The causative agent of thimerosal allergy (sodium ethylmercury thiosalicylate) has not previously been thoroughly investigated. To evaluate whether the organic mercury component or the thiosalicylic acid molecule induces thimerosal sensitization, 23 patients positive to thimerosal were patch tested with ethylmercuric chloride, thiosalicylic acid and 8 different derivatives of mercury. Delayed hypersensitivity retest reaction 3 and 6 weeks after induction of allergic and irritant inflammation, was studied in 13 females with known hypersensitivity to nickel. An increased retest reaction compared to controls was observed only in sites of earlier specific allergic inflammation. Also a general down-regulation of the degree of hypersensitivity was observed at retesting.

We report two connatal cases of Pelizaeus-Merzbacher disease (PMD) with cellular ultraviolet (UV)-hypersensitivity. Thirty-seven patients with chronic otitis externa were investigated for contact hypersensitivity to 49 agents using standardized patch testing. Positive reactions were found in 22 (58%) patients. This incidence is higher than the 40% found in the only previous similar study and is within the range of 32-72% found in patients with contact dermatitis. The average age of subjects showing sensitivity was 56.7 years compared with 34.9 years for those with negative tests. Seven subjects, who experienced systemic allergic reactions after the ingestion of a newly marketed food supplement, were evaluated to identify the responsible ingredient. Skin testing with extracts prepared from ingredients in the food supplements revealed marked sensitization of all of the subjects to cottonseed protein. Four patients and three healthy volunteers were submitted to delayed hypersensitivity skin tests to candidin, tuberculin, streptokinase/streptodornase (SK/SD) and mumps antigen before and after a 1 week treatment with nifedipine 10 mg q. 8h. A decrease in the scores of induration and erythema was observed, especially in response to SK/SD antigen challenge. We conclude that nifedipine may induce an anergy status in delayed hypersensitivity skin tests.

A case of tuberculoid leprosy is reported in a middle aged female, who presented with generalised erythematous maculopapular rash which followed ampicillin therapy. In spite of extensive and severe involvement of the skin all over the body by erythema and rash, the hypopigmented patch of leprosy on the face did not show any erythema or rash and stood out more clearly as an island of pale area in the midst of large area of erythema on the face. The possible mechanism of non development of erythema and rash in the patch is discussed.
