Intra-articular fractures of the upper extremity: new concepts in surgical treatment.

New techniques of internal fixation, postoperative rehabilitation, and emphasis on functional as well as radiographic outcome have refined the surgical treatment of complex fractures of the glenoid, humeral head, supracondylar and intracondylar humerus, olecranon, radial head, distal radius, and distal radioulnar joint over the past decade. Early stabilization and rehabilitation of these injuries leads to soft-tissue stabilization and facilitates the patient's ability to place the hand in three-dimensional space.

Combined Pediatric T-condylar Humeral Fracture and Monteggia Fracture-Dislocation in the Upper Extremity: A Case Report.

Monteggia fractures are characterized by a fracture of the ulna associated with radial head dislocation. T-condylar fractures of the humerus are intra-articular injuries that disrupt the distal humeral epiphysis. This report describes a rare case of a pediatric patient with an ipsilateral T-condylar humeral fracture and a Monteggia fracture-dislocation. A 10-year-old girl presented with elbow pain and deformity after falling while playing basketball. No open wounds or neurological deficits were observed, and the radial artery was palpable. The radiograph revealed a pediatric T-condylar humeral

Rare combined variations of the axillary artery: superficial ulnar artery and circumflex humeral arteries.

The variations in the branching pattern of the axillary artery are clinically significant. We report an extremely rare combination of arterial variations discovered during the routine anatomical dissection of the left upper limb in a 73-year-old male cadaver. The most significant findings were a superficial ulnar artery originating directly from the axillary artery, proximal to the ansa pectoralis. The superficial ulnar artery coursed superficially in the forearm and terminated as the primary contribution to the superficial palmar arch. Additionally, a common trunk for the circumflex scapular

Arthroscopic Humeroplasty in the Treatment of Acute Reverse Hill-Sachs Lesion Associated With Posterior Shoulder Instability.

Reverse Hill-Sachs lesions (RHSL) are common complications associated with posterior shoulder dislocations and represent a significant challenge for preserving joint stability and function. If untreated, these compression fractures of the anteromedial humeral head can compromise the integrity of the joint, predisposing patients to recurrent instability and arthropathy. While various treatment modalities exist, achieving an anatomic reduction of the defect while preserving the articular cartilage remains a desirable outcome, particularly in acute settings. This case report describes the success…

Is the pectoralis major tendon a reliable reference for restoration of humeral length with fracture hemiarthroplasty?

BACKGROUND: The primary objective was to calculate and to apply the numeric value of the distance from the pectoralis major tendon insertion to the superior aspect of the humeral head (PMTD) without any radiation exposure or equipment through basic data such as age, sex, height, and weight of Asian populations. METHODS: We analyzed shoulder magnetic resonance images of 260 patients (107 men and 153 women; average age, 59.8 years). The superior border of the pectoralis major was identified on the transverse section and cross-referenced with the coronal section. Measurements were made inferiorl…

Posterior shoulder fracture-dislocation: an update with treatment algorithm.

Posterior shoulder fracture-dislocation is a rare injury accounting for approximately 0.9 % of shoulder fracture-dislocations. Impression fractures of the articular surface of the humeral head, followed by humeral neck fractures and fractures of the lesser and grater tuberosity, are the more common associated fractures. Multiple mechanisms have been implicated in the etiology of this traumatic entity most commonly resulting from forced muscle contraction as in epileptic seizures, electric shock or electroconvulsive therapy, major trauma such as motor vehicle accidents or other injuries involvi…

Rate of avascular necrosis and time to surgery in proximal humerus fractures.

PURPOSE: Avascular necrosis (AVN) of the humeral head is a devastating complication of proximal humeral fracture (PHF) that often results in long-term morbidity for the patient. Rates of AVN depend on the number of fracture fragments and are highly variable. The literature suggests that timely stable and anatomic reduction may decrease the rate at which AVN develops after PHF. To our knowledge, there is no literature published investigating a temporal relationship between the timing of PHF fixation and rates of AVN. METHODS: Operative records of one orthopedic trauma surgeon were used to iden…

Lateral radial head offset can be associated with chronic epicondylopathia humeri radialis: A new radiological parameter.

PURPOSE: This study aimed to compare anatomical parameters on magnetic resonance imaging (MRI) scans between patients with symptomatic isolated chronic epicondylopathia humeri radialis (cER) and healthy controls. It was hypothesized that the radial head would show an increased lateral offset in patients with cER. METHODS: This retrospective radiological case-control study included patients with cER (symptomatic > 6 months) from November 2020 to July 2023. The MRIs of all included patients were compared to those of healthy controls. Radiological measurements were performed in millimetres (mm):

The effect of a combined glenoid and Hill-Sachs defect on glenohumeral stability: a biomechanical cadaveric study using 3-dimensional modeling of 142 patients.

BACKGROUND: Bone loss in anterior glenohumeral instability occurs on both the glenoid and the humerus; however, existing biomechanical studies have evaluated glenoid and humeral head defects in isolation. Thus, little is known about the combined effect of these bony lesions in a clinically relevant model on glenohumeral stability. HYPOTHESIS/PURPOSE: The purpose of this study was to determine the biomechanical efficacy of a Bankart repair in the setting of bipolar (glenoid and humeral head) bone defects determined via computer-generated 3-dimensional (3D) modeling of 142 patients with recurre…

[Arthropathies due to bismuth poisoning].

Encephalopathies caused by intoxication with bismuth are liable to be complicated by arthropathies of the shoulder, on one or both sides, of an osteolytic, and probably osteonecrotic, type involving to a greater or lesser extent the humeral head and/or by destruction of articular cartilage with ateration and flattening of the heumeral head. The relation between these arthropathies and bismuth intoxication appears certain but is not yet understood. The strict electivity of the condition for the shoulder is not understood either. Certain analogies indicate similarity with metabolic arthropathies…

Hemiarthroplasty for glenohumeral arthritis with massive rotator cuff tears.

Rotator cuff deficiency with associated glenohumeral arthritis poses a formidable clinical challenge. Humeral head replacement with maintenance of the coracoacromial arch is the current treatment of choice. Properly performed arthroplasty can provide pain relief and modest gains in motion.

Radioulnar Synostosis after Endomedullary Fixation of the Radius and Ulna in a Patient with Floating Elbow: A Case Report.

Floating elbow is a complex and rare entity caused by high-energy trauma. In this paper, we present the case of a patient who suffered a traffic accident with severe head trauma, floating elbow (humeral diaphyseal fracture, radial proximal diaphyseal fracture, and ulnar segmental fracture) and radial nerve injury. Fixations were made with a humeral plate and intramedullary rods in the forearm. Although the outcome was satisfactory, radioulnar synostosis was identified in postoperative controls. Due to the neurological compromise, type of fractures, and stabilization selected, we believe that t

[Extra-coracoid axillary approach to the gleno-humeral joint].

The authors advocate an elective approach to the lower part of the humeral (shoulder) joint by an original axillary route. This route is external to the short head of the biceps brachii muscle which is retracted together with the neurovascular bundle. The articular capsule is opened through the space between the subscapular and latissimus dorsi muscles. The skin incision is cosmetic. No muscle is cut, and in surgery for anterior instability of the shoulder the crucial area is automatically exposed. This technique, however, requires two assistants and may be arduous in very muscular subjects. I
