Ileal atresia and total colonic aganglionosis.

Herein, the authors report a case of ileal atresia who failed to have good bowel function after primary anastomosis. After the leakage of the revised anastomosis, a thorough pathological review found distal aganglionosis. An ileostomy followed by an ileocolic patch operation were performed for temporary decompression, awaiting intestinal adaptation. A definitive pull-through was performed, eight months later. The child also had bilateral cleft lip together with complete cleft palate, and hemivertebrae of the thoracic spines. A cluster of malformations is unusual in a single patient.

Diffuse neonatal hemangiomatosis: a new constellation of findings.

Diffuse neonatal hemangiomatosis is a rare, frequently fatal disease characterized by multiple cutaneous and visceral hemangiomas. The skin, liver, lungs, intestines, and central nervous system are the most commonly affected organs. Death can result from high-output cardiac failure as a result of arteriovenous shunting. In this report we present a new constellation of findings in a patient with diffuse neonatal hemangiomatosis, namely, an absent corpus callosum, ectopia cordis (sternal agenesis) with median abdominal raphe, and tricuspid atresia.

Experimental congenital malformations.

Traditional approaches to human and experimental teratology are briefly described, with roles of pathologists indicated. Some approaches to experimental teratology which pathologists might use are then described and illustrated. These include identification of subclasses of malformation types, study of the chronologic sequence of maldevelopment, study of embryo death, and examination of tumor-malformation relationships. The malformations used to illustrate these approaches are cleft palate, anencephaly, hydrocephalus, and intestinal atresia. The final section deals with general methodology in …

Neonatal intestinal obstruction associated with situs inversus totalis: two case reports and a review of the literature.

BACKGROUND: The association of neonatal intestinal obstruction with situs inversus totalis is extremely rare with only few cases reported in … CONCLUSIONS: Clinicians should have high index of suspicion for malrotation with midgut volvulus or intestinal atresias in neonates of situs inversus presenting with bilious vomiting. The surgical treatment should follow the same surgical principles. In situs inversus, because of transposition of viscera, midgut volvulus may occur in an anticlockwise direction, henc …

Second trimester prenatal findings in duodenal and esophageal atresia without tracheoesophageal fistula.

The combination of duodenal atresia and esophageal atresia without tracheoesophageal fistula leads to a closed loop of bowel involving the distal esophagus, stomach, and duodenum. Prenatally, this association of anomalies is visualized as a characteristic dilated C-shaped fluid collection in the fetal abdomen. We report three cases of the association of duodenal and esophageal atresia without tracheoesophageal fistula, identified sonographically in the second trimester of pregnancy.

Large intestine embryogenesis: Molecular pathways and related disorders (Review).

The large intestine, part of the gastrointestinal tract (GI), is composed of all three germ layers, namely the endoderm, the mesoderm and the ectoderm, forming the epithelium, the smooth muscle layers and the enteric nervous system, respectively. Since gastrulation, these layers develop simultaneously during embryogenesis, signaling to each other continuously until adult age. Two invaginations, the anterior intestinal portal (AIP) and the caudal/posterior intestinal portal (CIP), elongate and fuse, creating the primitive gut tube, which is then patterned along the antero‑posterior (AP) axis an

Defective upper gastrointestinal function after repair of combined esophageal and duodenal atresia.

BACKGROUND: Both esophageal atresia (EA) and duodenal atresia (DA) involve deficient anti-reflux barrier, poor esophageal function and eventually, duodenogastric reflux. This study aims at examining the upper gastrointestinal functional status in a cohort of patients with both EA and DA. METHODS: A retrospective survey of patients treated for EA and DA between 1965 and 2012 was conducted. Clinical charts, office visits, imaging, upper gastrointestinal endoscopy and esophageal pH metry/impedance were used to assess the long-term condition of the esophagus, the presence of gastroesophageal refl

[Intrauterine necrosis in closed gastroschisis].

INTRODUCTION: Strangulation of the intestine as the result of compression of its blood supply in a tightly closed gastroschisis defect is a very rare occurrence. CLINICAL CASES: We present the cases of two newborn patients who had extra-abdominal infarcted bowel and intra-abdominal jejunal atresia due to vascular compression for gastroschisis defect. One was associated with colonic, probably acquired aganglionosis. Both had similar clinical courses. CONCLUSIONS: This association is very uncommon. Prognosis of this complex is very poor.

Duodenal web and pancreas divisum causing pancreatitis in an adult.

Duodenal malformations are the third commonest cause of intestinal obstruction in infants. A spectrum of intrinsic obstructive lesions within the duodenum ranges from atresia to congenital bands. Rarely, duodenal malformations may first present in adulthood. Less than 70 cases of duodenal web presenting in an adult have been reported in the literature. In 10 patients the presentation was associated with pancreatitis. We report a case of congenital duodenal web associated with pancreas divisum which first presented in an adult with the clinical characteristics of recurrent acute pancreatitis.

Long-term gastrointestinal morbidity in patients born with gastroschisis: A national register-based cohort study.

OBJECTIVES: In gastroschisis, the intestines are exposed to amniotic fluid during pregnancy. The defect in the abdominal wall may also compress the mesentery and impair the intestinal blood supply. There is a varying degree of intestinal damage at birth. Complex gastroschisis is defined as gastroschisis with severe complications such as intestinal atresia, volvulus, necrosis and perforation. The aim of this study was to investigate long-term gastrointestinal morbidity and compare simple and complex cases. METHODS: A nation-wide retrospective cohort study with data from national registers was …
