may be performed in patients with active bleeding in whom endoscopy has f…

[Incidence of inflammatory bowel diseases in Bretagne (1994-1995). ABERMAD. Association Bertonne d'Etude et de Recherche des Maladies de l'Appareil Digesif].

OBJECTIVES: The aim of the study was to determine the incidence and the main clinical data of inflammatory bowel disease in Brittany. METHODS: According to EPIMAD registry's methodology, private and public gastroenterologists (n = 139) of Brittany (2836418 inhabitants) referred all patients consulting for the first time, in 1994 and 1995 with clinical symptoms compatible with inflammatory bowel disease. An interviewer practitioner completed at the gastroenterologist's consulting room a standard questionnaire for each patient. Each case was reviewed separately by four experts to assign a diagn…

Where Do We Go from Here: Transforming Teaching Endoscopy Knowledge Using the Expert Performance Approach.

At Digestive Disease Week 2025, practicing and training gastroenterologists participated in a pioneering day-long course illustrating the future of endoscopy education. The 5-hour international workshop focused on the management of flat neoplasms and malignant polyps, employed expert performance cognitive task analysis, case-based simulation, interactive polling, and structured feedback to teach how to do rather than what to know. In a single-group pre-post design, knowledge increased from 67.9% to 84.0% (P<.001; d = 1.34), and self-efficacy improved across recognition and performance domains.…

Beyond the Basics: Recommended Approach to Severe Acute Pancreatitis Management in the Critical Care Setting.

Acute Pancreatitis (AP) poses a significant healthcare burden globally. While most cases present with a mild course, approximately one-fifth progress to severe forms characterized by severe pancreatitis and systemic complications, leading to a mortality rate of up to 40%. Effective management of severe acute pancreatitis necessitates a multidisciplinary approach involving gastroenterologists, interventional radiologists, intensive care teams, and surgeons. The advances in treatment modalities, including early hydration, nutrition, and pain control, have led to a decrease in the morbidity and l…

Still elusive: Developments in the accurate diagnosis of indeterminate biliary strictures.

Indeterminate biliary strictures pose a significant diagnostic dilemma for gastroenterologists. Despite advances in endoscopic techniques and instruments, it is difficult to differentiate between benign and malignant pathology. A positive histological diagnosis is always preferred prior to high risk hepatobiliary surgery, or to inform other types of therapy. Endoscopic retrograde cholangiopancreatography with brushings has low sensitivity and despite significant improvements in instruments there is still an unacceptably high false negative rate. Other methods such as endoscopic ultrasound and …

Endoscopic Management of Jejunal Perforation During Endoscopic Ultrasonography: A Case Report and Literature Review.

Endoscopic ultrasound is a useful diagnostic and interventional device for gastroenterologists. Although extremely useful, endoscopic ultrasound is not a benign tool. Possible complications of endoscopic ultrasound include hemorrhage, infection, and perforation. Although rare, iatrogenic small bowel perforations have been reported largely on the duodenum and rarely on the jejunum or ileum. Traditionally, these iatrogenic small bowel perforations have been managed with open surgery. However, recent emerging clinical data has revealed that immediate endoscopic treatment may be a feasible and saf…

[Acute portomesenteric venous thrombosis - an active and multidisciplinary approach may prevent surgery and save suffering].

Acute mesenteric venous thrombosis causes impaired mesenteric blood supply which may lead to bowel infarction and, in a longer perspective, severe portal hypertension. Early diagnosis, immediate anticoagulation, and active expectancy are critical for the outcome. The patients should be evaluated and treated in a multidisciplinary context, involving gastroenterologists, interventional radiologists, vascular and colorectal surgeons, and consultants in clinical coagulation. Percutaneous thrombectomy, including transjugular intrahepatic portosystemic shunt (TIPS), should be considered in cases wit…

Endo-hepatology: An emerging field.

Gastroenterologists have long been spearheading the care of patients with various forms of liver disease. The diagnosis and management of liver disease has traditionally been a combination of clinical, laboratory, and imaging findings coupled with percutaneous and intravascular procedures with endoscopy largely limited to screening for and therapy of esophageal and gastric varices. As the applications of diagnostic and therapeutic endoscopic ultrasound (EUS) have evolved, it has found a particular niche within hepatology now coined endo-hepatology. Here we discuss several EUS-guided procedures…

[Spinal epidural abscess - a rare complication of Crohn´s disease: case report].

Spinal epidural abscess (SEA) is a rare disease that occurs mainly in immunocompromised patients after spinal surgery or spinal trauma and can lead to a severe neurological deficit or even death if diagnosed too late. However, cases of SEA have also been reported in patients with fistulising Crohn´s disease (CD). We present a case of a young patient with CD and a history of relapsing perianal disease followed by a complication of SEA in the thoracic spine. In close cooperation with the orthopedists and the neurologists, the gastroenterologists have successfully treated the SEA in this patient,…

Endoscopic ultrasound-guided pancreaticobiliary intervention in patients with surgically altered anatomy and inaccessible papillae: A review of current literature.

The management of pancreaticobiliary disease in patients with surgically altered anatomy is a growing problem for gastroenterologists today. Over the years, endoscopic ultrasound (EUS) has emerged as an important diagnostic and therapeutic modality in the treatment of pancreaticobiliary disease. Patient anatomy has become increasingly complex due to advances in surgical resection of pancreaticobiliary disease and EUS has emerged as the therapy of choice when endoscopic retrograde cholangiopancreatography failed cannulation or when the papilla is inaccessible such as in gastric obstruction or d…

Eosinophilic oesophagitis: a systematic review for otolaryngologists.

BACKGROUND: Eosinophilic oesophagitis is a chronic, immune/antigen-mediated oesophageal disease, only recently, but increasingly, recognised in the world literature. It is diagnosed and managed primarily by medical gastroenterologists and allergy specialists, and is a distinct disease entity, affecting both children and adults. Few studies have been published in otolaryngology journals, although otolaryngologists will encounter patients with undiagnosed eosinophilic oesophagitis. Patients may present with dysphagia, bolus obstruction or with other ENT disorders, such as atopic rhinitis, reflec…

A Case of In-Bore Transperineal MRI-Guided Prostate Biopsy of a Patient with Ileal Pouch-Anal Anastomosis.

Ulcerative colitis (UC) is an inflammatory disease that specifically affects the colon. Ulcerative colitis is primarily treated medically and refractory disease is treated with proctocolectomy and ileal pouch-anal anastomosis (IPAA). Gastroenterologists advise against digital rectal exams, pelvic radiation therapy, and transrectal ultrasound (TRUS) biopsies of the prostates of ileal pouch-anal anastomosis patients. Any form of pouch manipulation can lead to severe bleeding, inflammation, and pain. Urologists are therefore faced with the challenge of doing a prostate biopsy without a transrecta…

Blastocystis: getting to grips with our guileful guest.

Blastocystis, a common single-celled intestinal parasite of humans and animals, continues to puzzle clinical microbiologists, gastroenterologists, and general practitioners who are still unsure of the clinical significance of the organism. Here we consider some less well-addressed areas of Blastocystis research, which, facilitated by recent technological advances, could potentially turn out to be significant pathways to knowledge. First and foremost we discuss new trends in Blastocystis research, including the 'omics' perspectives, and then highlight some aspects of Blastocystis research in th…

Assessment of gastric emptying and small-bowel motility: scintigraphy, breath tests, manometry, and SmartPill.

Gastric and small-bowel dysfunction can include gastroparesis, functional dyspepsia, and even irritable bowel syndrome. Patients with symptoms suggesting these disorders are commonly encountered by a variety of physicians, especially gastroenterologists. In most patients, the physical examination and upper endoscopy are normal, and thus symptoms are suggested to be from a motility disorder or a functional gastrointestinal disorder. Further evaluation directed at evaluating the stomach and small-bowel motility may help the clinician to arrive at a correct diagnosis enabling proper treatment of …
