Systemic contact dermatitis is an under-recognized skin reaction that occurs secondary to systemic (oral, intravenous, intramuscular, inhaled, or subcutaneous) exposure to a hapten in a previously sensitized individual. Our recent experience with membrane reactions in three hemodialysis patients is reported. We believe that two distinct populations manifest this reaction. First, well-known chronic hemodialysis patients may be exposed to inadequately rinsed new membranes. The etiology may be related to ethylene oxide either as a direct toxin or as a hapten attached to serum proteins. Pseudomonas aeruginosa produces a factor (PF) which alters the enterobacterial common antigen (ECA). Over 90 different compounds having structural similarities to barbituric acid were analyzed by radioimmunoassay (RIA) using 125I-secobarbital reagents (Roche). Affinities were compared with molecular structures and a number of observations are made.
