The contribution of the tibial tubercle to patellar instability: analysis of tibial tubercle-trochlear groove (TT-TG) and tibial tubercle-posterior cruciate ligament (TT-PCL) distances.

PURPOSE: The purpose of this study is to assess the reliability of measuring the tibial tubercle to posterior cruciate (TT-PCL) distance compared to the tibial tubercle to trochlear groove (TT-TG) distance on magnetic resonance imaging (MRI), establish baseline TT-PCL values in patellar instability patients, and determine the predictive value of an excessive TT-PCL distance (≥24 mm) for recurrent patellar instability compared to a TT-TG distance ≥20 mm. METHODS: TT-TG and TT-PCL distances were calculated on MRI in a randomized and blinded fashion by two reviewers on 54 patients (59 knees) wit

[Knee ligament injuries].

Like no other joint of the human body the knee depends on intact ligaments. Knee instability due to ligament injuries will cause abnormal jo … lated medial collateral ligament rupture which will heal with conservative treatment, they range to knee dislocation, a serious injury which needs emediate care and is associated with a high incidence of complications. Surgical procedures aim to reconstruct knee ligaments as anatomical as possible to provide for a long term stable knee joint.

Congenital pseudarthrosis of the tibia: an atypical proximal location.

UNLABELLED: Proximal location of congenital pseudarthrosis of the tibia (CPT) is uncommon, and its management challenging, risking to end in amputation. We here report a case of proximal CPT managed in a limb-sparing perspective and followed up until the end of growth. A 17-year-old girl presented with type-1 neurofibromatosis and proximal CPT. Initial X-ray showed severe pseudarthrosis of the tibia with bone atrophy, 12-cm shortening and femorotibial and femoropatellar dislocation. Inter-tibiofibular graft and fibular tibialization were performed. At end of follow-up (age 33 years), fusion ha

[Evidence-based treatment protocol to manage patellar dislocation].

Patellar dislocation is a common knee injury with mainly lateral dislocations, leading to ruptures of the medial patellofemoral ligament in most of the cases. Reliable data and prognostic factors for stability of the patellofemoral joint and satisfaction of the patient after either conservative or operative treatment have not been established yet. Until now, there are no randomized controlled trials for recurrent patellar dislocation at all. As a synopsis of the randomized controlled trials about first-time patellar dislocation, no significant difference between operative and conservative mana

Isolated Talonavicular Arthrodesis for Traumatic Talonavicular Arthritis: Report of 2 Cases With Gait Analyses.

Isolated talonavicular arthrodesis is one of the surgical procedures for patients with talonavicular arthritis. However, the 3-dimensional kinematic behavior of the hip, knee, and foot/ankle complex during walking after the arthrodesis remains unclear. The clinical outcomes and gait analyses of 2 cases who underwent isolated arthrodesis for talonavicular osteoarthritis with chronic dislocated navicular fracture are presented. Gait analysis was carried out in both cases 1 year after surgery to clarify the side-to-side differences in the ranges of motion of the hip, knee, and foot/ankle complex

The favourable anisometry: an original concept for medial patellofemoral ligament reconstruction.

Medial patellofemoral ligament (MPFL) reconstruction for recurrent patellar dislocation has recently become more popular. We describe a technique that involves tensioning of the graft with the knee in full extension while simultaneously pulling with a bone hook on the patella in the direction of the quadriceps tendon, thus facilitating placement of the femoral attachment and accurate graft tensioning. We investigated the clinical outcome of this reconstructive procedure in 20 patients (23 knees), with a mean follow-up of 2 years. There was no recurrence of dislocation after surgery. At the 3 m

Rapid recovery after oxford unicompartmental arthroplasty through a short incision.

Forty Oxford medial unicompartmental arthroplasties (UCAs) were performed through a short incision medial to the patellar tendon, without dislocation of the patella, using updated instruments (Oxford Knee Phase III, Biomet Ltd., Bridgend, UK). The rate of recovery of these knees (measured by the time taken to achieve straight-leg raising, 70 degrees of flexion, and independent stair climbing) was compared with that of 20 Oxford UCAs performed through an open approach with dislocation of the patella. Both groups were compared with 40 AGC (Biomet) total knee arthroplasties performed for osteoart

Clinical outcomes after absorbable suture fixation of patellar osteochondral fracture following patellar dislocation.

BACKGROUND: Osteochondral fracture (OCF) is one of the severe complications following a patellar dislocation. The appropriate fixation method for patients with OCF remains controversial. METHODS: Eighteen patients who had undergone surgery after a patellar dislocation were recruited retrospectively. Patellar OCF was fixed with an absorbable suture in an unreported method. The medial patellofemoral ligament (MPFL) was repaired or reconstructed if necessary. The Lysholm and Kujala knee scoring systems were used to evaluate the knee function. Imaging examinations were used to confirm the fractur

Patella Dislocation in Children and Adolescents.

Introduction Patellar dislocation is one of the commonest knee injuries in adolescents. Although treatment usually leads to good results, the influence of anatomical and functional factors on therapeutic strategy has been underestimated, especially in cases of recurrence. Patients and Methods The course of treatment has been analysed in 88 patients with 136 patellar dislocations. The importance of anatomical conditions was studied using X-ray and MRI findings. The treatment results were critically evaluated in comparison with current recommendations. Results From 2000 to 2015, 109 patellar dis

Congenital dislocation of the patella. Part I: pathologic anatomy.

There has been considerable confusion between true congenital dislocation of the patella and other patellar instabilities. Only very few papers describing the anatomical features of congenital dislocation of the patella are found in the literature. The purpose of this paper was to describe the anatomical anomalies found in two cadaver specimens of a true permanent and irreducible congenital patellar dislocation. The quadriceps femoris is short and displaced laterally and acts as a knee flexor. The patella is small, articulating with the outer aspect of the lateral condyle, with no possibility

Knee deformities in multiple hereditary exostoses. A longitudinal radiographic study.

For a mean period of 7.8 years, the morphology of knee deformity and its annual changes in 8 patients (16 knees; mean age at the initial examination, 8.4 years) with multiple hereditary exostoses were evaluated radiographically. Knee deformity developed during the growth spurt period. Genu valgum with a femorotibial angle that decreased more than 2 standard deviations from the mean of age-matched normal children was observed in 6 of the 16 knees. In 3 of these 6 knees, recurrent dislocation of the patella was observed. Angulation of the femur varied among the knees, but the tibia showed valgus

Extensor Mechanism Centralization Using Autograft Hamstring for Laterally Dislocating Patella.

Patellofemoral complications following total knee arthroplasty (TKA) are some of the most commonly cited complications in the literature, accounting for up to 50% of secondary procedures in the literature. Lateral dislocation of the patella, while rare, is one of many causes of extensor mechanism dysfunction. We sought to evaluate a novel patellar centralization procedure by comparing patients' pre- and postoperative clinical and radiographic data. A retrospective case-series study was performed on 12 patient knees (5 male and 7 female) presenting with ambulatory dysfunction due to laterally d

Open tibia plateau fracture with intra-osseous dislocation of the patella and quadriceps tendon rupture: a case report.

BACKGROUND: Both tibial plateau fractures and extensor apparatus injuries are serious injuries to the knee joint that generally do not occur in the same patient. We report a rare case of open tibial plateau fracture combined with quadriceps tendon rupture and complete displacement of the patella into the tibial plateau fracture. CASE PRESENTATION: The case involved a male who was 19 years old who had been in a motorcycle accident. The patient was admitted to our department with an open tibial plateau fracture 3 h post-injury. X-ray showed a tibial plateau fracture and complete displacement of

Recurrent dislocations and subluxations of the patella.

Seventy-four patients with patellar dislocations and subluxations were treated with a procedure described by Trillat. The Trillat method consists of stepwise correction of predisposing anatomic abnormalities including lateral retinacular release, patellar tubercle transfer, and advancement of the vastus medialis, if necessary. The follow-up period in 42 knees was from two to nine years with an average of 4.2 years. The results were excellent in six knees, good in 28 knees, fair in five knees, and poor in three knees. Thorough intraarticular examination is essential. If significant patellofemor

Femoral head contusion without hip dislocation: low-energy trauma in a female basketball player.

Femoral head contusions can occur concurrently with knee ligament injuries, patellar dislocations, or traumatic hip dislocations. Posterior hip subluxation is a possible mechanism in the absence of hip dislocation. Clinicians should have a high index of suspicion when evaluating an athlete who has significant hip or groin symptoms, despite having experienced seemingly minor or relatively low-energy trauma. Most patients should respond well to a period of non-weight bearing and a gradual return to exercise, but they may need to be monitored for future avascular necrosis and osteoarthritis in th

Acute patellar dislocation: results of immediate surgical repair.

Seventeen cases of acute, first time, lateral patellar dislocations without fracture treated by immediate surgical repair were reviewed and evaluated. Nine patellar tendon realignments and 8 medial side reefing procedures were performed. There were no recurrences of patellar dislocation. Twelve knees were painful, of which 8 were rarely painful. All but one patient were satisfied with their results and participated in any desired sport. Surgical repair of acute patellar dislocation is advocated in the young patient who anticipates future participation in athletics.
