BACKGROUND: The optimal strategy for the eradication of Helicobacter pylori has yet to be determined. This paper summarizes some of the latest treatment strategies for eradicating H. pylori infection.
METHOD: Literature review by author.
RESULTS: Successful eradication of H. pylori requires the use of combination therapy involving control of gastric acid secretion together with anti-microbial drugs. We examined secretion, mRNA expression, and histologic localization of interleukin-8 (IL-*) and growth-related gene product-alpha (GRO alpha) in the Helicobacter pylori-infected gastric antral mucosa. Antral biopsies were obtained from an area of endoscopically intact mucosa. Chronic Helicobacter pylori gastritis has been put forward as a risk factor for development of gastric mucosal atrophy and gastric cancer. The purpose of our study was to investigate the long-term effects of H pylori gastritis on the gastric mucosa. Two entities of considerable recent interest, Helicobacter pylori infection of the stomach and food-cobalamin malabsorption, are each intimately associated with gastric abnormalities. Forty-two gastric biopsies, in two transport media, were homogenized and cultured on three media under micro-aerophilic conditions. Brain-heart infusion agar with a commercial antibiotic supplement (giving 10 mg vancomycin, 5 mg trimethoprim and 2500 i.u. polymixin per litre) yielded the best results. In order to examine the effect of Helicobacter pylori colonization on the gastric mucus microclimate, antral juxtamucosal pH was measured in 47 patients attending as out patients for upper gastrointestinal endoscopy. The mean pH in 28 patients negative for H. pylori was 6.40 +/- 0.24 compared to 6.88 +/- 0.16 in 19 patients who were positive (P < 0.0001). Ultrastructural examinations of biopsy specimens of the gastric mucosa were performed in 20 patients with chronic gastritis proven by endoscopy and microscopy. The presence of the Helicobacter pylori bacteria was found in close contact with epithelial cells of the antrum and corpus of the stomach. The bacteria were not present in the areas of frequently observed intestinal metaplasia. There were ultrastructural changes on the surface and within the cytoplasm of the epithelial mucous cells, which indicated their disturbed metabolism.
