Osteoradionecrosis of the mandible: a review.

BACKGROUND: Osteoradionecrosis is a serious complication of radiotherapy that often leads to severe facial deformity, pain, pathological fracture, sequestration of devitalized bone, and orocutaneous fistulas. Preventive measures for osteoradionecrosis are the best treatment plan to avoid osteoradionecrosis. Radical surgery is indicated when conservative methods fail or when severe bone and soft-tissue necrosis prevails. CASE REPORT: The purpose of this paper is to explore the recent theories about the definition, classification, incidence, and pathophysiology of osteoradionecrosis (ORN) of th

New Perspectives on Maxillofacial Reconstruction in Patients With Skeletal Malocclusion: Deep Integration of Maxillofacial Reconstruction and Orthognathic Surgery Concepts.

The application of computer-assisted design and manufacturing (CAD/CAM) technology and digital guides has made maxillofacial defect reconstruction surgery faster, more convenient, precise, and highly accurate in restoration. However, in addressing maxillofacial reconstruction for patients with skeletal malocclusion, the restoration of facial contours has received less attention, and surgical planning for such cases remains challenging. In this study, we report a case where the deep integration of maxillofacial reconstruction and orthognathic surgery concepts was applied. The patient presented

Evaluation of bone height of the free fibula flap in mandible reconstruction.

BACKGROUND: Use of a vascularized free fibula flap has become a preferred method of mandible reconstruction after oncologic surgical ablation. Despite its many advantages, the low vertical height of the graft is a potential drawback and severe long-term atrophy of fibular bones may cause stress fracture and is disadvantageous for osseointegrated dental implants and facial contours. Therefore, it is important to investigate the degree of resorption based on the fibular height and the factors related to resorption over time. The influence of aspects of the intraoperative surgical procedure, such

Aneurysmal bone cyst-plus: a diagnostic enigma.

Aneurysmal bone cyst, commonly reported in the second and third decades of life, is characterized by a rapid growth pattern with resultant bony expansion and facial asymmetry. Interestingly, these lesions can occur in association with other primary bone pathologies, where they are referred to as "hybrid lesions" or "ABC-Plus." Their rarity, especially in the head and neck region, may pose a diagnostic challenge to clinicians. Following is a discussion of one such enigmatic lesion that includes description of a unique case of ABC-Plus that affected the right mandible of a young male.

Evaluation of panoramic radiography and axial computed tomography in cases of ameloblastoma and central giant cell granuloma of mandible.

Evaluation of mandibular mass lesions is a perplexing clinical problem. Some of these lesions have very similar clinical presentation and even similar or overlapping imaging features. Plain radiographs and orthopantomograms are frequently requisitioned for the diagnosis and are helpful in localization and characterization of these lesions. However, the specificity of diagnosis remains low. A study was undertaken to further characterize these lesions using Computed Tomography (CT) and analysis of cross sectional images of various mandibular mass lesions. Of these, imaging features of ameloblast

Condylar osteochondroma manifesting as Class III skeletal dysplasia: diagnosis and surgical approach.

This is a case report of a 21-year-old man who had a gradually progressive Class III malocclusion and crepitation of the right temporomandibular joint. Routine TMJ tomograms revealed a radiopaque-radiolucent lesion of the right mandibular condyle with a compensatory anteroinferior displacement of the left mandibular condyle. Nuclear bone scans confirmed increased asymmetric technetium 99 tracer uptake in the region of the right temporomandibular joint. A two-stage surgical approach was undertaken; this consisted of excision of the condylar lesion to correct the Class III malocclusion and poste

[Mandibular bone tumors. Diagnostic difficulties].

A session of the "Société de Stomatologie et de Chirurgie maxillo-faciale de France" was devoted to a discussion of diagnostic difficulties in cases of mandibular bone tumors. Several case-reports were presented as a basis for comments on clinical and radiologic criteria of the benign or malignant nature of bone tumors, these being the foundation on which to establish the most effective surgical approach to adopt. Pre-operative biopsy is not considered essential when confirmation of diagnostic has been obtained.

Immunoglobulin G4 related sclerosing disease mimicking a lytic lesion of the mandible: a case report and review of literature.

BACKGROUND: Immunoglobulin G4 related disease (IgG4-RD) is an immune-mediated, multifocal, fibroinflammatory disease with varied clinical manifestations. The involvement of head and neck region is infrequent. The objective was to report a case of localized IgG4-RD of mandible that clinically mimicked a lytic lesion. CASE PRESENTATION: A 22 year old female presented with restricted mouth opening and swelling over right ramus and angle region of mandible. Radiographic imaging showed an ill-defined radiolucent lytic lesion infiltrating adjacent muscles. An incisional biopsy was performed and his

Airway management in pediatric patients undergoing microvascular free tissue transfer reconstruction after mandibulectomy.

OBJECTIVES: Microvascular free tissue transfer (MVFTT) for head and neck reconstruction is infrequently performed in pediatric patients. There is a paucity of data on perioperative airway management in pediatric MVFTT, such as the need for tracheostomy, which can pose higher morbidity to young patients due to potential long-term effects on the softer, more pliable laryngotracheal cartilage. Our objective was to report airway outcomes on pediatric patients undergoing MVFTT after segmental mandibulectomy with or without tracheostomy. METHODS: Retrospective chart review of pediatric patients who

Does Free Fibular Flap Reconstruction Affect the Quality of Life in Pediatric Patients With Various Extend of Mandible Defects?

BACKGROUND: The long-term outcomes in pediatric patients with various extents of mandible defects have not been well-documented. METHODS: A retrospective case series in which this study looked at pediatric patients under the age of 13 who had mandibular reconstruction with free fibular flap and had not received another operation in the previous 6 months. The eating, swallowing, speech function, and quality of life are evaluated with EORTC QLQ-H&N35 after the mandible growth spurt has occurred. RESULTS: A total of 7 patients were included in this study with operation ages ranging from 6 years

Thirty-eight-year follow-up of the first patient of mandibular reconstruction with free vascularized fibula flap.

BACKGROUND: The mandible is responsible for vital functions of the stomatognathic system, and its loss results in functional and aesthetic impairment. Mandibular reconstruction with free fibula flap is considered the gold standard for mandibular reconstruction. CASE PRESENTATION: We describe here the 38-year follow-up of the patient who was the first case of mandibular reconstruction with free fibula flap reported in the literature. The original report describes a 27-year-old woman who had undergone extensive mandibulectomy due to an osteosarcoma. A microvascularized fibula flap was used for

Deep circumflex iliac artery perforator flap with iliac crest for oromandibular reconstruction.

When combined with iliac bone, perforator flaps are more chimeric, and there is increased mobile skin island to reconstruct soft tissue defects in the oral and maxillofacial region. This study examined oromandibular defects reconstructed using deep circumflex iliac artery perforator flap with iliac crest (DCIAPF). We retrospectively reviewed records of 23 patients with mandibular defects received DCIAPFs after oncological resection for oromandibular reconstruction from November 2015 to August 2016. All perforators, identified before surgery by Doppler examination, were terminal perforators of

Use of porous space maintainers in staged mandibular reconstruction.

The success of mandibular reconstructions depends not only on restoring the form and function of lost bone but also on the preservation of the overlying soft tissue layer. In this case study, 5 porous polymethylmethacrylate space maintainers fabricated via patient-specific molds were implanted initially to maintain the vitality of the overlying oral mucosa during staged mandibular reconstructions. Three of the 5 patients healed well; the other 2 patients developed dehiscences, likely due to a thin layer of soft tissue overlying the implant. The results presented provide evidence that a larger

Focal cemento-osseous dysplasia involving a mandibular lateral incisor.

AIM: To report a case of focal cemento-osseous dysplasia (FCOD) affecting a single tooth misdiagnosed as an inflammatory periapical lesion. SUMMARY: The patient, a black 47-year-old woman complained of pain affecting the right side of the mandible. Routine X-ray examination discovered a periapical radiolucency on the mandibular left lateral incisor (tooth 32), which was otherwise normal and not carious. As the response of this tooth to a vitality test was doubtful, the lesion was diagnosed as a periapical granuloma or cyst secondary to pulpal necrosis. Endodontic treatment and curettage of th

[Anaesthesia and spontaneous return of sensation following resection of the mandibular continuity with primary alloplastic materials].

Resection affecting the continuity of the mandible and the mandible nerve leads to complete anaesthesia of the lower lip and mental area, furthermore to deformity and functional disturbance like uncontrolled salivation. In our long term follow up study on multimodal neurological investigation sensibility is reported to have uncomplete regenerated spontaneously often already after one year. Complete sensory, however, could not be found; the incomplete sensory return was to be measured in individually different degrees of hypaesthesia. Furthermore the axonal qualities of the mandible nerve show
