Patterns of care study: comparison of process of post-mastectomy radiotherapy (PMRT) in Japan and the USA.

BACKGROUND: The Patterns of Care Study (USPCS) by the American College of Radiology (ACR) has made significant contributions to improvements in the procedures of care for patients with breast cancer in the USA. The purpose of this study was to identify problems associated with the process of care for patients undergoing post-mastectomy radiotherapy (PMRT) in Japan compared with those in the USA. METHODS: The Japanese Patterns of Care Study Subgroup (JPCS) conducted a national survey in 1998-2000, involving 79 institutions and using two-stage cluster sampling of institutions and patients, whic…

Deltopectoral flap for reconstruction of male breast after radical mastectomy for cancer in a patient on hemodialysis.

A rare case of advanced male breast cancer in a patient on hemodialysis was successfully treated with radical mastectomy and chemotherapy. Computed tomography of the chest revealed multiple pulmonary metastases. After administration of chemotherapy consisting of 5-fluorouracil (4000 mg) and epirubicin (280 mg), the pulmonary metastases disappeared and this was associated with a decrease of serum CEA levels and tumor size. Radical mastectomy and reconstruction for the chest wall defect using a deltopectoral flap (DP flap) were performed. Histopathological examination of the resected specimen sh…

Breast cancer presenting with the syndrome of inappropriate secretion of antidiuretic hormone after simple mastectomy.

A 71-year-old woman showed disorientation 7 days after simple mastectomy for right breast cancer. Computed tomography of the brain was normal. The level of serum sodium was very low (110 mEq/l), while the urine sodium level was normal. The osmolality of urine was higher (342 mosmol/kg) than that of serum (220 mosmol/kg). These data suggested a syndrome of inappropriate secretion of antidiuretic hormone. A fluid restriction, infusion of hypertonic saline and administration of diuretics gradually increased the level of serum sodium. Subsequently, disorientation disappeared. This is a rare case o…

[A case of the Stewart-Treves syndrome (post-mastectomy angiosarcoma) with special reference to a radiological study].

Reported is the case of a 54-year-old female manifesting the Stewart-Treves syndrome (a postmastectomy angiosarcoma.) The patient had developed an angiosarcoma in her lymphoedematous right shoulder and lower neck develop ten years after a radical mastectomy for a papillo-tubular adenocarcinoma of the right mammary gland. Radiologically, a well-enhanced tumor was revealed by computed tomography, which angiographically had more stains and tumor vessels than usual for a mammary carcinoma and less than usually seen in a cavernous hemangioma. Histologically, this tumor showed less of a luminal diff…

[Changes in the lymphatic vessels of the upper extremity following a radical mastectomy (based on lymphographic data)].

The study group included 171 patients suffering from postmastectomy syndrome which had developed either after combined treatment for breast cancer or after radical mastectomy alone. Lymphographic examination of 146 cases carried out long after treatment for primary treatment revealed frequent roentgenologic features characteristic of all patterns of secondary obstructive lymphatic edema. In 20 patients, lymphographically-detectable changes were followed at early stages (until day 40 after radical mastectomy) before edema became clinically apparent. Radiation therapy and erysipelatous inflammat…

[Conservative primary operation of carcinoma of the breast: subcutaneous mastectomy, lymphadenectomy and radiotherapy (author's transl)].

So-called nipple-preserving operations were performed as primary treatment of mammary carcinoma, predominantly in stage T1, in 135 women. The operation consisted of subcutaneous mastectomy, axillary lymphadenectomy, nipple biopsy and X-ray therapy. Most of the patients in whom more than two lymph nodes had carcinomatous changes also received chemotherapy. Survival of more than five years was achieved in 48 patients in stage T1. One woman died of carcinoma; in six there was further growth with metastases. In no case was there further growth in the nipple region. Although the numbers are small o…

Comparing radical mastectomy with quadrantectomy, axillary dissection, and radiotherapy in patients with small cancers of the breast.

From 1973 to 1980, we carried out a controlled study at the National Cancer Institute in Milan to consider the value of a conservative procedure in patients with breast cancer of small size. We randomized 701 patients with breast cancer measuring less than 2 cm in diameter and with no palpable axillary lymph nodes to Halsted radical mastectomy or to "quadrantectomy" with axillary dissection and radiotherapy to the ipsilateral residual breast tissue. We treated 349 patients with Halsted mastectomy and 352 with quadrantectomy. The two groups were comparable in age distribution, size and site of …

The Importance of Continued Vigilance after Risk-Reducing Mastectomy in BRCA Carriers: A Case Report.

INTRODUCTION: Risk-reducing mastectomy (RRM) is safe and effective in preventing breast cancer (BC) in BRCA mutation carriers. Nipple-sparing mastectomy (NSM) has become the new standard due to better esthetic result and comparable efficacy profile. After RRM, the risk of developing BC still exists though hardly quantifiable due to few data in literature. CASE PRESENTATION: We describe the case of a 70-year-old woman who was found to harbor a BRCA2 mutation after diagnosis of ovarian cancer (OC) in her sister, underwent prophylactic surgery for BC and OC, as per international guidelines, and …

Survival Outcomes With or Without Risk-Reducing Mastectomy in BRCA1 and BRCA2 Pathogenic Variant Carriers.

PURPOSE: Many women carrying pathogenic variants (pvs) of BRCA1 and BRCA2 genes (pvBRCA1/2) elect to undergo bilateral risk-reducing mastectomy (BRRM) in the belief that it will improve their overall survival (OS). Although many are satisfied with their decision to undergo BRRM a significant minority exhibit regret. We compared long-term oncology outcomes in women with pvBRCA1/2 choosing BRRM with those choosing a program of imaging surveillance. METHODS: All participants were attendees at a regional family history/genetics service and had undergone genetic testing for pvBRCA1/2. Carriers of …

Mastectomy for Breast Cancer in Nigeria: A Mixed-Methods Study of Barriers, Facilitators, and Patient Attitudes.

BACKGROUND: Approximately half of Nigerian women with non-metastatic breast cancer undergo surgery, yet psychosocial factors influencing mastectomy receipt are poorly understood. AIMS: This study aimed to understand barriers, facilitators, and patient attitudes toward mastectomy for breast cancer in Nigeria. METHODS: Using a prospective, convergent mixed-methods study design, we disseminated a structured questionnaire (N = 225) and conducted in-depth interviews (N = 18) among women with non-metastatic breast cancer recommended for mastectomy at 3 academic Nigerian institutions. Logistic regr…

The effect of gender role stress on psychological distress in women with breast cancer after mastectomy.

PURPOSE: Breast cancer is also the most common cancer in women in Turkey. The breast is closely linked to female identity and the feminine gender role. It plays an important role in a woman's body image, self-perception, and consequently, her psychological well-being. One of the variables that is not sufficiently recognized in breast cancer treatments, especially after mastectomy, is gender roles and the stress stemming from these roles. This study examined the impact of gender role stress on psychological distress in women. METHODS: A descriptive cross-sectional study was conducted with 264 …

Surgical Outcomes of Ambulatory Mastectomy and Immediate Alloplastic Reconstruction for Breast Cancer or Risk Reduction.

INTRODUCTION: There is a paucity of data on surgical outcomes after ambulatory mastectomy and immediate reconstruction. Our objective was to evaluate the feasibility and safety of ambulatory mastectomy with immediate alloplastic reconstruction. METHODS: We conducted a single institution retrospective cohort study of patients who underwent ambulatory mastectomy with immediate alloplastic reconstruction between 2015 and 2021. Demographic and clinical data were collected from medical chart review and summarized with descriptive statistics. A multivariable logistic regression model was used to de…

Osteopathic Manipulative Treatment as an Adjunctive Therapy for Post-mastectomy Pain Syndrome: A Case Report.

Post-mastectomy pain syndrome (PMPS) is a common yet often under-treated complication of breast cancer therapy, affecting an estimated 20-60% of patients. Characterized by persistent pain lasting greater than three months following breast surgery, PMPS is multifactorial in etiology, involving neuropathic injury, post-surgical fibrosis, radiation-induced tissue changes, and musculoskeletal dysfunction. Current management strategies emphasize a multidisciplinary approach, including pharmacological therapies and non-pharmacological modalities. Despite this, treatment outcomes remain variable, and…

A New Pedicle for Patients with Macromastia and Grade III Ptosis Undergoing Nipple-Sparing Mastectomy.

INTRODUCTION: The incidence of nipple-areolar complex (NAC) necrosis is high in patients with macromastia and grade III ptosis having a nipple-sparing mastectomy (NSM). The pedicle selected is based on empirical cadaver studies. However, recent studies using digital subtraction magnetic resonance studies (DCE-MRI) and dopplers show that the major supply comes from superomedial (second intercostal perforator) and superolateral (lateral thoracic perforator). This study reports on the outcome of 55 patients having a NSM, where the superomedial and superior pedicles werecombined, over a 2 1/2-year…

Impact of Obesity on Surgical Complications in Gender-Affirming Mastectomy: A Systematic Review and Meta-analysis.

BACKGROUND: Body mass index (BMI) is sometimes used to determine surgical eligibility for gender-affirming mastectomy (GAM) but there is insufficient data informing these criteria. Given the limitations of single-institution studies, which lack the ability to generalize the potential impact of BMI on GAM complications across various surgical practices, a systematic review is needed. METHODS: A systematic review and meta-analysis was performed for all studies that evaluated the relationship between BMI and surgical outcomes after GAM. Our institution's GAM cohort (n = 457) was also included. O…
