Review of Gastrointestinal Capsule Robots: Materials, Actuation, Diagnostics, and Therapeutics.

Gastrointestinal capsule robots are minimally invasive devices for diagnosis and therapy in the digestive tract. This review examines them from a systems engineering perspective, spanning the technological chain from materials and actuation to sensing, treatment, and clinical translation. Structurally, current designs employ rigid polymers and metals, flexible elastomers, and biodegradable or stimuli-responsive materials to balance mechanical stability, biocompatibility, and environmental adaptability. Locomotion strategies range from passive peristalsis-driven transit to magnetic, wireless-po

Longitudinal metabolomics profiles reveal increased gut microbial protein fermentation during Spaceflight.

Microgravity is a condition that may affect gastrointestinal function and metabolism during spaceflight. Despite attempts to keep normal dietary habits at the ISS, the food selection and meal timing may also be perturbed during the confinement. To understand the impact of spaceflight on human metabolism, we characterized the plasma metabolome of astronauts (n = 52) before, during, and after missions on board the International Space Station (ISS) using liquid chromatography coupled with mass spectrometry. Here we show that spaceflight affected around 40 circulating compounds. In this longitudin

Barley Leaves Improves Loperamide-Induced Constipation via Gut Barrier and Microbiota Modulation in Mice.

Constipation is a common gastrointestinal disorder that seriously affects quality of life and is associated with multiple secondary complications. Barley leaves (BLs) have been suggested as potential functional foods for constipation prevention. Here, we investigated the preventive effects of common barley leaves (CBLs) and hulless barley leaves (HBLs) in a loperamide-induced constipation model in C57BL/6 mice. Both BLs improved stool parameters and gastrointestinal transit. Notably, high-dose HBLs increased stool weight to 263.84 ± 66.70 mg and stool amount to 250.20 ± 66.88 pellets, which we…

Bile acids in liver and gastrointestinal cancer.

Bile acids (BAs) are cholesterol-derived metabolites synthesized by hepatocytes and secreted in bile to follow an inter-organ transit to the intestine, where they facilitate fat digestion and the absorption of lipids and liposoluble vitamins. Traditionally considered mere end products of cholesterol catabolism, they are now acknowledged to play intricate roles in regulating intermediary metabolism by controlling the expression of crucial genes. Additionally, they exert a significant impact on inflammation, cytotoxicity, and carcinogenesis. Moreover, BAs have a critical impact on the crosstalk

Unusual Inconsolable Crying: An Insight, Case Report, and Review of the Literature on the Pitt-Hopkins Gastrointestinal Phenotype.

Pitt-Hopkins syndrome (PTHS) is a rare, neurodevelopmental genetic disorder caused by mutations in the TCF4 gene. This gene encodes a ubiquitous, class I, basic helix-loop-helix factor, which is implicated in various developmental and regulatory processes. Predominant clinical manifestations of PTHS include facial dysmorphisms, intellectual disability, absence of expressive language, epilepsy, as well as visual and musculoskeletal impairments. Gastrointestinal (GI) complications, such as chronic intestinal pseudo-obstruction, gastroparesis with delayed bowel transit, chronic constipation culmi

Effects of curdlan and gellan gum on the surface structure of intestinal mucosa in rats.

The effects of curdlan and gellan gum on the gastrointestinal function were studied, and the morphological structure of the intestinal mucosal surface was observed by scanning electron microscopy of rats fed curdlan and gellan gum diets for four weeks. The rats fed the curdlan diet showed a significant increase in the weight of the cecum and its contents and a decrease in fecal weight as compared to the rats fed a cellulose diet. On the other hand, the rats fed the gellan gum diet showed a weight loss in cecal contents and weight gain in colonic contents. The transit time of the gastrointestin

Novel techniques to study colonic motor function in children.

Colonic motility is an essential component of normal colonic physiology and it controls essential bodily functions such as stool propulsion, storage, and expulsion. Disordered colonic motility may present with constipation or diarrhea as well as associated symptoms such as bloating, gas, pain, incontinence, and others. In order to assess colonic motor function, practitioners may use studies that either investigate transit time or that evaluate peristaltic activity. Transit time is the result of both the effectiveness of propulsive pressures and the physical characteristics of the stools. Its m

Physiology and pathophysiology of colonic motor activity (2).

The basic motor function of the colon is to mix and knead its contents, propel them slowly in the caudad direction, hold them in the distal colon until defecation, and provide a strong propulsive force during defecation. Infrequently, it also produces mass movements in the proximal colon. These motor functions are achieved in most species by three different types of contractions: the individual phasic contractions that include the short- and long-duration contractions, organized groups of contractions that include the migrating and nonmigrating motor complexes, and special propulsive contracti

Association between postprandial changes in colonic intraluminal pressure and transit.

The aim of this study was to correlate the movement of colonic luminal contents with the changes in intraluminal pressure. Studies were performed in 9 healthy volunteers. Intraluminal pressure was measured with perfused catheter ports in the transverse, splenic flexure, descending, and sigmoid colon. Movement of the luminal contents was measured by following the movement of technetium 99m-ethylenetriamine-pentaacetic acid that was instilled as a bolus in the splenic flexure. During fasting there was very little change in pressure or in the movement of intraluminal contents. After eating a 1000

Radiologic imaging of the transplanted bowel.

BACKGROUND: The radiologic evaluation of the transplanted bowel is largely unknown and rather complex because it involves several techniques that depend on indications and times that have not been fully defined. METHODS: From December 2000 to November 2002 in the Section of Radiology I of the University of Modena and Reggio Emilia (Modena, Italy), 11 patients with transplanted bowel were studied with different methods: traditional radiologic evaluation with contrast agent (all patients), evaluation of transit time with radiopaque markers (five patients), ultrasonographic (US) evaluation of th

P2X7R+CD8αβ+ T cells exacerbate postoperative ileus by inducing the loss of enterochromaffin cells.

Postoperative ileus (POI) is a common complication after abdominal surgery, driven by inflammation. This study investigated how extracellular-ATP (eATP) and intestinal CD8αβ+ intraepithelial lymphocytes (IELs) contribute to POI. Using an intestinal manipulation mouse model, we found that CD8αβ+ IELs were increased and activated during POI, producing elevated granzyme B. Depletion of these T cells or inhibition of granzyme B ameliorated gastrointestinal transit impairment. Damage-induced eATP promoted granzyme B production in CD8αβ+ IELs via P2X7R signaling. Mechanistically, granzyme B degraded…

Laparoscopic Repair of Traumatic Diaphragmatic Hernia Secondary to Gunshot Wound.

This report presents the case of a 40-year-old man with a history of emergency surgery due to a gunshot wound. Twelve years later, he presented with persistent symptoms, including epigastric pain, pyrosis, and dysphagia. Diagnostic studies reveal a complex paraesophageal and paracardial diaphragmatic hernia. Traumatic diaphragmatic hernias (TDHs) present diagnostic challenges, particularly during the latent phase, with symptoms ranging from gastrointestinal to respiratory. A comprehensive evaluation and correct diagnosis are required, including diagnostic modalities such as CT scans, upper end

[A Case of Wernicke Encephalopathy with Ileus by Small Intestinal Cancer].

The patient was a 74 year-old woman. She came to the hospital with the chief complaint of vomiting, difficulty walking, and disorientation. The MRI study showed increased FLAIR and DWI signals in the bilateral medial thalamus. Abdominal enhanced CT scan showed a 2.4 cm tumor in the jejunum. And ileus due to small intestinal tumor was observed. She diagnosed Wernicke's encephalopathy associated with gastrointestinal transit disorder. Vitamin B1 300 mg/day for 3 days was started, and eye movements and disorientation improved on the day following the start of administration. On the 10th day after…

Potential for Gut Peptide-Based Therapy in Postprandial Hypotension.

Postprandial hypotension (PPH) is an important and under-recognised disorder resulting from inadequate compensatory cardiovascular responses to meal-induced splanchnic blood pooling. Current approaches to management are suboptimal. Recent studies have established that the cardiovascular response to a meal is modulated profoundly by gastrointestinal factors, including the type and caloric content of ingested meals, rate of gastric emptying, and small intestinal transit and absorption of nutrients. The small intestine represents the major site of nutrient-gut interactions and associated neurohor

The patient presenting with decompensated cirrhosis.

The rates of liver disease in the UK are rising and hence more patients than ever are presenting to acute medical units with potentially life threatening sequelae. Early recognition and treatment of sepsis, kidney injury, bleeding and alcoholic hepatitis can significantly improve outcomes, but requires a comprehensive approach to assessment. This patient cohort often suffers from a perceived uniform poor prognosis, especially in alcohol related disease, but evidence for this is changing and reassessment of prognosis after 48 hours of organ support may be more accurate than that made 'at the fr

In vivo assessment of novel furosemide gastro-mucoadhesive delivery system based on a kind of anion ion-exchange fiber.

A novel gastro-mucoadhesive delivery system based on a kind of anion ion-exchange fiber has been developed. Furosemide (FM), which is site-specifically absorbed from the upper gastrointestinal (GI) tract, was used as model drug. A novel-modified dissolution system, which can also be called "flow through diffusion cell," was used to study the drug release from the drug fibers. The GI transit studies of the FM fiber complexes in rats and gamma imaging studies in volunteers were carried out to evaluate the gastro-mucoadhesive behavior of the fiber. The pharmacokinetic profile and parameters of th

Action of pinaverium bromide, a calcium-antagonist, on gastrointestinal motility disorders.

1. The evidence reviewed here indicates that pinaverium bromide (Dicetel) relaxes gastrointestinal (GI) structures primarily by inhibiting Ca2+ influx through potential-dependent channels of surface membranes of smooth muscle cells. 2. The in vivo selectivity of pinaverium bromide for the GI tract appears to be due mainly to its pharmacokinetic properties. Because of its low absorption (typical for quaternary ammonium compounds) and marked hepatobiliary excretion, most of the orally-administered dose of pinaverium bromide remains in the GI tract. 3. Orally-administered pinaverium bromide does
