The Forgotten Fetal Channel: Guidewire Entry Into Umbilical Vein During Central Line Placement in a Neonate.

Central venous catheterization in neonates presents unique challenges due to transitional circulatory physiology and potential persistence of fetal vascular structures. We describe a rare intraoperative observation in a 4-day-old baby boy weighing 1.8 kg, diagnosed with a large retroperitoneal mass causing abdominal compartment syndrome. The patient underwent emergency exploratory laparotomy. During ultrasound-guided internal jugular vein catheterization in the setting of an open abdomen, the guidewire was unexpectedly visualized emerging through the umbilical vein. This unusual finding indica

A case of total remnant pancreas resection performed for gastric-type IPMC after pancreaticoduodenectomy for intestinal-type IPMC.

Recurrence of intraductal papillary mucinous neoplasms (IPMNs) in the remnant pancreas after surgery is a significant clinical challenge. A 68-year-old woman was incidentally found to have a 50-mm mixed-type intraductal papillary mucinous carcinoma (IPMC) in the pancreatic head during a health check. She underwent subtotal stomach-preserving pancreaticoduodenectomy with pancreatogastrostomy. Pathology revealed intestinal-type IPMC (pStage IA) with venous invasion and negative margins. Adjuvant oral S-1 chemotherapy was administered for six months. Eighteen months later, she developed rapid inc

Polymicrobial Arthritis Following a Domestic Cat Bite Involving Rahnella aquatilis in an Immunocompetent Patient.

Cat bites frequently lead to polymicrobial infections due to deep puncture wounds that inoculate oral flora into poorly oxygenated tissues. While Pasteurella multocida is the most commonly implicated organism, environmental and atypical pathogens may also play a role, yet often go unrecognized. This article reports a rare case of polymicrobial septic arthritis caused by a domestic cat bite in an immunocompetent adult, with isolation of Rahnella aquatilis, a freshwater-associated Enterobacterales species not previously reported in this context. A 33-year-old immunocompetent male presented with

Facial Nerve Conduction Block during Vestibular Schwannoma Surgery: A Rare and Unclarified Transient Neural Dysfunction.

Preservation of facial nerve function while achieving sufficient tumor removal is a key objective in vestibular schwannoma surgery, and continuous facial nerve monitoring provides real-time, quantitative, and functional assessment of facial nerve during resection. In our experiences of more than 1,900 vestibular schwannoma surgeries, we have occasionally observed an abrupt and marked decrease in monitoring amplitude without any high-risk manipulation such as direct nerve dissection. This rare phenomenon, illustrated by 2 recent cases in this study, was transient or unrelated to postoperative f

The Wandering Gallbladder: A Rare Case of Acute Cholecystitis in the Falciform Ligament.

Ectopic gallbladders are uncommon congenital variants that can complicate management of acute cholecystitis. They are rare, and their unusual positioning can pose diagnostic and intraoperative challenges. Because of their rarity, many ectopic gallbladders are not suspected until the time of surgery, increasing the potential risk of bile duct injury and prolonging operative time. Here, we present the case of a 41-year-old woman presenting with severe overnight epigastric pain following six months of intermittent pain. Examination revealed right upper quadrant and epigastric tenderness with a po

Solitary pituitary metastasis from breast cancer masquerading as a pituitary macroadenoma: a report of 2 rare cases.

BACKGROUND: Breast cancer is one of the most common malignant tumors, occurring in the mammary glands, which often metastasizes to bones, lungs, and liver. However, pituitary metastasis (PM) originating from breast cancer is a rare phenomenon that can easily be mistaken for benign pituitary macroadenoma. CASE DESCRIPTION: This report details two cases of middle-aged and elderly Chinese women who presented with serious neurological symptoms, each with a history of breast malignancy. Both patients underwent magnetic resonance imaging (MRI), which showed a lesion in the sellar region, suggesting

Iatrogenic Calcinosis Cutis in the Pediatric Patient: Case Report and Literature Review.

Calcinosis cutis is the term used to describe the deposition of calcium compounds within the skin and subcutaneous tissue, which can occur after the administration of intravenous calcium compounds. Its etiology is broad, and the clinical presentation is variable, creating a diagnostic challenge. Although iatrogenic calcinosis cutis is extremely uncommon, awareness and early diagnosis of this entity can reduce the risks of severe complications, including soft tissue damage, restricted joint mobility, and even nerve compression. Clinical suspicion should prompt a thorough review of the medical h

Type A aortic dissection and pheochromocytoma: an indirect consequence of the Covid-19 pandemic.

Type A aortic dissection is a cardiovascular emergency. Its incidence seems to have increased in the last few years; it is not clear whether this is a consequence of the ageing population or better awareness of the diagnosis (Erbel R, Alfonso F, Boileau C, Dirsch O, Eber B, Haverich A et al.; Task Force on Aortic Dissection, European Society of Cardiology. Diagnosis and management of aortic dissection Task Force on Aortic Dissection, European Society of Cardiology. Eur Heart J 2001;15;22:1642-81). Acute type A aortic dissection is often lethal without urgent surgical treatment with mortality r

Thyroidectomy in Australia 2022: lessons from 21,000 consecutive cases.

In this article, we aim to describe our modern-day approach to total thyroidectomy, detailing the subtle refinements of our technique, as it has evolved over three decades and 21 000 cases. Since Delbridge's seminal paper in 2003, the major changes to our approach include a retrograde approach to the recurrent laryngeal nerve that allows dissection of the distal RLN from fascial bands within the ligament of Berry before medialisation of the thyroid lobe. Routine use of intraoperative nerve monitoring systems has increased our awareness of temporary neuropraxia, facilitated a reduction in the r

Improvements in outcomes have … on so as to expedite emergence and early recovery, and their use is advocated for the prevention of intraoperative awareness in the setting of administration of total intravenous anesthesia and neuromuscular blockade. Whether their use can minimize, or prevent, longer term adverse events is a matter of debate. In this narrative review of the most recent literature, we pro …

FROM DIABETES INSIPIDUS TO SELLAR XANTHOGRANULOMA - A "YELLOW BRICK ROAD" DEMANDING TEAM-WORK.

Xanthogranulomas are inflammatory lesions exceptionally rarely occurring in the sellar region. Sellar xanthogranulomas (SXG) result from secondary hemorrhage, infarction, inflammation or necrosis upon existing craniopharyngioma (CP), Rathkès cleft cyst (RCC) or pituitary adenoma (PA), or represent a stage in xanthomatous hypophysitis evolution. "Pure SXG" are independent of a preexisting lesion. A 70 year old male patient, laryngeal cancer survivor, presented with central diabetes insipidus (CDI). MRI revealed an intra-suprasellar mass of uncertain origin. Transsphenoidal surgery resulted in a

Temporal trends in the use of diagnostic imaging for inpatients with pancreatic conditions: How much ionizing radiation are we using?

BACKGROUND: Low-dose ionizing radiation from medical imaging has been indirectly linked with subsequent cancer and increased costs. Computed tomography (CT) is the gold standard for defining pancreatic anatomy and complications. Our primary goal was to identify the temporal trends associated with diagnostic imaging for inpatients with pancreatic diseases. METHODS: Data were extracted from the Healthcare Cost and Utilization Project Nationwide Inpatient Sample (NIS) database from 2000 to 2008. Pancreas-related ICD-9 diagnostic codes were matched to all relevant imaging modalities. RESULTS: Be

Postoperative (pressure) alopecia following sacrocolpopexy.

Postoperative alopecia is a rare occurrence seen after a variety of surgical procedures performed under general anesthesia. The speculated cause is pressure-induced ischemia due to prolonged head immobilization. This case describes a patient who developed this complication after undergoing sacrocolpopexy. A 57-year-old postmenopausal Caucasian female was consented to undergo a robotic-assisted sacrocolpopexy, perineoplasty, and midurethral sling with possible conversion to an open procedure. The indication was symptomatic proximal and distal rectocele with foreshortened vagina. It was converte

Primary synovial sarcoma of the thyroid gland: a CARE compliant case report and literature review.

RATIONALE: Synovial sarcoma is a subtype of soft tissue sarcoma. Synovial sarcoma in the head and neck region is relatively unusual. Primary synovial sarcoma of the thyroid gland (PSST) is first reported in 2003 by Inako Kikuchi. PSST is extremely rare with only 15 cases documented globally. PSST shows rapid disease progression and a relatively poor prognosis. However, diagnosis and therapy are challenging for clinical surgeons. In this article, we reported the 16th PSST case and reviewed the PSST cases globally for further clinical application. PATIENT CONCERNS: The patient was referred to u
