SAPHO syndrome in an adolescent: a clinical case with unusual severe systemic impact.

SAPHO (synovitis, acne, pustulosis, hyperostosis, and osteitis) syndrome includes both dermatological and rheumatologic symptoms. Being a rare condition, the diagnosis is frequently late. The authors report a case of a 13-year-old boy diagnosed with synovitis, acne, pustulosis, hyperostosis, and osteitis syndrome with unusual severe systemic repercussions. The patient presented with acne conglobata, inability to walk due to pain and weakness and weight loss. Bone scintigraphy was suggestive of sacroiliitis, and lumbar spine x-ray showed signs of hyperostosis. His clinical state improved after …

[SAPHO syndrome and aseptic osteitis].

Palmoplantar pustulosis and severe acne are sometimes associated with aseptic skeletal lesions, but such skeletal conditions can be observed without skin lesions. The acronym SAPHO (Synovitis, Acne, Palmoplantar pustulosis, Hyperostosis, Osteitis) has been suggested for this cluster of manifestations. The most frequent site of the disease is the upper anterior chest wall, characterized by predominantly osteosclerotic lesions, hyperostosis, and arthritis of the adjacent joints. Osteosclerosis of the vertebral bodies, hyperostosis, and erosions of the vertebral plates can be encountered. Unilate…

[The enthesopathic shoulder].

61 shoulders of rheumatoid diseases, 23 of ankylosing spondylo-arthritis, 22 of psoriatic rheumatism and 30 of hyperostoses (Forestier's disease) were analysed and compared. There is a very significant difference between rheumatoid disease, on the one hand, and the other diseases, on the other hand. In ankylosing spondylo-arthritis, psoriatic rheumatism and hyperostosis, enthesophytes are observed; it is not the case in rheumatoid disease (unless associated with hyperostosis). The most frequent entesophyte is the one at the acromial insertion of the acromio-coracoid ligament ("bearded acromion…

[Spondarthritis hyperostotica pustulo-psoriatica: nosologic study with clinical and radiologic presentation in relation to the SAPHO syndrome].

Spondarthritis hyperostotica pustulo-psoriatica (Spond.hyp.pp-Schilling), corresponding nosologically to pustulotic arthroosteitis, is a dermato-skeletal "double system" disease of adults. It consists of the triad (a) palmo-plantar pustulosis (Ppp) or, alternatively, Königsbeck-Barber-type psoriasis, (b) sternocostoclavicular hyperostosis (SCCH), and (c) truncal-skeletal changes with syndesmophite-like, hyperostotic and/or parasyndesmophite-like ossifications of layers of the anterior vertebral ligament taken together in the sense of a desmophytal hyperostosis. There is also a potential for sc…

The SAPHO syndrome in a 16-year-old boy: coincidence of acne conglobata and osteoarthritis.

A 16-year-old boy developed arthritis and osteitis (anterior thoracic wall, ankle of left foot, spinal column) within 5 weeks of an exacerbation of acne. Both the local skin condition and the locomotory system improved after treatment, but sternocostoclavicular hyperostosis persisted. The syndrome of acne, pustulosis, hyperostosis, and osteitis (SAPHO syndrome) identified in 1987, represents an entity usually classified as one of the seronegative spondylarthrites close to ankylosing spondylarthritis and psoriatic arthritis.

[Spondylosis hyperostotica--a rare cause of dysphagia, dyspnea, and neck-shoulder pain].

Anterior or seldom posterior hyperostosis of the spine (Forestier disease, "diffuse idiopathic skeletal hyperostosis") is a common finding in elderly patients but is rarely associated with clinical symptoms. In some cases there is a wide range of symptoms which may not lead primarily to the correct diagnosis. We report a case in which the diagnosis was first made based on an unusual combination of symptoms after ENT and neurological examinations revealed no evidence for the underlying cause.

[Significance of cranial hyperostoses for the neurologic-psychiatric practice].

The present paper attempts to comment on the relation between diffuse concentric hyperostoses of the cranial vault and neurologic and psychiatric symptoms in a male aged 42 and a woman aged 23 when death occurred. In the past, similar discussions mainly concerned the hyperostosis frontalis interna (Hfi), although the demonstration of a true relation is difficult in the Hfi because of its late occurrence at an advanced age (60-70 years), in which mental disturbances due to other pathological circumstances are frequent as well. The diffuse concentric hyperostosis in young individuals has compara…

SAPHO syndrome with a purely osteolytic form: A case report.

SAPHO (synovitis, acne, pustulosis, hyperostosis, and osteitis) syndrome is a rare, chronic autoinflammatory disorder that can present with a constellation of cutaneous and osteoarticular symptoms. Osteodestructive lesions are not pathognomonic, whereas hyperostosis and osteitis are the most prominent imaging findings. We report the case of a man with osteolytic changes of the lumbar vertebra and a history of palmoplantar pustulosis. Biopsy revealed no neoplasm, suggesting SAPHO syndrome. Our case demonstrates that knowledge of atypical radiologic findings is necessary for the diagnosis of SAP…

Two cases of musculoskeletal syndrome associated with acne.

We report two cases of musculoskeletal syndrome (myalgia, arthralgia, arthritis, hyperostosis) that developed in male adolescents who had severe acne (acne conglobata and acne fulminans). In both patients the hyperostosis of the right clavicle aroused the suspicion of a bone tumor or osteomyelitis, which was excluded by histologic examination. Radiologic and laboratory characteristics of musculoskeletal syndrome associated with acne conglobata and acne fulminans are reviewed as well as isotretinoin therapy. The problems of differential diagnosis are also discussed.

Melorheostosis: segmental osteopoikilosis or a separate entity?

PURPOSE: Melorheostosis is a progressive hyperostotic bone disease that commonly affects the appendicular skeleton. Melorheostosis has a significant degree of overlap with other hyperostosis conditions including osteopoikilosis and likely represent varying degrees of a clinical spectrum. METHODS: This is a report of 2 patients with melorheostosis who presented with different clinical presentations and involvement of different anatomic locations. RESULTS: One of the patients presented with foot size asymmetry along with intermittent foot pain and limping. This patient also had irregular nonbl…

[Surgery for hyperostotic cranial orbital meningiomas].

The paper analyzes surgical techniques for removal of hyperostotic cranial orbital meningiomas in 36 patients operated on in 1998 to 2000. In 19 cases hyperostosis extends to the upper and lower lid slits without involving the optic canal. It also spreads to the ethmoidal sinus in 6 cases to the frontal sinus in 3, and to the maxillary one in 2 patients. In 3 patients, hyperostosis was beyond the wing of the basic bone, by involving the temporal and frontal squamous. In 19 cases, hyperostosis was resected and the tumor was removed without creating any additional bone flaps. To make an addition…

Dysphagia and Forestier's disease.

Dysphagia is a common complaint of patients seen by otolaryngologists. Cervical osteophytes have been widely reported as a cause of dysphagia. Recently, Forestier's disease (vertebral ankylosing hyperostosis or diffuse idiopathic skeletal hyperostosis) has also been identified as a cause of dysphagia. Two patients had dysphagia due to Forestier's disease. Their barium esophagograms demonstrated narrowing of the esophagus due to the vertebral osteophytes and their computed tomographic scans show the extent of the osteophyte deformity.

Professional awareness is needed to distinguish between child physical abuse from other disorders that can mimic signs of abuse (Skull base sclerosis in infant manifesting features of infantile cortical hyperostosis): a case report and review of the literature.

BACKGROUND: Infantile cortical hyperostosis is characterised by hyperirritability, acute inflammation of soft tissue, and profound alterations of the shape and structure of the underlying bones, particularly the long bones, mandible, clavicles, or ribs. CASE PRESENTATION: We report on a clinical case of a 3-months-old baby girl of non-consanguineous parents. Multiple long bone swellings were the motive of referral to our department for clinical evaluation. Radiographic documentation was consistent with infantile cortical hyperostosis (Caffey disease). Interestingly, skull base sclerosis assoc…

Occurrence of bilateral sacroiliitis and ossification of anterior spinal ligaments in the same patient.

Abstract Enthesopathy and ligament calcification may occur in both the degenerative and inflammatory types of diffuse idiopathic skeletal hyperostosis and ankylosing spondylitis. Despite their fairly frequent occurence in rheumatic disease care settings, few reports of their presence in the same patient have appeared in the literature. In this report, we present the case of a male patient with chronic low back pain, who had both ossification of anterior spinal ligaments and bilateral sacroiliac joint ankylosis, and discuss the known pathogenesis of diffuse idiopathic skeletal hyperostosis.

Stenosis of the cervical canal in craniodiaphyseal dysplasia.

Craniodiaphyseal dysplasia (CDD) is a rare sclerosing bone disorder, the severity of which depends on its phenotypic expression. Hyperostosis can cause progressive foraminal stenosis leading to palsy of cranial nerves, epilepsy and mental retardation. We report the only case of CDD in an adult, with stenosis of the cervical canal leading to quadriparesis as a late complication of hyperostosis, and describe the problems associated with its treatment. Although the syndrome is rare, its pathophysiological and therapeutic considerations may be applicable to the management of stenosis of the spinal…

Craniometaphyseal dysplasia: operative findings and treatment.

Craniometaphyseal dysplasia (CMD) is a genetic bone disorder involving an abnormality of modeling of the long bones and sclerosis of the cranium. Both conductive and sensorineural hearing may be frequently associated with this syndrome. Conductive hearing loss is due to attic fixation of the lateral ossicular chain and hyperostosis formation with stapes ankylosis. Two patients with CMD were treated surgically. Two of three ears operated on had closure of the air-bone gap, whereas the third ear failed ossicular reconstruction because of hyperostosis of the promontory with deepening of the oval …

[Computerized tomography imaging of the lumbar spine in arthroosteitis pustulosa (case report)].

Computed tomography of lumbar spine in a case of pustulotic arthroosteitis demonstrates unusual proliferative changes of syndesmophytes and of the vertebral body. The osseous structures look "frayed out" and resemble the changes of sternoclavicular hyperostosis in early stages. A three dimensional picture shows that these proliferative changes apparently consist of irregular strips of bone. In addition, the vertebral body is surrounded by a soft tissue structure which probably indicates a perivertebral inflammatory process. Radiological changes of the lumbar spine in pustulotic arthroosteitis …

Primary ectopic meningioma.

Primary ectopic meningiomas are rare. A case of a 16-year-old male who presented with a large mass in temporofrontal region is reported. X-ray skull showed soft tissue shadow with hyperostosis of frontal and temporal bone. CT scan of brain demonstrated a markedly enhancing lesion and bony hyperostosis with no intracranial component. Total excision of tumour with hyperostotic bone was done. Patient is well without any evidence of recurrence two years after surgery. Relevant literature is reviewed.
