Are Clinical Decision Support Systems Compatible with Patient-Centred Care?

Few, if any, of the Clinical Decision Support Systems developed and reported within the informatics literature incorporate patient preferenc … ence-based recommendations are arguably in breach of both the reasonable patient legal standard for informed consent and simple ethical principles. The answer is not to localise, but personalise, decisions through the use of preference-sensitive multi-criteria decision support tools engaged with at the point of care.

Tracheostomy decannulation methods and procedures for assessing readiness for decannulation in adults: a systematic scoping review.

Despite the undisputable benefits of tracheostomy, it has been reported to have links with impaired communication, reduced quality of life a … atient groups. The common themes that emerged in the assessment for readiness for decannulation are informed consent, clinical stability, airway patency, physiological decannulation, swallowing assessment, level of consciousness, effectiveness of cough and clearance of secretions. In conclusion, the current body of evidence is inadequate and requires further resea …

Clinical Vignettes: Donor-Derived Infections.

Patients undergoing solid organ transplantation (SOT) may acquire infections from the transplanted organ. Routine screening for common infections are an established part of the pretransplant evaluation of donors and recipients. Likewise, strategies exist for prophylaxis and surveillance for common donorassociated infections including hepatitis B, CMV and EBV. However, despite advances in diagnostic testing to evaluate the infectious risk of donors, unanticipated transmission of pathogens occurs, particularly when donors are asymptomatic or have subtle or unusual manifestations of a transmissib

Medical and ethical challenges during the first successful human kidney transplantation in 1954 at Peter Bent Brigham Hospital, Boston.

BACKGROUND: The first successful major organ transplantation, a kidney transplant, took place on December 23, 1954, at Peter Bent Brigham Hospital, Boston, Massachusetts. This was the beginning of major organ transplants commonly performed today, heralding one of the most significant achievements of medicine. A half-century later, heart, liver, limb, and even face transplants have become standard practice. In this report, we explore details of the preparations, the ethical dilemmas and the unknowns, and how these issues were addressed and overcome. METHODS: Published works, hospital records,

Ethics of facial transplantation revisited.

PURPOSE OF REVIEW: There have been 26 cases of facial transplantation reported, and three deaths, 11.5%. Mortality raises the issue of risk … ce abuse. SUMMARY: Early publications on ethical aspects of face transplantation focused mainly on informed consent. Many other ethical issues have been identified, including lack of coercion, donor family consent and confidentiality, respect for the integrity of the donor's body, and financial promotion of the recipient and transplant team, as well as the cost t …

[Assisted suicide - medical, legal, and ethical aspects].

Unlike in most European countries, assisted suicide is not illegal in Switzerland. The number of assisted suicides procured by right-to-die organisations such as Exit or Dignitas has sharply increased in the last twenty years. Central part of the doctor's involvement is the prescription of a lethal dose of sodium pentobarbital. In doing so, the doctor has to apply to the rules of medical due care. A proper examination of the patient is required, who must be informed about his diagnosis, about the expected prognosis, and about different treatment options. Verification of the patient's decisiona

Money for consent--psychological consideration.

CONTEXT: Regarding the increasing gap between demand and supply of donor organs the question is increasingly discussed, if families of organ donors should receive a financial incentive for consenting to organ donation. However, little attention has been paid to the psychological consequences of such incentives. OBJECTIVE: We discuss the question of financial incentives for families of presumed organ donors in the light of relevant psychological theories. DATA SOURCES AND STUDY SELECTION: Overview of the psychological literature. Only well established theories were included. CONCLUSIONS: We

[Patient participation in the Cochrane Collaboration--barriers, experience, and concepts in Germany].

The aim of the Cochrane Collaboration (CC) is to bridge the gap of information transfer between the clinician and the patient. For this purpose, the CC pursues since years the concept of involving the consumers in the process of collaborative review groups. In spite of a positive experience altogether, some barriers (for example the communication between the scientists involved in CC review groups and the patients) remain to be overcome. To improve the information transfer, the Cochrane Hematological Malignancies Group (CHMG) has built a consumer network within the initiatives of the German Ca

A stronger policy of organ retrieval from cadaveric donors: some ethical considerations.

Taking organs from dead people seems, prima facie, to raise fewer ethical complications than taking organs from other sources. There are, however, serious ethical problems in taking organs from the dead unless there is premortem evidence that this is what the deceased would have wanted, or at least, not have objected to. In this paper we will look at a "strong" opting out policy as proposed by John Harris. We will argue that people can be harmed after their death and that the posthumous removal of organs against their expressed wishes is one form that such harm might take. We also argue that H
