We aimed to compare graft loss due to acute rejection (AR) and chronic allograft failure (CAF) in first cadaveric kidney recipients, subsequently grafted during 1992-2000. All recipients (n=225) were divided into 3 age groups: Gr 1 (n=39) - 13-25 yrs, Gr 2 (n=169) - 26-55 yrs and Gr 3 (n=47) 56-71 yrs. There were no differences between the groups in terms of sex (m/f ratio 1.2-1.4), ischemia time and HLA matching. Incidence of high presensitization (PRA> or =50%) were observed in 15%, 5% and 9%, correspondingly (statistically significant difference was found only between Gr 1 and Gr 2). Immuno

Chronic allograft rejection (CAR) is characterized by progressive fibrosis and remains a major cause of long-term graft failure after transplantation. Here, we investigated the contribution of recipient TLR9 signaling to macrophage-associated fibrotic remodeling in CAR and evaluated pharmacological modulation of this pathway. Using murine heterotopic heart transplantation models of acute and chronic rejection, combined with single-cell RNA sequencing, global TLR9-deficient recipients, and primary cell co-culture systems, we found that macrophages with high TLR9 pathway activity were enriched i…

Accurate and non-invasive monitoring of allograft posttransplant is essential for early detection of acute cellular rejection and determines the long-term survival of the graft. Clinically, tissue biopsy is the most effective approach for diagnosing transplant rejection. Nonetheless, the procedure is invasive and potentially triggers organ failure. This work aims to design and apply GzmB-responsive nanosensors (GBRNs) that can readily size-change in graft tissues. Subsequently, we investigate the activity of serine protease granzyme B by generating a direct colorimetric urinary readout for non

Allergic diseases rob corneal allografts of immune privilege and increase immune rejection. Corneal allograft rejection in BALB/c allergic hosts was analyzed using a short ragweed (SWR) pollen model of allergic conjunctivitis. Allergic conjunctivitis did not induce exaggerated T-cell responses to donor C57BL/6 (B6) alloantigens or stimulate cytotoxic T lymphocyte (CTL) responses. Allergic conjunctivitis did affect T regulatory cells (Tregs) that support graft survival. Exogenous IL-4, but not IL-5 or IL-13, prevented Treg suppression of CD4(+) effector T cells isolated from naïve mice. However…

Specific immunomodulatory strategies are required to eliminate the need for lifelong dependence on debilitating immunosuppressants. One proposed strategy is to simultaneously transplant the kidney and infuse donor-specific bone marrow cells. We prospectively studied the effect of unmodified donor-specific bone marrow infusion (DSBMI) on rejection, infection, graft-versus-host disease (GvHD), and graft survival. We performed 57 kidney transplants in mixed lymphocyte culture (MLC)-reactive, outbred pigs. The groups of recipient pigs differed according to the use of (1) indefinite versus short-te

We showed that intragraft TLOs are sufficient to activate the alloimmune response and mediate graft rejection in a model where the only lymphoid organs are TLOs in the allograft. Chronic liver allograft rejection (CR) remains a significant cause of late graft dysfunction and failure despite advances in surgical techniques and immunosuppression. This chapter reviews the current understanding of CR, integrating diagnostic histopathological findings, atypical histological patterns of late rejection that are frequently associated with CR, and emerging diagnostic tools. CR is driven by overlapping T cell-mediated and antibody-mediated mechanisms, leading to progressive bile duct loss, vascular injury, and structural alterations. Diagnostic histopathological criteria include…

BACKGROUND: Kidney transplantation remains the treatment of choice for end-stage kidney disease. Maintaining immunosuppression within the appropriate therapeutic range is essential to prevent rejection and ensure long-term graft survival. This study evaluated the clinical relevance of different tacrolimus exposure metrics and their association with post-transplant outcomes in a real-world kidney transplant cohort.
METHODS: Of 881 adult deceased-donor kidney transplants performed between 2011 and 2020 at Heidelberg University Hospital, 372 recipients with a functioning graft at day 180 met the

BACKGROUND: Acute cellular rejection, though mostly encountered during the first 3 months after liver transplant, may occur later on. Clinical features and outcome of children experiencing late cellular rejection (LCR) have not been described to date.
PATIENTS AND METHODS: A total of 20 children who developed acute rejection 6 months or more after liver transplant were studied for history of early rejection, levels of immunosuppression, causes of low immunosuppression, viral infections, signs of autoimmunity, and HLA mismatch. All liver biopsies were reviewed. Fifteen patients with no history

BACKGROUND: The 2013 HIV Organ Policy Equity (HOPE) Act expanded kidney transplantation eligibility for people with HIV/AIDS (PWH), marking a significant milestone and necessitating updated reviews on the outcomes of renal transplantations on PWH. OBJECTIVE: To assess kidney transplantation outcomes in PWH, including patient and graft survival, graft rejection, and infection rates. METHOD: We conducted a systematic review of studies published between 1990 and 2023. Included studies reported on kidney transplantation outcomes in PWH, focusing on graft and patient survival, rejection, and infect…

Solid abdominal organ transplantation is fraught with variable rates of rejection and graft versus host disease (GVHD). We sought to determine the safety and efficacy of an advanced extracellular vesicle (EV) investigational product (IP) derived from mesenchymal stem cells (MSC) in the transplant patient population. Seven separate emergency investigational new drug (eNIDs) were filed with the Food and Drug Administration (FDA) for the emergency treatment of rejection of an isolated intestinal graft (n = 2), liver allograft graft (n = 2), modified multivisceral graft (n = 3), and GVHD in isolat

The influence of ethnicity on outcome of simultaneous pancreas-kidney transplantation (SPK) is poorly defined. After excluding technical failures, we retrospectively reviewed 96 consecutive SPKs (63 Caucasians [C], 33 African-Americans [AA]). All patients received antibody induction, tacrolimus, mycophenolate mofetil, and steroids. One-, 3-, and 5-year actuarial patient survival was similar between C (98%, 95%, 87%) and AA (90%, 90%, 81%), p=NS. One-, 3-, and 5-year kidney graft survival was similar between C (98%, 86%, 81%) and AA (85%, 85%, 78%), p =NS. One-, 3-, and 5-year pancreas graft su

Antibody-mediated rejection (AMR) is relatively uncommon in AB0-compatible liver grafts; however, recent data suggest that antibody-mediated mechanisms may play a role in the differential pathogenesis of liver graft rejection. In this sense, it has been shown that staining for diffuse C4d deposition (in endothelium or stroma of portal tracts or sinusoids) could be used as a tissue biomarker of humoral allograft rejection and fibrosis in biopsies obtained in post-transplantation period, in addition to the determination of donor specific antibodies (DSA). Therefore, more stringer criteria have t…

Kidney transplant recipients require lifelong immunosuppression to prevent graft rejection. However, immunosuppression is associated with adverse effects. A minority of kidney transplant recipients can be weaned off immunosuppression and maintain their graft function, a situation referred to as "functional or operational tolerance". We describe a case of a 70-year-old man who received a haploidentical hematopoietic cell transplant for lymphoma 22 years before receiving a kidney transplant from the same donor and was weaned off all immunosuppression by four months post-transplant. Tolerance was…

Liver transplantation is now widely recognised as an effective treatment option for patients with advanced liver disease. Many units now achieve greater than 85% survival at 1 year, with the majority of patients having a high quality of life. The maintenance of a high quality of life requires careful clinical management to ensure that the continued maintenance of excellent liver graft function is not achieved at the expense of immunosuppressive drug complications or morbidity. Acute liver rejection will occur in between 30 to 45% of patients, although with modern immunosuppressive protocols, u

BACKGROUND: In current non-human primate models, full-thickness porcine grafts could not achieve long-term survival without using potent systemic immunosuppressors. Moreover, the thickness disparity in xeno-PKP proved to be hard to manipulate and may cause several complications which also could prevent the grafts from long survival. Considering the advantages of Descemet's stripping automated endothelial keratoplasty (DSAEK) derived from its ultrathin graft and lower rejection rate, we hypothesize xeno-DSAEK may overcome these imperfections in xeno-PKP. The aim of this study was to explore the

Introduction Cytomegalovirus (CMV) is often associated with mortality and significant morbidity following renal transplantation leading to graft rejection or dysfunction. Primary CMV infection refers to the first detection of the virus in a person who has no prior evidence of CMV exposure before transplantation. CMV has a unique property called latency. After the initial infection, CMV can enter a dormant state within the body, residing in myeloid cells without causing active disease. CMV reactivation is likely when a latent CMV infection switches to a lytic phase of replication, which can be …

BACKGROUND: Few studies have examined the possible role of blood pressure (BP), independent of acute rejection and graft function, on outcomes after kidney transplantation.
METHODS: We investigated the prevalence, treatment, control, and clinical correlates of hypertension and its association with outcomes, using multivariate analyses with time-dependent covariates, in a retrospective cohort of 1,666 kidney transplant recipients.
RESULTS: Hypertension was common, and its control was poor. For example, at 1 year, only 55.5% had a BP less than 140 mm Hg. Control improved only slightly in 1993-

The first human heart transplantation was performed by Christian Barnard in 1967. While the technical aspect had been worked out, allograft rejection was a major limitation in the early days of heart transplant. The discovery of cyclosporine revolutionized the field and led to the modern era of transplant. Heart transplantation now offers the best survival benefit for patients with end-stage heart failure with a median survival over 12 years. However, there are still limitations including the impact of limited availability of graft, graft dysfunction, and rejection, and long-term non-cardiac c…

In kidney transplantation, the composition of the B-cell compartment is increasingly identified as an important determinant for graft outcome. Whereas naive and transitional B cells have been associated with long-term allograft survival and operational tolerance, memory B cells have been linked to graft rejection and graft loss. Chronic antibody-mediated rejection now represents a major complication in transplantation and is a challenge in current therapeutics. Here, we show that patients with chronic antibody-mediated rejection display a unique B-cell phenotype with a reduced ratio of activat…
