The comparison of usefulness prosthetic rehabilitation with removable and fixed suprastructures on endosseous implants.

Advanced atrophy bone in edentulous jaws is a serious problem for conventional prosthodontics. The advent of implantology, as well as guided reconstruction of bones, has opened new possibilities for conventional prosthodontic suprastructures. The turning point in implantology occurred in the sixties and was promoted by a Swedish orthopaedist Brånemark. Nowadays, implants are inserted routinely without creating any significant problems to a skillful maxillofacial surgeon. This study was performed in 73 patients who had 77 prosthodontic suprastructures (fixed suprastructures in 58 and removable

Osseous tumours of the hand: a review of 99 cases in 20 years.

BACKGROUND: Tumors involving the hand skeleton are rare. However, a basic knowledge of hand tumors is necessary for every clinician. This is due to the importance of distinguishing typical benign tumors from life or limb threatening malignant ones. METHODS: This study is a review of 99 cases of osseous hand tumors presented to the department of orthopedic surgery, Imam Khomeini Hospital in Tehran, Iran, from December 1990 to February 2011. RESULTS: Ninety-one cases were benign osseous tumors of the hand and eight tumors were malignant which four of them were considered as primary and four co

Prediction of hip and hand fractures in older persons with or without a diagnosis of periodontitis.

PURPOSE: In a prospective study, we assessed if a diagnosis of osteoporosis and periodontitis could predict hip and hand fractures in older persons. MATERIALS AND METHODS: Bone density was assessed by a Densitometer. Periodontitis was defined by evidence of alveolar bone loss. RESULTS: 788 Caucasians (52.4% women, overall mean age: 76 years, S.D.± 9.0, range: 62 to 96) were enrolled and 7.4% had a hip/hand fracture in 3 years. Calcaneus PIXI T-values < -1.6 identified osteoporosis in 28.2% of the older persons predicting a hip/hand fracture with an odds ratio of 3.3:1 (95% CI: 1.9, 5.7, p <

[Kienböck's disease. Diagnosis and therapy].

Kienböck's disease (carpal lunate necrosis or lunatomalacia) is the most common aseptic osteonecrosis of the upper extremity. Other bone necroses in the hand occur less frequently. The name indicates a disease with unclear etiology which eventually always leads to necrosis of the lunate bone. It usually affects the dominant hands of males between 20 and 40 years of age. Treatment methods extend from immobilization to revascularization surgery on the affected bone. There is still no gold standard for the treatment of Kienböck's disease.

Cytological Findings and Diagnostic Pitfalls of a Rare Tumor Arising From a Rare Site: "The Story of Chondromyxoid Fibroma of Great Toe".

Chondromyxoid fibroma (CMF) is a rare, benign cartilaginous tumor composed of myxoid and fibrous components, accounting for approximately 1% of all bone tumors. It typically arises in the metaphysis of long bones, with epiphyseal involvement of the short tubular bones of the hand and foot being uncommon. We report the case of a 14-year-old girl who presented with pain and a gradually progressive swelling of the left great toe, occurring after a fracture sustained 2.5 years earlier. Radiological findings were suggestive of an aneurysmal bone cyst or giant cell tumor. Fine-needle aspiration cyto

Comparison of the fixation ability of headless compression screws and locking plate for metacarpal shaft transverse fracture.

Metacarpal shaft fractures are common hand fractures. Although bone plates possess strong fixation ability, they have several limitations. The use of headless compression screws for fracture repair has been reported, but their fixation ability has not been understood clearly. This study aimed to compare the fixation ability of locked plate with that of headless compression screw for metacarpal fracture repair.A total of 14 artificial metacarpal bones (Sawbones, Vashon, WA, USA) were subjected to transverse metacarpal shaft fractures and divided into 2 groups. The first group of bones was fixed

Microsurgical second dorsal metacarpal artery cutaneous and tenocutaneous flap for distal finger reconstruction: anatomic study and clinical application.

In this paper, we report on the anatomical study of 34 cadaveric forearms with red latex injection and the clinical application of this study to 11 cases of microsurgical second dorsal metacarpal artery (SDMA) flaps. There were 8 cutaneous cases and 3 tenocutaneous cases using SDMA flaps for distal finger reconstruction. The SDMA was classified into 2 types and 4 subtypes according to its anatomical origin and course. Type I (76.5%) originated from the dorsal branch of the radial artery at the snuffbox. Type II (23.5%) originated from the perforating branch of the deep palmar arch at the bases

Use of multiple acoustic wave modes for assessment of long bones: model study.

Multiple acoustic wave mode method has been proposed as a new modality in axial bone QUS. The new method is based on measurement of ultrasound velocity at different ratio of wavelength to the bone thickness, and taking into account both bulk and guided waves. It allows assessment of changes in both the material properties related to porosity and mineralization as well as the cortical thickness influenced by resorption from inner layers, which are equally important in diagnostics of osteoporosis and other bone osteopenia. Developed method was validated in model studies using a dual-frequency (1

[Osteoarticular sarcoidosis].

Sarcoidosis (S) is a systemic disease affecting above all lymph nodes and lung tissue. Skeletal involvement is reported to occur in 14% of patients, the most common manifestations is cystoid osteitis, an asymptomatic lesion localized to the small bones of hand and feet. Lytic lesions are rare and usually accompanied by visceral involvement, the lesions may be detected in vertebral bodies and in long bones, pelvis and scapulae. MRI may be a good diagnostic tool, but open biopsy is often necessary. Joints are involved in about 89% of patients with acute sarcoidosis. Arthritis is localized more f

Hemodialysis-related lesions of the hand.

Chronic hemodialysis patients often have lesions of the hands characterized by distinctive etiopathogenic mechanisms and functional consequences. We conducted a prospective cross-sectional study in 116 patients with a mean age of 55.9 years and a mean hemodialysis duration of 8.17 years. Carpal tunnel syndrome was present in 28.4% of patients. Median nerve entrapment was bilateral in a large proportion of cases. Wasting of the lateral thenar muscles was often present at diagnosis, denoting advanced nerve compression. Amyloid was demonstrated in the carpal tunnel in 74% of cases of carpal tunne

Clinical anatomy of the m. flexor carpi radialis tendon sheath.

At the transitional zone from the forearm to the hand the insertion tendon of the m.flexor carpi radialis (FCR) glides on a fibrous and fatty cushion, which is connected dorsally with the joint capsule of the radiocarpal articulation. The tendon distally crosses the palmar side of the scaphoid tubercle and enters the dorsally curved rim of the trapezoid tubercle. At the level of the wrist joint the narrow tendon sheath begins, which extends to the insertion at the metacarpus. Immediately after entering the gliding tunnel the tendon branches off radially as a rule with an accessory fibre strand

Relationship of smoking status to genomic profile, chemotherapy response and clinical outcome in patients with advanced urothelial carcinoma.

Carpal instability includes a broad spectrum of osseous and ligamentous injuries which have been subclassified into greater and lesser arc injuries, in addition to combinations of both (Mayfield et al. J Hand Surg [Am] 5:226-241, 1980; Yaeger et al. Skeletal Radiol 13(2):120-30, 1985). The injuries typically occur from a fall on the outstretched hand with the wrist in ulnar deviation, hyperextension, and intercarpal supination (Yaeger et al. Skeletal Radiol 13(2):120-30, 1985). The force classically propagates from the radial to the ulnar side of the wrist resulting in a fracture (greater arc)

Retrospective review of outcome after surgical treatment of enchondromas in the hand.

BACKGROUND: Tumours of the skeleton of the hand are rare. While the majority of bone tumours are benign (89.4%), a small number show signs of malignancy (4.4%). Among the benign bone tumours of the skeleton of the hand, enchondromas are the most common, at 35-65%. METHODS: In the period from 1998 to 2005, a total of 35 enchondromas on the hand were diagnosed at the Trauma Center Lorenz Boehler. These were 16 women and 19 men with an average age of 36 years (age range 16-66). The most common site of an enchondroma was the proximal phalanx in 17 cases, followed by the metacarpal bone in 8 cases

The molecular genetics of human appendicular skeleton.

Disorders that result from de-arrangement of growth, development and/or differentiation of the appendages (limbs and digit) are collectively called as inherited abnormalities of human appendicular skeleton. The bones of appendicular skeleton have central role in locomotion and movement. The different types of appendicular skeletal abnormalities are well described in the report of "Nosology and Classification of Genetic skeletal disorders: 2019 Revision". In the current article, we intend to present the embryology, developmental pathways, disorders and the molecular genetics of the appendicular

[Fractures of the upper extremity in restrained automobile front seat passengers. Injury pattern and incidence in relation to accident mechanism].

During 1985 and 1995, 3,260 restrained car drivers (1,228 front seat passengers) were injured in 9,380 crashes involving cars. 179 (5.5%) (front seat passengers: 53, 4.3%) of those sustained fractures, of the arm. Among the 384 single fractures the hand (25%), wrist (23%) and forearm (23%) were affected most often, elbow (9%), upper arm (10%) and shoulder (10%) rather seldom. Half of the evaluated fractures of the long tubular bones were type A-fractures due to the AO classification and the other two quarters, respectively type B- and C-injuries in each case. 16% were open fractures. [Evolution and functional morphology of primate facial skulls].

Both ontogenetically and phylogenetically the facial skull of primates consists of two components: the endocranial nasal capsule, and the exocranial membrane bones. The cartilaginous nasal capsule of the fetal period constitutes the framework for the nasal cavity, and it also functions as an expansive basis for the developing facial skull. In adult animals, its ossified parts form the fragile ethmoid bone. The structure of the nasal capsule is determined on the one hand by the spatial requirements of the orbits and of the nasal cavity (with respiratory and olfactory components), and on the oth

[Muscular rupture of the extensor pollicis longus].

Closed ruptures of the extensor pollicis longus tendon usually appear as a consequence of fractures of the wrist joint or the carpal bones or ensue from polyarthritic changes or result from a process of degeneration. Mechanical injury of the tendon is quite rare but can be observed after direct trauma or after operative treatment of a distal fracture of the radius. Closed traumatic ruptures of the extensor pollicis longus tendon in the absence of pathological changes are--in spite of the frequency of rotation injuries of the forearm--very rare. The patient in the case described is a 45-year-ol

Macrodactylia fibrolipomatosis.

The clinical, pathologic, and roentgenographic features of macrodactylia fibrolipomatosis described are based on 40 cases from the literature and five cases seen by the authors. This condition previously has been described as macrodactyly, megalodactyly, macrodystrophia lipomatosa, dactylomegaly, and local gigantism. We believe macrodactylia fibrolipomatosis is the best term because it describes the characteristics which separate this condition from other forms of macrodactyly. Macrodactylia fibrolipomatosis is a form of macrodactyly noticed soon after birth. Usually affected are multiple adja

Bone-Vascular Axis in Chronic Kidney Disease.

Patients with chronic kidney disease (CKD) are at increased risk of osteoporosis and vascular calcification. Bone demineralization and vascular mineralization go often hand in hand in CKD, similar to as in the general population. This contradictory association is independent of aging and is commonly referred to as the "calcification paradox" or the bone-vascular axis. Various common risk factors and mechanisms have been identified. Alternatively, calcifying vessels may release circulating factors that affect bone metabolism, while bone disease may infer conditions that favor vascular calcifica
