A shearing osteochondral fracture of the humeral head following an anterior shoulder dislocation in a child.

A dislocation of the shoulder joint is rare in children with an open physis. The fractures associated with an anterior dislocation generally reported in the literature have been Hill-Sachs lesions, avulsions of the greater tuberosity and glenoid fractures. We present a case of a previously unreported shearing osteochondral fracture, which is distinct from a classic Hill-Sachs lesion of the humeral head, in 12-year-old boy. The patient suffered a traumatic anterior shoulder dislocation with a spontaneous reduction along with this associated fracture. The fracture subsequently healed with no fur

Severe humeral erosion in a bedridden patient: a comprehensive intervention to prevent bone fracture.

This article reports a 1-yr prospective study related to a bedridden patient with rotator cuff tear arthropathy featuring severe erosion of the proximal part of the humerus. To prevent spontaneous humeral fracture, a comprehensive intervention was planned. This included drug therapy for osteoporosis and pain, nurse team training on patient handling techniques, and patient and caregiver education. After 1 yr, clinical and radiologic findings are stable.

The functional outcome of operative treatment of ununited fractures of the humeral diaphysis in older patients.

Twenty-two elderly patients (average age, seventy-two years) who had an atrophic, unstable, ununited fracture of the humeral diaphysis were managed with plate-and-screw fixation and application of an autogenous bone graft from the iliac crest. Fifteen of the patients had had at least one previous operation in an attempt to obtain union of the fracture. One patient had an active infection and two had a quiescent infection, all with Staphylococcus epidermidis. The average duration of nonunion before the patients were first seen by us was two years and four months (range, five months to sixteen y

A slowly evolving brachial plexus injury following a proximal humeral fracture in a child.

The association of fractures and neurological injuries is well recognized, especially with certain upper limb fractures. Typically, the nerve injury occurs at the time of initial fracture displacement. A case is reported of an unusual combination of fracture and nerve injury in a child, with the extremely rare occurrence of a slowly evolving nerve injury. The potential for delayed nerve injury following a fracture needs to be appreciated to prevent delays in diagnosis and treatment.

[Indications and results of the surgical treatment of distal intra-articular humeral fractures in the elderly].

In this paper we review 16 patients with closed intercondylar fractures of the distal end of the humerus that had open reduction and internal fixation. All patients were older than 60 years of age. Using the classification system of Müller et al. 6 fractures were classified as type C2 and 10 fractures as type C3. The surgical procedures and the types of internal fixation of the fractures allowed early postoperative mobilisation (3rd postoperative day). A rating scale modified according to Cassebaum was used for the results. At a mean follow-up of 4.2 years 3 patients were rated as excellent 9

Surgical versus non-surgical treatment of humeral SHAFT fractures compared by a patient-reported outcome: the Scandinavian Humeral diAphyseal Fracture Trial (SHAFT)-a study protocol for a pragmatic randomized controlled trial.

BACKGROUND: The outcome of non-surgical treatment is generally good, but the treatment course can be long and painful with approximately a quarter of the patients acquiring a nonunion. Both surgical and non-surgical treatment can have disabling consequences such as nerve injury, infection, and nonunion. The purpose of the study is to compare patient-reported outcomes after surgical and non-surgical treatment for humeral shaft fractures. METHODS: A pragmatic randomized controlled trial (RCT) is planned with two study groups (SHAFT-Young and SHAFT-Elderly). A total of 287 eligible acute humeral

Intramedullary Nailing Versus Modular Megaprosthesis in Pathological Humeral Fractures: A 10-Year Retrospective Comparative Study.

BACKGROUND: Pathological fractures of the humerus secondary to metastatic disease represent a significant cause of pain, disability, and reduced quality of life in oncologic patients. Surgical management aims to restore stability, reduce pain, and allow early mobilization. However, the optimal strategy between intramedullary nailing and modular megaprosthesis remains debated, particularly in relation to functional outcomes and long-term results. METHODS: A retrospective observational study was conducted on 48 patients treated for pathological or impending humeral fractures between January 201…

A novel single-portal arthroscopic technique for the management of pediatric humeral lateral condylar fractures.

BACKGROUND: Surgical intervention is indicated for significantly displaced lateral humeral condyle fractures (LHCFs) in children. Arthroscopic-assisted closed reduction represents a minimally invasive alternative; however, its widespread adoption has been limited by the technical challenges inherent in both pediatric fracture management and elbow arthroscopy. This study introduces a simplified technique utilizing a single proximal anterolateral portal for arthroscopic-assisted reduction, which has shown promising efficacy and safety. METHODS: A retrospective analysis was performed on 18 pedia

Humero-ulnar external fixation of pediatric supracondylar humeral fractures using an adult wrist external fixator.

BACKGROUND: Although cross-pinning is widely used to treat pediatric supracondylar humeral fractures, it involves risks of nerve injury, malunion, and compartment syndrome. In this two-center retrospective observational study, we compared the outcomes of humero-ulnar external fixation using an adult wrist fixator with those of conventional pinning for pediatric supracondylar humeral fractures. Given the technical complexity of Slongo lateral external fixation, we evaluated a simpler joint-spanning external fixation method. METHODS: This study enrolled 37 children: 19 treated with pinning betw…

Retrospective surgical outcomes of Gartland type III supracondylar humeral fractures with absent pulse in children.

INTRODUCTION: Supracondylar humeral fractures are the most common elbow injuries in children. Gartland type III fractures require urgent treatment due to the high risk of neurovascular injury. Vascular compromise occurs in 5%-15% of cases, most commonly involving the brachial artery, and clinical presentation ranges from complete ischemia ("cold, pale hand") to the "pink pulseless hand" (PPH). MATERIALS AND METHODS: Twelve children (9 males, 3 females) with Gartland type III supracondylar humeral fractures and absence of a palpable radial pulse, treated between April 2023 and September 2025, …

Geriatric-Typical Characteristic Complexes Predict Short-Term Outcome of Proximal Humeral Fractures in Geriatric Patients.

PURPOSE: Geriatric-typical multimorbidity (GTMM) categorizes older patients with multiple geriatric syndromes, but its impact on PHF outcomes remains unexplored. This study evaluates GTMM's influence on PHF patients within three months after fracture, aiming to improve geriatric care strategies. METHODS: Patients ≥65 years with a PHF diagnosis (ICD S42.2) between 2011 and 2022 were included. GTMMs were collected within two years prior to PHF. Patients were categorized into five treatment groups based on surgical intervention (reverse total shoulder arthroplasty (RTSA), locked plate fixation (

[Biomechanical advantages of personalized Y-shaped plates in treatment of distal humeral intra-articular fractures].

OBJECTIVE: To compare the biomechanical properties of personalized Y-shaped plates with horizontal plates, vertical plates, and traditional Y-shaped plates in the treatment of distal humeral intra-articular fractures through finite element analysis, and to evaluate their potential for clinical application. METHODS: The study selected a 38-year-old male volunteer and obtained a three-dimensional model of the humerus by scanning his upper limbs using a 64-slice spiral CT. Four types of fracture-internal fixation models were constructed using Mimics 19.0, Geomagic Wrap 2017, Creo 6.0, and other

Outcomes of intramedullary nailing versus plate fixation for humeral shaft fractures: a retrospective cohort study.

BACKGROUND: Humeral shaft fractures account for 1-3% of all bone fractures. Conservative treatment often leads to complications such as non-union and shoulder stiffness. Surgical fixation with a dynamic compression plate (DCP) has been the gold standard treatment. Intramedullary nailing (IMN) has recently gained popularity due to its minimally invasive approach and reduced risk of radial nerve injury. This study aims to compare the outcomes of IMN and plate fixation for humeral shaft fractures. METHODS: This retrospective study included patients with humeral shaft fractures treated with eithe

Intramedullary Nailing for Humeral Shaft Fractures: Functional Outcome Assessment Within a Cohort of 202 Patients.

Background: Humeral shaft fractures (HSFs) represent 13-25% of humeral fractures and are frequently complicated by radial nerve palsy and nonunion. While conservative management was historically preferred, surgical fixation with intramedullary nailing (IMN) has gained increasing popularity. The aim of this study was to evaluate the effectiveness of IMN in the treatment of HSFs, focusing on postoperative complications and functional outcomes. Methods: A bicenter retrospective analysis was conducted on 202 patients who underwent antegrade IMN fixation for HSF between 2014 and 2019, with a minimu…

Role of three-dimensional computed tomography with humeral subtraction in assessing anteromedial facet coronoid fractures.

BACKGROUND: The anteromedial facet (AMF) of the coronoid is a key structure in resisting varus posteromedial rotatory instability (PMRI) of the elbow. However, not all isolated coronoid fractures involve the AMF and not all fractures involving the AMF are the result of a PMRI mechanism. There is debate regarding the management of isolated coronoid fractures. A reliable method of differentiating this heterogeneous group of isolated coronoid fractures is essential to develop an appropriate management algorithm. The aim of this study was to evaluate the role of additional humeral subtraction thre

Arthroscopically assisted closed reduction for displaced lateral humeral condyle fractures over 4 mm in children.

INTRODUCTION: Lateral humeral condyle fractures (LHCFs) are the most common intra-articular fracture occurring at the elbow in children. Conventional treatment for displaced pediatric LHCFs is open reduction and percutaneous pinning, and few studies have regarded the efficacy of arthroscopic-assisted techniques. We aimed to evaluate the efficacy of anatomic reduction via elbow arthroscopy for pediatric humeral lateral condyle fractures with displacements >4 mm. METHODS: A total of 32 children with LHCFs featuring displacements >4 mm were enrolled in this retrospective study. Arthroscopically
