Chronic herpes simplex virus type I glossitis in an immunocompromised man.

Herpes simplex virus (HSV) type 1 (HSV-1) infection of the tongue commonly accompanies acute primary herpetic gingivostomatitis. However, recurrent infection of the tongue is exceptional and is restricted to immunocompromised individuals. A 57-year-old man with corticosteroid-dependent chronic obstructive pulmonary disease and sciatica presented with a chronic median glossitis due to HSV-1. The main clinical and histological feature was massive necrosis of the entire mucosa. Immunohistochemistry demonstrated a considerable amount of HSV gB, gC and gD envelope glycoproteins dispersed in the cho…

Evolution of herpes simplex virus type 1 under herpesviral evolutionary processes.

Herpesviruses, the genomes of which are double-stranded DNA of 120 kilobase pairs or more, infect a wide range of vertebrates from mammals to fish. Herpes simplex virus type 1 (HSV-1), a representative of family Herpesviridae, is a ubiquitous human pathogen. HSV-1 relates to common mucocutaneous diseases, while HSV-1 infection can mean a serious outcome, e.g. blindness and insult to the central nervous system. Evolution of herpesviruses includes DNA rearrangements, often generating tandemly or invertedly repeated sequences. Studies of HSV-1 DNA dynamics substantiated these processes of DNA rec…

Etiopathology of Behçet's disease: herpes simplex virus infection and animal model.

The etiology of Behçet's disease has been proposed as being a viral, bacterial, genetic and immunological disorder. After Hulusi Behçet's viral etiological hypothesis, many investigators have tried to confirm this. Scrapings and fluids from patients were applied to subculture in chorioallantoic membranes of fertilized eggs as well as in mice and rabbits by direct intracerebral injection. Since the 1980s, in situ hybridization, blotting, and polymerase chain reaction has also been applied to identify the herpes simplex virus DNA and RNA in patients. Animal models were developed based on environ…

[Central nervous system infection due to Herpes simplex virus in AIDS].

Infections of the central nervous system by Herpes simplex viruses (Herpes simplex type 1 and Herpes simplex type 2) are uncommon in acquired immune deficiency syndrome and are often clinically and pathologically atypical. We have collected 11 cases of herpes simplex encephalomyelitis in AIDS patients reported in the literature. Only 3 of these cases presented with a typical, necrotizing, limbic encephalitis. Other clinicopathological patterns included ventriculitis, rhombencephalitis and myelitis. Ventriculitis and rhombencephalitis were usually due to infection by HSV-1, whereas myelitis was…

[Herpes simplex virus type II infection as an exophytic endobronchial tumor].

We report the case of a 52 year-old homosexual AIDS patients suffering from Herpes simplex virus type II infection of the esophagus and the bronchial system. The chest radiograph revealed a homogeneous, well-defined consolidation of the right upper lobe. Gastroscopy showed an ulcer of the esophagus and gastritis of the antrum. Bronchoscopically, marked necrotizing tracheobronchitis and an exophytic tumour causing complete blockage of the apical segment of the right upper lobe were demonstrated. Histological examination of the biopsy specimens of the esophagus and of the bronchus revealed Herpe…

Management of acyclovir-resistant herpes simplex and varicella-zoster virus infections.

Persons with AIDS who have CD4+ counts < or = 100 and transplant patients, especially bone marrow allograft recipients, may experience clinically significant infections with acyclovir-resistant herpes simplex virus (HSV) or varicella-zoster virus (VZV). Patients who have received prior repeated acyclovir treatment appear to be at the highest risk of harboring acyclovir-resistant strains. Algorithms for the management of these infections were developed at a recent roundtable symposium. The consensus of the panelists was that treatment with foscarnet should be initiated within 7-10 days in patie…

[Facial nerve paralysis induced by herpes simplex virus infection in mice].

The etiology of Bell's palsy remains unknown but clinical serological investigations have suggested herpes simplex virus (HSV) induced facial neuritis to be a potential cause. In order to verify the viral etiology of Bell's palsy it must be proved by animal experimentation. The author first succeeded in producing a transient facial paralysis of mice, with a herpes simplex viral neuritis simulating human Bell's palsy. Type 1 HSV (strain KOS, 4.5 X 10(6) pfu/ml) was inoculated into the posterior aspect of the auricle. Fifty-nine out of 104 mice (56.7%) developed facial paralysis, on the experime…

Non-traumatic acquisition of herpes simplex virus infection through the eye.

Primary ocular herpes is usually seen as a follicular conjunctivitis and blepharitis, with or without involvement of the cornea. It is unkno … ralising antibodies and the cornea, iris, and trigeminal ganglion were investigated for evidence of herpes simplex virus type 1, by cocultivation and the polymerase chain reaction. Some animals showed a severe ulcerative blepharitis with little to no involvement of the cornea, while disease was undetectable in others. The development of disease depended on the d …

Enhanced replication of herpes simplex virus type 1 in human cells.

The effects of DNA-damaging agents on the replication of herpes simplex virus type 1 (HSV-1) were assessed in vitro. Monolayers of human lung fibroblast cell lines were exposed to DNA-damaging agents (methyl methanesulfonate [MMS], methyl methanethiosulfonate [MMTS], ultraviolet light [UV], or gamma radiation [GR]) at specific intervals, before or after inoculation with low levels of HSV-1. The ability of cell monolayers to support HSV-1 replication was measured by direct plaque assay and was compared with that of untreated control samples. In this system, monolayers of different cell lines in…

Evolution of post-natal herpes simplex virus encephalitis to multicystic encephalopathy.

A 3-week-old, previously healthy infant developed biopsy-proven Herpes virus type 2 (HSV-2) encephalitis. The encephalitis was characterized by cells having intranuclear inclusions and was without evidence of inflammation or hemorrhage. Neuroimaging studies did not show any destructive lesions in the brain. In spite of antiviral therapy, the infant's neurological conditions deteriorated, and the patient died at the age of 18 weeks. Post-mortem examination showed that most of the cerebral hemispheres were replaced by multiloculated cystic cavities of various sizes, typical of multicystic enceph

Management of non-genital herpes simplex virus infections in immunocompetent patients.

Non-genital herpes simplex virus in immunocompetent hosts causes a variety of primary infections--gingivostomatitis, keratoconjunctivitis, herpetic whitlow, and encephalomyelitis. Recurrent infections with orolabialis are very common, but are usually mild and self-limiting. Cutaneous complications of herpes simplex virus infections include eczema herpeticum and erytherma multiforme. Systemic treatment with acyclovir is indicated in encephalomyelitis, progressive eczema herpeticum, and frequent severe erythema multiforme. Chronic, suppressive acyclovir treatment may be helpful in severe recurre…

[Acute meningoencephalitis in children caused by herpes simplex virus type I].

In 14 observations of acute meningoencephalitis in children mainly under 3 years of the age, data pointing to an etiological role of herpes simplex type I virus, were obtained on laboratory examination. Six children died, and in the rest 8 children who survived gross residua were observed. The morphological picture showed extensive colliquation necroses in the parietal, temporal, and less frequently, in occipital lobes and the Varolian pons. In 4 cases out of 6 intranuclear inclusions were detected. The incidence of herpetic encephalitis was 16%.
