Distinguishing IDH mutation status in gliomas using FTIR-ATR spectra of peripheral blood plasma indicating clear traces of protein amyloid aggregation.

BACKGROUND: Glioma is a primary brain tumor and the assessment of its molecular profile in a minimally invasive manner is important in determining treatment strategies. Among the molecular abnormalities of gliomas, mutations in the isocitrate dehydrogenase (IDH) gene are strong predictors of treatment sensitivity and prognosis. In this study, we attempted to non-invasively diagnose glioma development and the presence of IDH mutations using multivariate analysis of the plasma mid-infrared absorption spectra for a comprehensive and sensitive view of changes in blood components associated with th

Long-term survival in patients with IDH-wildtype glioblastoma: clinical and molecular characteristics.

BACKG ROUND: Glioblastoma is an aggressive tumor that has a dismal prognosis even with multimodal treatment. However, some patients survive longer than expected. The objective of this study was to revisit patients diagnosed with glioblastoma according to the 2021 WHO classification and analyze clinical and molecular characteristics associated with long-term survival (LTS). METHODS: We retrospectively analyzed 120 IDH-wildtype glioblastomas operated on at our institution between 2013 and 2018. We divided them into LTS patients, surviving more than 3 years, and non-LTS patients, and then compar

Transcription factor networks of oligodendrogliomas treated with adjuvant radiotherapy or observation inform prognosis.

BACKGROUND: Recent international sequencing efforts have allowed for the molecular taxonomy of lower-grade gliomas (LGG). We sought to analyze The Cancer Genome Atlas (TCGA, 2015) gene expression datasets on molecularly defined oligodendrogliomas (IDH-mutated and 1p/19q-codeleted) patients treated with adjuvant radiation or those observed to discover prognostic markers and pathways. METHODS: mRNA expression and clinical information of patients with oligodendroglioma were taken from the TCGA "Brain Lower Grade Glioma" provisional dataset. The role of a monoclonal antibody 11C8B1 as a diagnostic marker of IDH2-mutated sinonasal undifferentiated carcinoma.

IDH2 R172 mutations occur in >80% sinonasal undifferentiated carcinomas ("SNUC") and ~80% of these are R172S and R172T variants. We examined the utility of the monoclonal antibody 11C8B1 to IDH2 R172S in IDH2 R172-mutated tumors to establish an immunohistochemistry protocol as a surrogate method for IDH2 R172S mutation detection. Eighty-eight formalin-fixed paraffin-embedded tumors including 42 sinonasal tumors and a variety of IDH1/2-mutated malignancies were tested by immunohistochemistry. The IDH1/2 mutation status was determined in 86 cases by a targeted massively parallel sequencing MSK-I

Subgrouping of gliomas on the basis of genetic profiles.

Management of gliomas depends on histological diagnosis; there are, however, limitations to the systems presently used. Tumors in the same entity can have different clinical courses, especially when they are diagnosed as WHO grade II-III. Previous studies revealed that genetic subgrouping of gliomas provides useful information that could help establishment of treatment procedures on the basis of the genetic background of the tumors. Recently, the authors analyzed the chromosomal copy number aberrations (CNAs) of adult supratentorial gliomas by comparative genomic hybridization using microdisse

The prognostic significance of TSPO-PET imaging in IDH-mutant glioma: a single-center, retrospective study.

PURPOSE: Clinical prognostication and decision-making in IDH-mutant glioma is increasingly complex, especially with new targeted treatment options like IDH-inhibitors. Individual patient risk stratification for better treatment planning is needed; however, standard prognostic models rely on clinical and histologic parameters as well as MRI, which may not fully reflect the tumor's biological behavior. Positron emission tomography (PET) imaging of the 18 kDa translocator protein (TSPO) is known as surrogate marker of activated microglia and macrophages and enables non-invasive assessment of the

Environment-sensitive fluorescent probes targeting mutant IDH1 for in vivo glioma imaging.

BACKGROUND: Surgery serves as a cornerstone in glioma treatment, yet achieving complete resection remains a significant challenge due to the infiltrative nature of these tumors. Microscopic residual lesions frequently lead to recurrence, whereas excessive resection risks damaging critical neurological functions. Fluorescent imaging technology enables the real-time visualization and precise delineation of glioma margins. However, commonly clinical fluorescent agents for gliomas, such as 5-ALA, fluorescein sodium, and ICG, are limited in their widespread application due to low selectivity and se

Dedifferentiated Chondrosarcoma: Current Advances and Future Perspectives.

Dedifferentiated chondrosarcoma (DDCS) remains one of the most aggressive and therapeutically challenging primary bone malignancies. While surgery continues to represent the cornerstone of treatment for localized disease, outcomes remain poor due to high rates of local recurrence and metastatic progression. The role of adjuvant therapies remains less clear. Radiotherapy may be considered in selected patients with unresectable tumors, positive margins, or for symptom palliation, whereas conventional chemotherapy has demonstrated inconsistent and generally limited benefit despite the use of oste

Impact of Surgical Ventricular Entry on Survival Outcomes in IDH-Mutant Gliomas.

BACKGROUND AND OBJECTIVES: Although some studies suggest ventricular entry (VE) is associated with complications and poor survival in glioblastoma, it remains unclear whether this association applies to isocitrate dehydrogenase (IDH)-mutant gliomas. This study evaluated the impact of VE on progression-free survival (PFS) and overall survival (OS) in these tumors. METHODS: A retrospective analysis of patients with supratentorial IDH-mutant gliomas, treated between 2006 and 2021 at the University of Pittsburgh Medical Center was performed. VE was identified through postoperative imaging review.

Concurrent myelodysplastic malignancies and plasma cell neoplasms; a clinicopathological study with prognostic implications.

Plasma cell neoplasms (PCN) have infrequently been reported in patients with myelodysplastic syndrome (MDS) and even more rarely in those with myelodysplastic syndrome/myeloproliferative neoplasm (MDS/MPN). We report the clinicopathologic features of 26 patients with bone marrow myelodysplasia accompanied by PCN, including 21 patients with MDS and 5 with MDS/MPN. The clinicopathologic features of the MDS/MPN-PCN were compared to those of the MDS-PCN group and 68 cases of MDS/MPN without PCN, respectively. The MDS/MPN-PCN group was notable for increased reticulin fibrosis > grade 1 when compare

SIRT5 alleviates hepatic ischemia and reperfusion injury by diminishing oxidative stress and inflammation via elevating SOD1 and IDH2 expression.

Hepatic ischemia/reperfusion (I/R) injury, a common and unavoidable pathophysiological process during liver transplantation or resection operation, may impede postoperative liver function recovery, and its mechanism and targeted therapy remain largely unknown. SIRT5 is a well-known deacetylase and participates in the regulation of many physiological and pathological processes, including I/R. The role of SIRT5 in I/R is controversial or tissue-specific, restricting I/R progression in the heart while deteriorating injury in the kidney and brain, while its effect on hepatic I/R remains unclear. Loss of MycBP may be associated with the improved survival in 1P co-deletion of lower grade glioma patients.

OBJECTIVES: The chromosome 1p/19q co-deletion is a favorable prognostic factor in patients with low grade glioma. In the current analysis, we examined MycBP expression in low grade glioma. MycBP lies on chromosome 1p. PATIENTS AND METHODS: We evaluated the association between MycBP and overall survival in the TCGA Lower Grade Glioma (LGG) dataset in TCGA (The Cancer Genome Atlas). RESULTS: Loss of MycBP copy number segment expression coincides with co-deletion of 1 P. The deleterious effect of MycBP on survival is significant (p = 0.00006306, hazard ratio 2.02, 95% CI 1.4-2.9). Patients with

Updates in Molecular Pathology of Central Nervous System Gliomas in Adults.

Central nervous system (CNS) tumors are a heterogeneous group of neoplasms divided into two broad categories, glial and non-glial. Non-glial tumors are derived from such diverse structures as the pineal gland, meninges, germ cells, and hematopoietic cells, as well as metastases. Primary glial neoplasms, or those which originate from astrocytes, oligodendrocytes, or ependymal cells, include astrocytomas, oligodendrogliomas, ependymomas, and mixed gliomas. Each entity has a unique morphology and pattern of biologic behavior which portends a distinct prognosis and outcome. Individual outcomes sho

Novel cases of D-2-hydroxyglutaric aciduria with IDH1 or IDH2 mosaic mutations identified by amplicon deep sequencing.

BACKGROUND: Mosaic IDH1 mutations are described as the cause of metaphyseal chondromatosis with increased urinary excretion of D-2-hydroxyglutarate (MC-HGA), and mutations in IDH2 as the cause of D-2-hydroxyglutaric aciduria (D-2HGA) type II. Mosaicism for IDH2 mutations has not previously been reported as a cause of D-2HGA. Here we describe three cases: one MC-HGA case with IDH1 mosaic mutations, and two D-2HGA type II cases. In one D-2HGA case we identified mosaicism for an IDH2 mutation as the genetic cause of this disorder; the other D-2HGA case was caused by a heterozygous IDH2 mutation,
