This article reports on the transhepatic closure of post-Fontan hepatic veins to left atrium fenestration in a 13-year-old girl with left atrial isomerism, dextrocardia, azygos continuation to the left superior vena cava, and median liver. The transhepatic route seemed to be the less tortuous access to the fenestration which was previously partially occluded with a CardioSeal device. The hepatic veins were entered at the left abdominal side using fluoroscopic guidance and the fenestration was uneventfully occluded with an Amplatzer septal occluder device.

A successfully corrected case of a 39-year-old female with viscero-atrial heterotaxic syndrome was reported. The cardiovascular anomalies were sinus venosus atrial septal defect, mitral cleft, and interruption of inferior vena cava with azygos continuation. She also had bilateral bilobed lungs, symmetric liver, right sided stomach, and malrotation of the bowel, indicating left isomerism. The atrial septal defect was closed with Teflon patch and mitral cleft was repaired with direct suture of the cleft. Complex complete atrioventricular septal defects were repaired in 29 patients including 12 with associated tetralogy of Fallot, 10 with double-outlet right ventricle, and seven with additional anomalies associated with atrial isomerism. Construction of an intraventricular tunnel connecting the left ventricle with the aorta was accomplished in 23 of 25 patients with ventriculo-arterial concordant or double outlet connections. There was one (4 per cent) hospital death and three (11 per cent) late deaths. Anatomic and technical considerations are presented pertinent to the care of this group.

We described two sibs born to consanguineous Sicilian parents who died of severe congenital heart malformation. Both had dextrocardia; however, only the girl had situs viscerum inversus. At necropsy she was found to have a right spleen and right pulmonary isomerism (three lobes in each lung, as commonly found in the asplenia syndrome). This observation, together with other literature reports, suggest that isolated dextrocardia, situs viscerum inversus, and the asplenia-polysplenia complex may be different end results of a unique dysmorphogenetic process involving the embryonic midline.

A capillary-scale high-pH anion-exchange chromatography (HPAEC) system for the analysis of carbohydrates was developed, in combination with two parallel on-line detection methods of sub-picomolar sensitivity: (1) pulsed amperometric detection (PAD); (2) capillary-scale desalting followed by electrospray ion-trap (IT) mass spectrometry (MS). The capillary chromatographic system combined the superb selectivity of HPAEC that allows routine separation of isomeric oligosaccharides with the information on monosaccharide sequence and linkage positions obtained by MS/MS fragmentation using the IT-MS.

The retinoids and carotenoids in bovine pineal gland were analyzed by high-performance liquid chromatographic methods. The major retinoid observed was all-trans retinol, with an average level of 579 pmol/g wet tissue. In agreement with a previous report, retinal was not detected; however, we could not detect any retinyl ester in bovine pineal gland. Directing groups play an important role in controlling the selectivity of C-H bond activation. Here we demonstrate that for iron, the nature of the directing group (e.g., ketone, ester, or amide) influences the C-H activation process. The syntheses of all possible stereoisomers of desfluorinated side products of the potent antihypertensive β-blocker nebivolol are reported. A straightforward approach using a common racemic precursor was employed to obtain the desired optically active building blocks. For one series of compounds, a Sharpless asymmetric epoxidation (SAE) route yielded in a direct fashion the required compounds whereas a Mitsunobu reaction was selected to obtain the other series of compounds. This offers a flexible approach to all desfluoronebivolol side-products in order to fully characterize them.
