[The usefulness of thermography as a diagnostic method in dermatology on the basis of clinical trials in 2001-2005].

INTRODUCTION: Thermal imaging is a powerful tool for the study of temperature of the human body. Even though the skin lies superficially and as such is an ideal object for thermography, the method has not gained widespread acceptance as a diagnostic modality in dermatology. The aim of this study was to describe the methodology of thermography for applications in dermatology and to develop a method for computer processing ofthermograms. In addition, we searched for skin factors, which could be responsible for false results in thermography and errors during interpretation of thermal images. MAT

Developing a comprehensive scale for measuring explanatory models of mental disorders among Japanese adults.

INTRODUCTION: Individuals differ in their understanding of the causes of illnesses or problems and the appropriate methods for addressing them. Mental disorders can be interpreted from multiple perspectives shaped by cultural backgrounds and personal experiences. The conceptual framework through which individuals recognize and interpret illnesses or problems is referred to as an explanatory model. This study aimed to develop the Explanatory Model Scale for Mental Disorders (EMS-MD), based on Haslam's (2003) explanatory styles-"Medicalizing," "Moralizing," and "Psychologizing"-with the addition

Qualitative and Quantitative Workplace Analysis of Staff Requirement in an Academic Radiology Department.

PURPOSE: The role of today's hospital-based radiologists goes far beyond interpretation-related tasks. This observational study defines these types of activities and quantifies the type of value-adding interactions radiologists experience on a daily basis with referring departments and other health personnel. The purpose of this study is to evaluate the quality and quantity of these value-adding non-image interpretation tasks in the daily routine of hospital-based residents and attending radiologists. METHODS: A prospective, observational study was performed in the radiology department of a G

Certain performance values arising from mammographic test set readings correlate well with clinical audit.

INTRODUCTION: Test sets have been increasingly utilised to augment clinical audit in breast screening programmes; however, their relationship has never been satisfactorily understood. This study examined the relationship between mammographic test set performance and clinical audit data. METHODS: Clinical audit data over a 2-year period was generated for each of 20 radiologists. Sixty mammographic examinations, consisting of 40 normal and 20 cancer cases, formed the test set. Readers located any identifiable cancer, and levels of confidence were scored from 2 to 5, where a score of 3 and above

The importance of patient-reported outcomes: a call for their comprehensive integration in cardiovascular clinical trials.

Patient-reported outcomes (PROs), such as symptoms, health-related quality of life (HRQOL), or patient perceived health status, are reported directly by the patient and are powerful tools to inform patients, clinicians, and policy-makers about morbidity and 'patient suffering', especially in chronic diseases. Patient-reported outcomes provide information on the patient experience and can be the target of therapeutic intervention. Patient-reported outcomes can improve the quality of patient care by creating a holistic approach to clinical decision-making; however, PROs are not routinely used as

Beliefs about medicines in Dutch acenocoumarol and phenprocoumon users.

AIMS: Adherence to the generally complex regimen of coumarin derivatives is vital in order to keep patients in the adequate International Normalized Ratio range. Patients' beliefs about medicines are associated with the level of therapy adherence. Our first aim was to assess beliefs about coumarins. Secondly, we compared the beliefs about coumarins with the beliefs about other cardiovascular drugs. METHODS: The Beliefs about Medicines Questionnaire was used to assess medication beliefs. The questionnaire was completed by new users of coumarins indicated for venous thromboembolism or atrial fi

Perceptions of Hispanic Patients With Advanced CKD About Communication and Engagement Regarding Kidney Transplantation: A Qualitative Study.

RATIONALE & OBJECTIVE: Barriers in access to kidney transplantation (KT) for Hispanic patients with advanced chronic kidney disease (CKD) are often attributed to spoken languages different from those of their health care providers. Effective communication, however, is not limited to language, and involves consideration of social context and background; if successful, it can lead to better engagement. Understanding patients' perspectives on communication and engagement with their providers regarding KT may inform clinical practice, and this was the objective of the present study. STUDY DESIGN:

Adjusting the Effect of Integrating Antiretroviral Therapy and Tuberculosis Treatment on Mortality for Noncompliance: A Time-varying Instrumental Variables Analysis.

BACKGROUND: Using intent-to-treat comparisons, it has been shown that the integration of antiretroviral therapy (ART) and tuberculosis (TB) treatment improves survival. Because the magnitude of the effect of ART initiation during TB treatment on mortality is less well understood owing to noncompliance, we used instrumental variables (IV) analyses. METHODS: We studied 642 HIV-TB co-infected patients from the Starting Antiretroviral Therapy at Three Points in Tuberculosis trial. Patients were assigned to start ART either early or late during TB treatment or after TB treatment completion. We use

Stopping guidelines for an effectiveness trial: what should the protocol specify?

BACKGROUND: Despite long-standing problems in decisions to stop clinical trials, stopping guidelines are often vague or unspecified in the trial protocol. Clear, well-conceived guidelines are especially important to assist the data monitoring committees for effectiveness trials. MAIN TEXT: To specify better stopping guidelines in the protocol for such trials, the clinical investigators and trial statistician should carefully consider the following kinds of questions: 1. How should the relative importance of the treatment benefits and hazards be assessed? 2. For decisions to stop a trial for b

Statistical evaluation of the diagnostic criteria for meniscal tears.

The accuracy of diagnosis was tested in a prospective study of 145 patients who were arthroscoped because a meniscal tear was suspected clinically. Multiple variate analysis was used to evaluate 68 different clinical parameters. The clinical accuracy was 61% as a tear was demonstrated in 88 knees; 73 of these tears were associated with other intraarticular abnormalities. Thirteen knees were entirely normal at arthroscopy. It was possible to predict a meniscal tear in 8 out of 10 patients.

[The pathography of Dostoyevsky or the dangers of being the Father of the Idiot].

The widespread genre of pathography, or medical biographies of famous people, may be interpreted as a case of stigmatisation. It has been suggested by proponents of the idea of stigmatisation that, by labelling geniuses "mentally ill", psychiatrists advanced their own interest and gained prestige for their own profession. Though this explanation may sound plausible, it appears too schematic to match a more sophisticated reality. The use of medical diagnosis, which doctors applied to celebrities, may appear to stigmatise, but the motives and consequences of writing medical biographies were undo
