Primary endobronchial actinomycosis is an extremely rare disease that presents with endobronchial mass. We report 2 cases of primary endobronchial actinomycosis associated with broncholithiasis. There was no foreign body material, suggesting that these broncholiths were formed endogenously. The Authors report a case of cholecystobiliary fistula due to cholelithiasis and bile duct lithiasis, treated laparoscopically with an endostapler. In view of the rarity of this condition, we believe it may be useful to discuss the diagnostic work-up in the light of the diagnostic and therapeutic measures currently available. We also review the literature with a view to defining the existing classifications of cholecystobiliary fistulas. We conclude by stressing the importance the endoscopic placement of nasobiliary catheters preoperatively to facilitate laparoscopic treatment.

We report about one case of cholecystoduodenal fistula complicated by antropyloric lithiasic obstruction, which was treated surgically with gastrotomy and extraction of the calculus, in an 82-year-old woman. This case represents an anatomic variant of Bouveret's syndrome, which is classically defined as a duodenal lithiasic obstruction. On the basis of this case, the authors discuss the diagnostic and possibly therapeutic merits of digestive endoscopy and define the main clinical, anatomical and evolutive characteristics of this unfrequent complication of biliary lithiasis.

Internal biliary fistulae are a well-recognized complication of biliary lithiasis. Among these, the cholecystoduodenal fistulae are the commonest while cholecystocolic fistulae (CCF) occur much less frequently. CCF secondary to gallbladder carcinoma is a rare occurrence and has been reported in very few studies. Here, the author reports a case of cholecystocolic fistula secondary to gallbladder carcinoma. Preoperative diagnosis of this condition requires high index of suspicion and is usually difficult. Computed tomography scan is helpful in establishing a preoperative diagnosis.

Sialolithiasis of the submaxillary gland is rare in the pediatric population. We report the case of a 13-year-old boy who presented submandibular tumefaction of 7 months evolution, which increased after meals. Localized lithiasis in Wharton s duct was observed in submaxillary sialolithiasis. Treatment was intraoral extraction with CO(2) laser and after recovering from the anesthetic, the patient was discharged.This technique shortens surgical time, facilitates hemostasia and reduces edema and postoperative pain. The procedure is suitable for ambulatory surgery.

A nonperative method has been employed over a period of 11 years in the treatment of 570 patients with residual biliary duct lithiasis. A successful instrumental extraction was accomplished in 543 of these patients (95.3%). These extractions were performed via the transcholedochal and the transcholecystic routes, the latter being used both for choledochal and gallbladder stones. The high percentage of successes, the extremely few complications, and the good long-term results turn this method into a definitely useful adjunct to the conventional therapy for biliary pathology.
