B-vitamin deficiency in hospitalized patients with heart failure.

The impact of heart failure and its treatment on specific nutrient requirements is unknown. Furthermore, depletion of water-soluble B vitamins that play key roles in the production of cellular energy in patients with heart failure can contribute to depletion of energy reserves observed in the failing heart. A cross-sectional study recently reported that approximately one third of hospitalized patients with heart failure had tissue levels suggestive of thiamin deficiency (vitamin B-1). Riboflavin (vitamin B-2) and pyridoxine (vitamin B-6) are similar to thiamin in that they are water-soluble, s

[Remote catamnesis of paroxysmal supraventricular arrhythmias which debuted in childhood].

This article deals with the 23-years assessment of natural history of 57 patients with paroxysmal supraventricular tachycardia debuted in childhood. Variants of clinical course of disease are described: cardiac, syncopal, abdominal, asymptomatic. In absence of treatment majority of paroxysmal tachycardias were characterized by wavy change of their clinical manifestations with periods of pronouncedly increased and decreased activity lasting from 3 to 5 years. Time interval between attacks was important for prognosis. Severity of disease was determined by relationship of three factors: frequency

Congenital complete heart block in patients without anatomic cardiac defects.

Forty-three patients with congenital complete heart block and an otherwise structurally normal heart were reviewed in an effort to better define the profile of an anatomically homogenous group and to identify factors that may predict the need for pacing. Fourteen patients (32%) developed "symptoms" during follow-up, including two with out-of-hospital cardiac arrest. Heart rate on electrocardiogram or Holter monitor did not clearly distinguish this subgroup. The presence of alternate "risk factors," such as atrial enlargement seen on electrocardiogram, cardiomegaly seen on x-ray film, or prolon

[Use of sympathomimetic agents in the treatment of acute circulatory insufficiency in the immediate postoperative period of heart surgery patients].

Sympathomimetic agents: isoproterenol (novodrin), dopamine, noradrenalin, adrenalin were used in 137 patients with acute circulatory insufficiency. Their hemodynamic effects were assessed, using catheterization of heart chambers and radiocardiography, and studies of circulating blood oxygen transport, acid-base state and metabolic product levels. A differential approach to sympathomimetic treatment has been developed. Isoproterenol is primarily indicated in those cases where myocardial failure is combined with decreased heart rate, conductivity disorders and markedly increased total peripheral

Greater Relative First and Second Lactate Thresholds in Females Compared With Males: Consideration for Exercise Prescription.

PURPOSE: To investigate differences between females and males at lactate thresholds 1 (LT1) and 2 (LT2). METHODS: Twenty-four female and twenty male participants performed an incremental cycle-ergometer test until exhaustion, where LT1 and LT2 were determined in each volunteer. RESULTS: Power output at LT1 and LT2 was lower in females than in males (P < .001). In addition, power output relative to peak power was higher in females at LT1 and LT2 (P < .001). However, heart rate was higher in females than males at LT1 and LT2 (P = .008). Furthermore, the heart rate relative to maximal heart rat

Cardiac dysautonomia and serotonin plasma levels in Rett syndrome.

BACKGROUND: In Rett syndrome the autonomic nervous system is abnormal at various levels, from the central to the peripheral nervous system. A role for serotoninergic dysfunction has been suggested. OBJECTIVES: The aim of our study was to evaluate the relation between cardiac dysautonomia (expressed by means of heart rate variability) and plasma serotonin levels in girls affected with Rett syndrome. Heart rate variability and plasma serotonin levels were evaluated in 28 Rett girls aged 1-14 years. A Pearson correlation was used to determine whether there was a relationship between plasma serot

Evidence of alpha-adrenoceptor-mediated chronotropic action in children.

We investigated alpha-adrenoceptor-mediated positive chronotropic action in children using a pharmacological autonomic function test. Under cardiac vagal and sympathetic blockade, chronotropic responses to intravenous infusion of an alpha-agonist, phenylephrine, were determined in 129 patients with suspected autonomic dysfunction. Heart rate changes to phenylephrine infusion (0.4 microg/kg/min) for 5 minutes ranged from -2 to 50 beats/min (mean +/- SD, 13.8 +/- 9.7 beats/min). alpha-Adrenoceptor-mediated chronotropic (APC) action more than 5 beats/min was observed in 88 (68%) subjects. APC was

Time-motion analysis of and heart rate responses to amateur ice hockey officiating.

The physical activity of referees (n = 4) and linesmen (n = 6) was analyzed from videotapes of four varsity hockey games to determine the frequency, average duration, and total time spent in each of six activity categories. The average duration of activity was low while frequency of occurrence was high. Low intensity activities occupied 96% of total time for referees and 94% of time for linesmen. Heart rate responses were recorded every 5 seconds and synchronized to the videotapes for all referees and 5 linesmen. Heart rates were above 70% of maximum for 70% of the total ice time, which was gr

Dopamine pharmacokinetics in critically ill newborn infants.

To compare clinical responses with plasma concentrations of dopamine and to compare dopamine pharmacokinetics in infants of different gestational age or clinical condition, dopamine was administered under carefully controlled conditions of dose and rate of infusion. The dose was increased stepwise from 1 to 2, to 2 to 4, and 4 to 8 micrograms/kg/min. Plasma concentrations of catecholamines, including dopamine, were compared with blood pressure, heart rate, and Doppler cardiac output. The data were analyzed to determine the threshold or minimal plasma concentration of dopamine necessary to prod

Determination of cardiac output by Doppler echocardiography: a critical appraisal.

Doppler echocardiography enables noninvasive determination of blood velocity and flow area through which quantitation of blood flow in vessels and across valvular orifices can be achieved. The stroke volume is rendered as the product of the flow area and the area beneath the velocity curve; on taking the heart rate into consideration, the cardiac output can be calculated. Essentially, this method can be used in the region of all four cardiac valves, the ascending aorta and the pulmonary artery. For calculation of the mitral and tricuspid velocity, the sample volume is positioned in the region

Body position and activity, but not heart rate, affect pump flows in patients with continuous-flow left ventricular assist devices.

OBJECTIVES: The aim of this study was to determine the contribution of pre-load and heart rate to pump flow in patients implanted with continuous-flow left ventricular assist devices (cfLVADs). BACKGROUND: Although it is known that cfLVAD pump flow increases with exercise, it is unclear if this increment is driven by increased heart rate, augmented intrinsic ventricular contraction, or enhanced venous return. METHODS: Two studies were performed in patients implanted with the HeartWare HVAD. In 11 patients, paced heart rate was increased to approximately 40 beats/min above baseline and then d

An analysis of stereotypical motor movements and cardiovascular coupling in individuals on the autism spectrum.

One of the core diagnostic features of autism spectrum disorder (ASD) is engagement in stereotypical motor movements, although the etiology of this repetitive behavior is unknown. Since the 1960s, it has been hypothesized that stereotypical motor movements serve a homeostatic regulation function, and thereby a putative coupling mechanism to cardiovascular arousal. However, to date, surprisingly few reports explicitly assess cardio-somatic coupling and stereotypical motor movements. The present exploratory study investigates coupling of stereotypical body rocking and hand flapping to heart rate

Decrease in arterial blood pressure in spontaneously hypertensive rats by the TXA2 receptor antagonist BM 13177 and by acetylsalicylic acid.

The effects of the selective TXA2 receptor antagonist BM 13177 and of the cyclooxygenase inhibitor acetylsalicylic acid on blood pressure and heart rate were investigated in anaesthetized spontaneously hypertensive rats (SHR). After intravenous injection of BM 13177 (60 mg/kg) there was a strong short-lasting decrease in arterial blood pressure (BP) with a maximum reduction of 21.6% in the third minute after injection. The heart rate was only slightly decreased. The formation of TXB2 and 6-keto-PGF1 alpha in serum, aorta thoracica, renal medulla, and renal cortex, which was determined by radio

Assessment of muscle oxygenation in children with congenital heart disease.

BACKGROUND: Adaptive responses to congenital heart disease result in altered muscle perfusion and muscle metabolism. Such changes may be detectable using noninvasive spectroscopic monitors. AIMS: In this study we aimed to determine if resting muscle oxygen saturation (MOx) is lower in children with acyanotic or cyanotic congenital heart disease than in healthy children and to identify differences in muscle oxygen consumption in children with cyanotic and acyanotic congenital heart disease. METHODS: Using a custom fiber optic spectrometer system, optical measurements were obtained from the ca

Comparison of Troponin in a Prehospital Blood Sample and Risk Scores.

Background/Objectives: Prehospital identification of non-ST-elevation acute coronary syndrome remains unresolved, and available risk scores incorporate troponin categorically. We compared the discriminative performance of cardiac troponin I measured in a blood sample obtained at first prehospital medical contact, analysed as a continuous variable, against the HEART, GRACE, and TIMI scores for predicting 30-day major adverse cardiac events. Methods: We conducted a prospective cohort study in 333 adults with non-traumatic chest pain suggestive of non-ST-elevation acute coronary syndrome, which w

Outcome of right ventricular bifocal pacing in patients with permanent atrial fibrillation and severe dilated cardiomiopathy due to Chagas disease: three years of follow-up.

OBJECTIVES OF STUDY: Several studies have shown that heart failure may benefit from cardiac resynchronization therapy (CRT). Studies have demonstrated a beneficial effect of right ventricular (RV) bifocal pacing, using two leads at different positions, in similar patient populations. The aim was to evaluate this approach in Chagas disease patients who developed both severe dilated cardiomiopathy and chronic atrial fibrillation. METHODS: The study included 30 patients with a mean age of 52 +/- 6 years (16 male), who had atrioventricular block at functional class II or IV (NYHA). Patients under

Adenosine infusion to patients with ischaemic heart disease may provoke left ventricular dysfunction detected by echocardiography.

Infusion of the endogenous vasodilator adenosine to patients with ischaemic heart disease (IHD) frequently provokes myocardial ischaemia, possibly caused by a coronary steal. The aim of this study was to detect the occurrence and incidence of disturbances in left ventricular (LV) wall motion and Doppler indices of altered LV function during infusion of adenosine. Thirty-seven patients (six female) aged 37-74 years with IHD, verified by coronary angiography, were given up to 200 micrograms kg-1 min-1 (mean 155 +/- 5) adenosine i.v. for 18 +/- 1 min. LV wall motion was monitored by two-dimension
