Stage-assessment of the progress of continuous vertigo of peripheral origin by means of spontaneous and head-shaking nystagmus findings.

The stages of continuous vertigo of peripheral origin were classified into nine categories according to the findings of spontaneous nystagmus (SPN) and head-shaking nystagmus (HSN). The patients analysed were 18 with vestibular neuronitis, 6 with sudden deafness and 6 with unilateral inner ear disorders. 1) Irritative SPN (Stage I) was rarely encountered. 2) SPN of the paralytic type (Stage II) was usually observed in the period less than one month after the onset of diseases. 3) HSN directed toward the intact side with or without a reversal phase (Stages III-2 and III-1, respectively) were th

Otosclerosis and vestibular dysfunction.

Vestibular dysfunction associated with otosclerosis has been observed for many years. In a recent clinical study of patients exhibiting progressive sensorineural hearing loss of unknown origin, it was demonstrated that approximately 50% of the cases had cochlear otosclerosis/otospongiosis as the most probable cause for the hypacusis. Of these, approximatley 55% had vestibular symptoms. In the present study, the effect of sodium fluoride therapy in this group of patients is analyzed. It will be demonstrated that such therapy is beneficial not only in controlling the progressive sensorineural he

[Vestibular meanifestations of multiple sclerosis. A new diagnostic element revealed by bithermic tests].

We studied 56 cases of disseminated sclerosis, all during an attack of the disease and all presenting with symptoms of incoordination. The diagnosis was certain both clinically and biochemically. Out of this population, 64% of the patients had unprovoked nystagmus; of these patients 48% had central type nystagmus (multiple, vertical superior or vertical inferior, or bilateral "gaze nystagmus"). We investigated particularly abnormalities in the bithermic tests (vestibular hypovalency, directional preponderance of nystagmus, thermic preponderance) and in particular we studied an index which to t

Vestibular symptoms in mumps deafness.

Deafness appears in as many as 4% of adult cases of epidemic parotitis. It is often severe, though reversible and is usually unilateral. Vertigo has been reported in mumps, as have occasional cases with impaired caloric reaction. Twenty cases of unilateral hearing impairment in mumps have been investigaed with audiometry and electronystagmography in order to determine the degree of permanent lesions. Nine of the patients had noted vertigo when falling ill with protitis. Five of these had normal calorics, 3 were impaired and one was without response in the deaf ear. No certain correlation betwe

Cochlear Implantation: Systematic Approach to Preoperative Radiologic Evaluation.

Sensorineural hearing loss results from abnormalities that affect the hair cells of the membranous labyrinth, inner ear malformations, and conditions affecting the auditory pathway from the cochlear nerve to the processing centers of the brain. Cochlear implantation is increasingly being performed for hearing rehabilitation owing to expanding indications and a growing number of children and adults with sensorineural hearing loss. An adequate understanding of the temporal bone anatomy and diseases that affect the inner ear is paramount for alerting the operating surgeon about variants and imagi

Aberrant growth and pattern formation in Peromyscus hybrid placental development.

Crosses between the North American deer mouse species Peromyscus maniculatus (BW) and P. polionotus (PO) produce dramatic asymmetric developmental effects. BW females mated to PO males (female bw × male po) produce viable growth-retarded offspring. In contrast, PO females mated to BW males (female PO × male BW) produce overgrown but dysmorphic conceptuses. Most female PO × male BW offspring are dead by midgestation; those surviving to later time points display numerous defects reminiscent of several diseases. The hybrid effects are particularly pronounced in the placenta. Here we examine place

Subclassification of vestibular disorders by means of statistical analysis in caloric labyrinth testing.

OBJECTIVE: In the past, various attempts were made to perform a quantitative analysis of nystagmographic findings but their diagnostic value was limited. Therefore, the authors present a multivariate analysis of nystagmus findings with the aim of increasing the precision of diagnostic differentiation in cases of vestibular dysfunction. MATERIAL AND METHODS: A group of 387 patients and 40 healthy volunteers were examined over a 14-month period using electronystagmography after stimulation by bithermal, bilateral irrigation of the labyrinth. Amplitude, slow-phase velocity, frequency and directi

Inner ear DNA receptors in MRL/lpr autoimmune mice: potential 30 and 70 kDa link between autoimmune disease and hearing loss.

Inner ear function and systemic autoimmune disease were evaluated in the MRL/lpr mouse to determine their relationship with alterations in cell surface DNA receptors of 28-30 and 68-70 kDa size. Auditory brainstem response thresholds in the autoimmune disease mice were significantly elevated as early as 2 months of age when compared to MRL/++ controls. Hearing thresholds continued to rise with progression of the disease, manifested as increasing spleen weights, antinuclear (anti-DNA) antibodies, and serum immune complexes. Cochlear membranous labyrinth cells in the autoimmune mice bound less D

Long-term effects of destruction of the endolymphatic sac in a primate species.

The endolymphatic sacs of ten vervet monkeys were surgically ablated unilaterally, and the animals were assigned predetermined survival times ranging from 6 months to 5 years. The left and right ears were prepared in anatomical continuity for light microscopic study. Four experimental ears had openings in their membranous labyrinths of unknown etiology and were not considered acceptable for correlative studies. The remaining five experimental ears showed mild cochlear endolymphatic hydrops, which was localized to the apical regions or was more severe in the apical regions than elsewhere. Four

What does the head impulse test versus caloric dissociation reveal about vestibular dysfunction in Ménière's disease?

In patients with Ménière's disease (MD), caloric testing can show, depending on the stage and activity of the disease, a variety of results. Between attacks, many, or perhaps even most, patients with unilateral early or mild MD have normal caloric tests; late MD can show abnormalities ranging from mild to severe unilateral canal paresis with or without directional preponderance. The explanation of canal paresis in MD is not clear. The most obvious explanation, severe loss of lateral canal hair cells, is not likely to be correct because hair cell loss will not explain the fluctuating canal pare

[MR imaging of the inner ear; findings of Ménière's syndrome].

Symptoms in Ménière's disease are explained by hydrops of endolymphatic system with recurrent ruptures of the membranous labyrinth. The primary cause of the increased endolymphatic volume appears to be an imbalance between secretion and resorption of endolymph which may be due to an obstruction of the endolymphatic duct and sac, located in the vestibular aqueduct. Non-visualization or narrowing of the vestibular aqueduct of the paper have been demonstrated by conventional tomography and high resolution computed tomography (HR-CT). But the endolymphatic duct and sac can not be obtained by HR-CT

[Clinical emergency-complicated infections of the middle ear and paranasal sinuses].

Complicated infections of the middle ear and paranasal sinuses remain a relevant clinical challenge. Although acute otitis media and acute sinusitis are generally self-limiting, a small but clinically significant proportion of cases progress to severe local, orbital, or intracranial complications. These include acute mastoiditis, labyrinthitis, facial nerve palsy, orbital complications, frontal bone osteomyelitis, intracranial abscess, meningitis, and cerebral venous sinus thrombosis, all of which are associated with considerable morbidity and potential mortality. The current review provides a

Vestibular function is associated with immune inflammatory response.

Association between vestibular function and immune inflammatory response has garnered increasing interest. Immune responses can lead to anatomical or functional alterations of the vestibular system, and inflammatory reactions may impair hearing and balance. Vestibular disorders comprise a variety of conditions, such as vestibular neuritis, benign paroxysmal positional vertigo, Meniere's disease, vestibular migraine, posterior circulation ischemia, and bilateral vestibular disease. Moreover, some patients with autoimmune diseases develop vestibulocochlear symptom. This paper offers an overview

Ménière's disease is a disorder of the inner ear.

PURPOSE OF REVIEW: Ménière's disease is a disorder characterized by recurrent episodes of vertigo, hearing loss, tinnitus, and pressure in the ear. Its pathogenesis and optimal management continue to be the subject of ongoing debate. Although it is classically believed to be a primary disorder of the inner ear, some clinicians have hypothesized that it is a form of vestibular migraine. RECENT FINDINGS: Evidence supporting vestibular migraine in the differential of Ménière's disease includes overlap of symptoms, high prevalence of migraines in patients with Ménière's disease, and the efficacy

Diagnosing the cause of vertigo: a practical approach.

Dizziness is among the commonest of chief complaints. It often presents a significant challenge to the attending physician, because the symptoms and signs are often vague and non-specific. However, a robust systematic approach can usually arrive at the diagnosis. Maintaining balance requires sensory inputs from the vestibular, visual, and somatosensory systems and the cerebellum fine-tunes inaccurate motor outputs. Causes of vertigo are most commonly otological, followed by central, somatosensory, and visual. The first question in approaching patients with dizziness is to categorise dizziness

[Sign-report: torsional nystagmus from head-shaking in patients with labyrinth lithiasis of the vertical canal].

The clinical picture of Positional Paroxysmal Vertigo (PPV) induced by vertical canal labyrintholithiasis has been clearly described, eliminating previous interpretations of pathogenesis of this disorder. The diagnosis of PPV is based on the well-known picture of positional paroxysmal Nystagmus. The Authors report a clinical sign which has not previously been reported in the literature: torsional Nystagmus induced by the Head-Shaking Test (HST). The Authors encoutered this sign in 30% of the cases of vertical canalolithiasis and in 50% of the cases diagnosed as vertical cupulolithiasis. This s
