The general physician can help patients and families living with advanced multiple sclerosis and motor neurone disease in a number of practical ways. The key areas requiring a palliative approach are the management of physical symptoms, psychosocial support, decisions around potentially life-prolonging treatments and end-of-life care.

Amyotrophic lateral sclerosis (ALS) is a fatal motor neuron disease that occurs in 4 among 100 000 people in the United States. Individuals with ALS gradually lose their ability to control voluntary muscles, diminishing their ability to communicate. A comprehensive multidatabase search retrieved 31 qualitative research articles that addressed persons with end-of-life experiences with ALS. Inclusion/exclusion criteria were applied and a critical appraisal was applied for the final 8 included articles. First-person data extraction from the final articles represented emergence of 3 themes signifi

OBJECTIVES: As health care systems face limited resources and workforce shortages to address the complex needs of older adult populations, innovative approaches utilizing information technology can support aging. Smart Home and Ambient Assisted Living (SHAAL) systems utilize advanced and ubiquitous technologies including sensors and other devices that are integrated in the residential infrastructure or wearable, to capture data describing activities of daily living and health related events. This paper highlights how data from SHAAL systems can lead to information and knowledge that ultimately

This author's experience in oncology proposes "Pawspice," a new concept that offers early supportive care for pets with life-limiting disease, embracing palliative care and standard care. Pawspice offers compassionate and comprehensive symptom relief at diagnosis while addressing life-limiting diseases. The concept of Pawspice is to maintain quality of life with palliative care that improves the patient's debilitating conditions by 30% to 50%, while simultaneously administering standard care via gentle chemotherapy modified for low toxicity. This combination makes Pawspice different than palli

Chronic life-limiting illnesses such as chronic kidney disease (CKD) require integral support to the families concerned in addition to medical care. Palliative care is an option to facilitate families to address future concerns, such as procedures for acute life-threatening complications, or to relieve physical and psychosocial suffering. The exact needs of patients or parents have not yet been investigated. To assess needs in supportive palliative care, we conducted a monocentric qualitative interview study. We included patients 14 to 24 years old as well as parents of younger children (below

BACKGROUND: Co-infection with Visceral Leishmaniasis (commonly known as Kala Azar, KA) and Human Immunodeficiency Virus (HIV) is increasingly being diagnosed among patients in Bihar. This qualitative study is the first assessment of self-reported quality of life among patients co-infected with KA-HIV in the Asian context.
METHODS: We conducted semi-structured, in-depth interviews and adopted an inductive thematic analysis to generate evidence on the quality of life of patients co-infected with KA and HIV. Patients were purposively sampled until saturation was attained.
RESULTS: We found that

BACKGROUND: Smoking rates are up to four times higher among people experiencing homelessness compared to the housed population and how to support them to quit is unclear.
OBJECTIVES: To test the effectiveness and cost-effectiveness of providing e-cigarettes to people accessing homeless support services to stop smoking. To explore implementation fidelity, contextual influences and mechanisms of change.
TRIAL DESIGN: A two-arm cluster randomised controlled trial with cost-effectiveness and mixed-methods process evaluation.
METHODS: Setting and participants: Thirty-two homeless centres (cluste

The elimination of suffering and the cure of disease are the fundamental goals of medicine. While medical advances have transformed previously fatal conditions such as cancer and heart disease into illnesses that people can live with for many years, they have not been accompanied by corresponding improvements in the quality of life for these patients and their families. Living with a serious illness should not mean living in pain or experiencing symptoms like shortness of breath, nausea, or fatigue. Yet, multiple studies over the past decade suggest that medical care for patients with advanced

PURPOSE OF REVIEW: To review the impact of nutrition on critically ill older adults, focusing on the role of comorbidities such as malnutrition, sarcopenia, and frailty, as well as the importance of nutritional support for outcomes such as mortality, recovery, and long-term disability.
RECENT FINDINGS: Malnutrition is highly prevalent in the elderly and is strongly associated with adverse outcomes, including increased mortality, longer ICU stays, and higher rates of infection. In addition, frailty, multimorbidity, and cognitive decline significantly contribute to the vulnerability of older pa

BACKGROUND: Heart failure is highly prevalent with poor outcomes, yet only a small proportion of patients receive specialist palliative care services.
AIM: To explore if a programme focusing on support and symptom management of people with heart failure in a hospice environment would be acceptable and of benefit to patients.
METHODS: A pre/post intervention study design using mixed methods was used to evaluate the programme. The programme was delivered in 2-hour time slots over a period of 8 weeks. Participants completed three questionnaires pre- and post- and were interviewed within 1 week

OBJECTIVE: To determine if early palliative care intervention in patients with recurrent, platinum-resistant ovarian cancer is potentially cost saving or cost-effective.
METHODS: A decision model with a 6 month time horizon evaluated routine care versus routine care plus early referral to a palliative medicine specialist (EPC) for recurrent platinum-resistant ovarian cancer. Model parameters included rates of inpatient admissions, emergency department (ED) visits, chemotherapy administration, and quality of life (QOL). From published ovarian cancer data, we assumed baseline rates over the fin

BACKGROUND: Cachexia is a wasting syndrome found within a range of chronic illnesses/life-limiting conditions, however awareness and understanding of cachexia amongst renal Health Care Professionals has not been investigated.
OBJECTIVES: To ascertain the awareness, understanding and treatment practices of Health Care Professionals who provide care for people with cachexia and end-stage renal disease.
METHODS: Health Care Professionals were recruited via the European Dialysis and Transplant Nurses Association/European Renal Care Association in September 2018. This was an exploratory study usi

INTRODUCTION: The importance of work or productive activity for the well-being, community integration, and quality of life of people living with disabilities is addressed, with implications for life care planning and case management.
BACKGROUND: The role of work or productive activity in our society, and consequences of deprivation if rehabilitation services do not address vocational effects of disabilities, is explored. A continuum of productivity options is introduced; types of vocational rehabilitation assessment processes and interventions are described.
PURPOSE: The role of vocational r

Approximately, 15% of women in the United States experience postpartum depression, but Black and Latina women continue to suffer at disproportionately higher rates. While literature has shown that the experience of life stressors is a strong predictor of postpartum depressive symptoms, less research has examined personal and contextual resources that can protect minoritized groups from poor mental health outcomes during the perinatal period. The current article investigated whether four types of protective factors (family support, doctor support, nurse support, and perceived control) moderated

Advances in fetal health detection and neonatal care have improved outcome predictions but have outpaced the development of treatments, leaving some families facing the heartbreaking reality of their baby's short life expectancy. Families with a fetus that has a life-limiting condition must make tough decisions, including the possibility of termination, perinatal palliative care options, and the extent of newborn resuscitation. Access to abortion services is crucial in decision-making, underscoring the significance of palliative care as an option. Perinatal palliative care programs offer vital

Social determinants of health (SDoH) play important roles not only in a patient's health status but also in access to palliative care, the family's decision-making process, and the formation of patient values and preferences. In this scoping review, we analyze the social determinants that influence palliative care decision-making and examine the relationship between palliative care decisions and SDoH in the intensive care unit (ICU). We conducted a literature search of five databases including the search terms "Intensive care units," "palliative care," and "social determinants of health." The
