Characterization of deletions in the LDL receptor gene in patients with familial hypercholesterolemia in the United Kingdom.

A sample of 200 patients with a clinical diagnosis of heterozygous (189) or homozygous (11) familial hypercholesterolemia (FH) attending lipid clinics in the London area have been screened for the presence of major gene defects in the low density lipoprotein (LDL) receptor gene by Southern blotting of genomic DNA with specific probes. This study is part of a project to determine the frequency of known mutations in the LDL receptor gene in this population. A new polymorphism for the enzyme Bgl II was identified by hybridization with a probe specific for the promoter plus exon 1 of the LDL recep…

Teratogenic action and embryo lethality of AY 9944R. Prevention by a hypercholesterolemia-provoking diet.

Embryomortality and teratogenesis provoked by inhibitors of cholesterol synthesis are well demonstrated. Teratogenic action is particularly reflected by holoprosencephalies, but also by uro-genital abnormalities. A hypercholesterolemia-provoking diet has been shown to be completely effective for preventing holoprosencephaly, but only partially so for preventing the uro-genital malformations and fetal mortality. It is thus possible that the two types of abnormalities are governed by different mechanisms. In addition, the diet itself, whose hypercholesterolemic effect is considerable, has certai…

Understanding Barriers and Motivators of Familial Hypercholesterolemia Genetic Testing: Insights From the Singapore Public.

Familial hypercholesterolemia (FH) is a common monogenic disorder associated with elevated low-density lipoprotein cholesterol and premature cardiovascular disease. Despite Singapore's high FH prevalence and the launch of the National FH Genetic Testing Programme in 2025, uptake remains suboptimal. This study aimed to assess public attitudes, perceptions, and willingness to undergo FH genetic testing. A cross-sectional survey of 333 Singaporean adults was conducted. Descriptive and comparative analyses assessed willingness, concerns, and information needs. While 71% of participants expressed s…

Probiotic intervention attenuates hypercholesterolemia and modulates gut microbiota in high-fat diet-fed rats.

Hypercholesterolemia is a major risk factor for cardiovascular diseases and is often associated with lipid metabolism disturbances, liver dysfunction, and gut microbiota imbalances. Probiotics have emerged as promising agents to improve cholesterol management through gut microbiome modulation and anti-inflammatory effects. This study investigated the impact of probiotics on weight, lipid profiles, liver function, and gut microbiota composition in high-fat diet (HFD)-induced hypercholesterolemic rats. Rats were divided into five groups: normal diet (NG), HFD, HFD + Rosuvastatin (SG), HFD + prob…

Myocardial Ischemia in Young Women With Familial Hypercholesterolemia: Similar Presentation and Divergent Treatment Response.

BACKGROUND: Familial hypercholesterolemia (FH) is a disorder often underdiagnosed in young women, delaying prevention of premature coronary disease. CASE SUMMARY: We report 2 women with FH and strong family histories of cardiovascular disease who developed early ischemic events. The first patient experienced STEMI followed by NSTEMI with progressive ventricular dysfunction, requiring repeated percutaneous coronary intervention. The second patient presented with unstable angina and severe 3-vessel disease, managed successfully with coronary artery bypass grafting. Both cases highlight the cons…

In vivo base editing gene therapy for heterozygous familial hypercholesterolemia: a phase 1 trial.

Heterozygous familial hypercholesterolemia is a common genetic disorder characterized by lifelong elevation of serum low-density lipoprotein cholesterol (LDL-C) and premature atherosclerotic cardiovascular disease. YOLT-101 is an investigational in vivo gene therapy that uses adenine base-editing technology, delivered via GalNAc-modified lipid nanoparticles to inactivate PCSK9 and achieve sustained LDL-C reduction. Here we report interim results from an ongoing clinical trial evaluating primary (safety and tolerability) and secondary (lowering of PCSK9 and LDL-C levels) outcomes of a single in…

Opportunities and Challenges in Translating Genomics into Population Health Impact: Lessons from Familial Hypercholesterolemia.

BACKGROUND: Familial hypercholesterolemia (FH) is a common, autosomal dominant genetic disorder characterized by lifelong elevations in low-density lipoprotein cholesterol and a markedly increased risk of premature atherosclerotic cardiovascular disease. Despite a well-defined genetic architecture, highly effective therapies, and strong evidence demonstrating the benefits of early identification and treatment, FH has historically remained underdiagnosed and undertreated worldwide. This persistent gap highlights broader challenges in translating genomic discovery into measurable population heal…

Case Report: Compound heterozygous familial hypercholesterolemia in a pediatric patient with multiple cutaneous xanthomas.

Familial hypercholesterolemia (FH) is an inherited disorder of lipid metabolism characterized by markedly elevated plasma low-density lipoprotein cholesterol (LDL-C) levels, formation of xanthomas, and early-onset atherosclerotic cardiovascular disease (ASCVD). This report describes a rare pediatric case of compound heterozygous FH (cHeFH), in which two mutations were identified in the low-density lipoprotein receptor (LDLR) gene through whole-exome sequencing (WES), including c.682G > A and the rarely reported c.1187-10G > A variant. This case provides key insights into the association betwee…

Homozygous familial hypercholesterolemia, experience with Evinacumab treatment in two Mexican pediatric patients: case report.

Homozygous familial hypercholesterolemia (HoFH) is a rare and life-threatening genetic disorder characterized by extremely elevated low-density lipoprotein cholesterol (LDL-C) levels from birth, leading to accelerated atherosclerotic cardiovascular disease and premature mortality. Conventional lipid-lowering therapies often provide insufficient LDL-C reduction, particularly in patients with minimal or absent LDL receptor (LDLR) function. Evinacumab, an angiopoietin-like protein 3 (ANGPTL3) inhibitor, lowers LDL-C independently of LDLR activity and represents a major therapeutic advance. Here w…

Salvianolic acid B attenuates hypercholesterolemia via modulating the gut microbiota and bile acid metabolism.

Regulation of bile acid metabolism offers an effective approach of reducing hypercholesterolemia. Salvianolic acid B, a phenolic acid compound found in salvia miltiorrhiza Bge, has therapeutic effects on cardiovascular diseases. Nevertheless, insufficient is known concerning the impact on hypercholesterolemia. This study has investigated the cholesterol-lowering effect of salvianolic acid B and delved into its potential mechanism. Hypercholesterolemia mice model was induced by a high-cholesterol diet. We evaluated lipid parameters in serum and liver and examined pathology changes and gene expr…

Oligogenic Familial Hypercholesterolemia Treated by Combination Therapy of Statin, Ezetimibe, PCSK9 Inhibitor, and Lomitapide.

We encountered a 40-year-old man diagnosed with homozygous familial hypercholesterolemia (FH) based on clinical findings. The initial low-density lipoprotein (LDL)-cholesterol level was 393 mg/dL. He underwent coronary artery bypass graft (CABG) surgery for three-vessel disease. Genetic testing revealed a pathogenic variant in the LDL receptor (LDLR) and a missense variant in apolipoprotein E (APOE), known as APOE4, leading to the diagnosis of oligogenic FH. His LDL-cholesterol level was well controlled by the introduction of a proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor an…

Prevalence of genetically diagnosed familial hypercholesterolemia in Vietnamese patients with premature acute myocardial infarction.

Familial hypercholesterolemia (FH) is a genetic disorder that results in elevated low-density lipoprotein cholesterol (LDL-C) levels, which manifest early in the first decades of life. It is a major cause of premature coronary artery disease worldwide, leading to significant public health challenges. The prevalence of genetically determined FH in patients with premature coronary artery disease remains underestimated, particularly in developing countries. This study aimed to assess the prevalence of genetically defined FH in Vietnamese patients with premature acute myocardial infarction (AMI) i…

Nailfold capillaroscopy reveals early peripheral microcirculation abnormalities in children affected by heterozygous familial hypercholesterolemia.

BACKGROUND: nailfold capillaroscopy (NCF) is a non-invasive imaging technique to seek peripheral microcirculation abnormalities in children and adults. Familial hypercholesterolemia is a genetic disorder caused by mutations capable of increasing blood levels of low-density lipoproteins cholesterol (LDL-C), thus triggering early atherosclerosis. The study aims at evaluating peripheral microcirculation in children with heterozygous familial hypercholesterolemia (HeFH) by means of NFC in comparison with healthy peers and at searching for possible correlations between these abnormalities and patie…

Increased interleukin-6 and TP53 levels in rotator cuff tendon repair patients with hypercholesterolemia.

BACKGROUND: A previous study reported that hyperlipidemia increases the incidence of tears in the rotator cuff tendon and affects healing after repair. The aim of our study was to compare the gene and protein expression of torn rotator cuff tendons in patients both with and without hypercholesterolemia. METHODS: Thirty patients who provided rotator cuff tendon samples were classified into either a non-hypercholesterolemia group (n=19, serum total cholesterol [TC] <200 mg/dL) and hypercholesterolemia group (n=11, serum TC ≥240 mg/dL) based on their concentrations of serum TC. The expression of…

Gestational hypercholesterolemia programs hepatic steatosis in a sex-specific manner in ApoE-deficient mice.

Maternal hypercholesterolemia (MHC), a pathological condition characterized by an exaggerated rise in maternal serum cholesterol during pregnancy, may influence offspring hepatic lipid metabolism and increase the risk of nonalcoholic fatty liver disease (NAFLD). As NAFLD is characterized by a sexual dimorphic response, we assessed whether early-life exposure to excessive cholesterol influences the development of NAFLD in offspring and whether this occurs in a sex-specific manner. Female apoE-/- mice were randomly assigned to a control (CON) or a high cholesterol (CH; 0.15%) diet prior to breed…
