Arthroscopic reconstruction of anterior cruciate ligament using BTB patellar ligament in the press-fit technique.

Arthroscopic reconstruction of the anterior cruciate ligament (ACL) with a bone-patellar tendon-bone graft in the press-fit technique is an efficient procedure, which provides high primary stability without use of implants and thus ensures early functional rehabilitation. The procedure is based on graft harvest with an oscillating hollow saw, which allows collection of cylindrical bone blocks. Femoral fixation using a template and file has been standardized to an extent that ensures secure press-fit fixation. Tibial anchorage is achieved by refilling the tibial tunnel with the bone cylinder re

High tibial osteotomy and ligament reconstruction for varus angulated anterior cruciate ligament-deficient knees.

In a consecutive series, we treated 41 young patients who had anterior cruciate ligament deficiency, lower limb varus angulation, and varying amounts of posterolateral ligament deficiency. Seventy-three percent of the patients (N = 30) had lost the medial meniscus and 63% (N = 26) had marked articular cartilage damage in the medial compartment. All patients were treated with high tibial osteotomy and, in the majority (N = 34), anterior cruciate ligament reconstruction a mean of 8 months later. Posterolateral reconstructions were also required in 18 knees. A 100% follow-up was obtained at a mea

Anterior oblique ligament reconstruction of the thumb using the transverse carpal ligament: description of a new procedure.

Traumatic dislocation of the thumb carpometacarpal joint is an uncommon injury that may have long-term sequelae after treatment. Joint instability may cause pain, weakness of pinch, and the increased likelihood of development of posttraumatic arthritis. The anterior oblique ligament has generally been accepted as the most important static stabilizer of the trapeziometacarpal joint. Numerous operative techniques have been described to reconstruct this ligament. The author introduces a novel operative procedure to reconstruct the anterior oblique ligament of the thumb using the transverse carpal

Surgical treatment of combined injury to anterior cruciate ligament, posterior cruciate ligament, and medial structures.

Injuries involving the anterior cruciate, posterior cruciate, and medial collateral ligaments represent one combination of injuries representing knee dislocation. Prompt reduction and neurovascular evaluation are necessary when treating these injuries. Operative management involving reconstruction and repair of the injured structures produces optimal results. This article outlines the principles involved in evaluating and managing these injuries, combined with specifics of the authors' treatment approach, and review of the literature.

Knee dislocation following anterior cruciate ligament disruption without any other ligament tears.

We report a rare case of complete knee dislocation following anterior cruciate ligament (ACL) disruption without any other ligament tears. The pathology of the knee joint was torn ACL, intact other ligaments, osteochondral fractures and bone bruise of the lateral femoral condyle, and torn lateral meniscus. In this case, osteochondral fracture resulting from the anteriorly sublaxiation of the tibia following ACL disruption was considered to prevent from spontaneous reduction. This case suggests that anterolateral knee dislocation and spontaneous reduction may occur in ACL injuries.

[Evaluation of results after arthroscopic reconstruction of the anterior cruciate ligament with a free graft of the patellar ligament].

From among 312 patients treated with arthroscopic anterior cruciate ligament reconstruction (one-incision, transtibial with the use of ipsilateral bone-patellar tendon-bone autograft) between 1992 and 1994 256 (82%) were reviewed with a mean follow-up of 26.4 months. An aggressive rehabilitative protocol advocated by Shelbourne has been employed. Stability of the knee has been assessed with KT-1000 arthrometer and IKDC and OAK forms served to evaluate the results. In 77% of the cases nearly normal function of the knee has been restored (87% excellent and good results in OAK form). Arthroscopic

Comparative animal study of three ligament prostheses for the replacement of the anterior cruciate and medial collateral ligament.

Three different ligament prostheses (Leeds-Keio, Gore-Tex and a prototype of Aramid) were implanted in sheep knee joints replacing the anterior cruciate ligament and the medial collateral ligament. After 1 yr the knees were explanted and their biomechanical properties were assessed by a drawer test and tensile tests of the implants. Additionally the ligament replacements, the synovial membrane and the lymph nodes were inspected histologically. For each type of prosthesis partial or total ruptures occurred. None of the operated joints regained normal stability and stiffness. Anterior knee stabi

Integrin display increases in the wounded rabbit medial collateral ligament but not the wounded anterior cruciate ligament.

The differential capacities of the anterior cruciate and medial collateral ligaments to heal may be related to differences in cellular function. This study tested the hypothesis that differential expression of integrins occurs in these ligaments after injury. The integrins are a family of cell surface receptors that mediate adhesion, migration, and other cellular functions critical to the healing of a wound. A similar complement and amount of the beta 1 subfamily of integrins are known to be present on the unperturbed anterior cruciate and medial collateral ligaments in humans and rabbits. A p…

Results of use of the central one-third of the patellar ligament to compensate for anterior cruciate ligament deficiency.

In a consecutive series of 91 patients with 92 injured knees in which the anterior cruciate ligament was irrevocably destroyed, 79 were males; 12 were females. After a 7-year interval one male patient had surgery on his other knee. Twenty-five of the knees demonstrated isolated injuries to the anterior cruciate ligament, but the incidence of this lesion may be even greater than recorded here. The anterior cruciate ligament was repaired by a patella ligament reconstruction along with other surgical procedures when indicated. There is no acceptable alternative treatment for surgical repair, or s

Length Changes of the Lateral Knee: Fibular Collateral Ligament, Anterolateral Ligament, and Iliotibial Band Fibers.

BACKGROUND: The lateral knee is stabilized by the fibular collateral ligament (FCL), anterolateral ligament (ALL), and iliotibial band (ITB) … angles closer to extension may be favorable for avoiding overconstraint in reconstruction of these ligaments. These findings are anatomic and descriptive rather than prescriptive. Further loaded biomechanical studies are needed to determine optimal graft tensioning protocols.

All-Arthroscopic Reconstruction of the Medial Patellofemoral Ligament Using Artificial Ligament for Recurrent Patellar Dislocation.

Medial patellofemoral ligament injury is the primary stability barrier after patellar dislocation, with high recurrence rates with conservative treatment, making reconstruction surgery increasingly mainstream. This article describes an all-arthroscopic technique for medial patellofemoral ligament reconstruction using high-strength synthetic suture tape combined with suture anchors, eliminating the need for tendon harvesting. It involves anatomic localization of the patellar and femoral insertion sites, with appropriate tensioning and fixation at 30° of knee flexion to restore medial stability.

Modified Endoscopic Distal Soft Tissue Procedure With Medial Metatarsosesamoid Ligament and Intermetatarsal Ligament Augmentation.

Hallux valgus is one of the most common foot and ankle conditions faced by orthopaedic surgeons. Surgical correction of hallux valgus is frequently indicated for symptomatic deformity. Recently, the endoscopic technique of hallux valgus correction has been reported, which is basically an endoscopic approach to the classic distal soft tissue procedure. In this technical note, the technical details of the modified endoscopic distal soft tissue procedure with medial metatarsosesamoid ligament and intermetatarsal ligament augmentation is described. In this modified technique, the intact intermetat

Biomechanical Comparison of Two Fixation Techniques for Lateral Ulnar Collateral Ligament Repair With Ligament Bracing.

PURPOSE: Ligament bracing is a technique of suture reinforcement that can be used to augment lateral ulnar collateral ligament repair in the treatment of posterolateral rotatory instability of the elbow, thereby improving early stability of the repair. However, multiple failures of the ulnar anchor during implantation have been documented. We hypothesized that the use of a cortical button for ulnar fixation of the ligament brace would be biomechanically comparable to a suture anchor construct. METHODS: Sixteen elbows were tested with a materials testing machine. The intact, dissected, and rep

Comparative Study of Spring Ligament Reconstructions Using Either Hamstring Allograft or Synthetic Ligament Augmentation.

BACKGROUND: Idiopathic flatfeet are usually caused by attenuation of the medial soft tissues rather than a lateral osseous deficiency. Debate continues on whether spring ligament attenuation or posterior tibial tendon (PTT) dysfunction is the initial driver for the deformity. Our comparative prospective study aimed to quantify the radiological and clinical outcomes of 2 techniques for spring ligament reconstruction using a hamstring graft or a synthetic ligament. METHODS: Seventeen spring ligament reconstructions (SLRs) were performed in 17 patients using synthetic ligament augmentation and 1

Arthroscopic femoral tensioning and posterior cruciate ligament reconstruction in chronic posterior cruciate ligament injury.

For the new arthroscopic femoral tensioning and posterior cruciate ligament (PCL) reconstruction in chronic PCL injury, we make a transtibial tunnel, using the posterior trans-septal portal, while preserving the remnant bundle of the original PCL and meniscofemoral ligament. Also, to preserve the remnant femoral fibers of the PCL and provide femoral tensioning, we manipulate the suture hook with No.1 PDS by hand so that the sharp tip penetrates the remnants of the PCL near the femoral attachment site. After pulling the sutures previously placed at the remnant PCL fibers, the notchplasty osteot
