The role of myringotomy and ventilation tube insertion in maxillary swing approach nasopharyngectomy: review of our 10-year experience.

OBJECTIVES/HYPOTHESIS: The use of myringotomy with ventilation tube insertion after maxillary swing nasopharyngectomy was originally described to prevent the occurrence of otitis media with effusion. The outcome of this otologic procedure has never been reviewed and discussed. The purpose of this study is to examine the role of myringotomy with ventilation tube insertion in this group of patients. STUDY DESIGN: Retrospective review. METHODS: One hundred forty-two patients with maxillary swing nasopharyngectomy were recruited from 1999 to 2008. The otologic status was evaluated using otoscopy

Analysis of Risk Factors for Myringosclerosis Formation after Ventilation Tube Insertion.

BACKGROUND: This study examined possible risk factors for myringosclerosis formation after ventilation tube insertion (VTI). METHODS: A retrospective study was performed in a single tertiary referral center. A total of 582 patients who underwent VTI were enrolled in this study. Patients were divided into two groups based on the presence or absence of myringosclerosis: MS+ and MS-. Characteristics of patients were collected through medical chart review; these included age, gender, nature and duration of effusion, type of ventilation tube (VT), duration and frequency of VTI, incidence of post-V

Medial displacement of grommets: an unwanted sequel of grommet insertion.

Grommet insertion is one of the commonest surgical procedures performed in the UK. We have come across three cases in which grommets have displaced medially in the middle ear after establishing a satisfactory post-insertion position. We suggest that an abnormally long myringotomy incision and improper placement of the grommet are responsible for this unwanted outcome.

[Severe Bleeding from the Middle Ear Cavity after Myringotomy: Review Based on a Case Report].

Report of a rare case of severe bleeding from the middle ear cavity after myringotomy. On the basis of the case report, the procedure for such bleeding is discussed in the context of the literature. A 6-year-old boy received a revision myringotomy in an ambulant setting. During the procedure a severe bleeding occurred. The external auditory canal was adequately packed. The patient was extubated and transferred to the clinic as an emergency. Computer tomography of the temporal bone showed the anatomical variant of a dehiscent high jugular bulb, which had been injured. Because no rebleeding occu

Tympanostomy tubes and otic suspensions: do they reach the middle ear space?

The treatment of patients with tympanostomy tubes (TTs) and otorrhea with medicated otic suspensions is well known, but confirmation of penetration into the middle ear is difficult. To address this question, we created an in vitro model of the human head and ear and then tested it with 5 different types of liquid exposure: tap water, soapy water, polymyxin B sulfate (Cortisporin), tobramycin and dexamethasone (TobraDex), and ciprofloxacin (Cipro) suspensions. A positive test result corresponded to liquids entering the middle ear through the TT. No positive test result was elicited with tap wat

Nasal microbial composition and chronic otitis media with effusion: A case-control study.

OBJECTIVES: Chronic otitis media with effusion (COME) in children can cause prolonged hearing loss, which is associated with an increased risk of learning delays and behavioural problems. Dispersal of bacterial pathogens from the nasal passages to the middle ear is implicated in COME. We sought to determine whether there is an association between nasal microbial composition and COME in children. METHODS: A case-control study of children aged 3 and 4 years was conducted. Cases undergoing placement of tympanostomy tubes for COME were compared to healthy controls. Nasal swabs were collected and

Unusual Otolaryngologic Manifestations of Paracoccidioidomycosis: A Case Report and Review of Literature.

AbstractParacoccidioidomycosis is a systemic mycosis caused by Paracoccidioides brasiliensis. It occurs more frequently in its chronic form, which particularly affects male adults from rural areas. These patients present with pulmonary involvement and systemic symptoms. Skin and mucosal lesions are rather typical and might suggest the diagnosis. The involvement of the upper airway mucosa is common and the patients usually complain of dysphagia and dysphonia. Nonetheless, in endemic areas, physicians should maintain a high level of suspicion even when faced with some atypical symptoms. We prese

Evoked otoacoustic emissions in children in relation to middle ear impedance.

The influence of changes in middle ear impedance with and without serotympanon on the measurement of evoked otoacoustic emissions (EOAE) was investigated in 108 children between 3 and 12 years of age. Children with proven serotympanon never showed good EOAE. In those who only had changes in middle ear impedance without a serotympanon, the decrease in EOAE amplitude was more related to the magnitude of conductive hearing loss than to the change in impedance itself. These results are compared with those of a neonatal screening project in which some of the 532 healthy fullterm newborns showed poo

[Cochlear implantation in children with otitis media: third stage of a long-term prospective study].

INTRODUCTION: The age at which cochlear implantation (CI) is performed in children generally corresponds to the age at which the prevalence of otitis media (OM) is highest. The risks of problematic middle ear infection and of potential spread of middle ear infection along the electrode array into the cochlea and the central nervous system are relatively high. Thus, it is necessary to establish a practicable protocol aimed at controlling OM prior to and after CI in young candidates. OBJECTIVE: To assess the risk for otitis media after cochlear implantation in otitis media (OM)-prone and non-OM

Acute mastoiditis in infancy: the Soroka experience: 1990-2000.

UNLABELLED: Acute mastoiditis (AM) is an uncommon but serious complication of acute otitis media (AOM). In the pre-antibiotic era, AM was seen more frequently than it is today, but it was rare in infants. However, in the last two decades an increase in the incidence of AM in infancy has been reported in the literature. During the years 1990-2002, we treated 113 patients with 128 episodes of AM; of them, 24 were infants (median age 6 months; 18 males) who suffered from 26 episodes of AM. Twenty developed AM as a complication of their first episode of AOM. One of the four infants with a prior hi

Functional cleft palate surgery.

Cleft lip and palate (CLP) as a dislocation malformation confronts parents with a malformation of their child that could not be more central and visible: the face. In addition to the stigmatizing appearance, however, in cases of a CLP, food intake, physiological breathing, speech and hearing are also affected. In this paper, the principles of morphofunctional surgical reconstruction of the cleft palate are presented. With the closure of the palate, and restoration of the anatomy, a situation is achieved enabling nasal respiration, normal or near normal speech without nasality, improved ventila

Nitric oxide in health and disease from the point of view of the otorhinolaryngologist.

Nitric oxide (NO) plays role in a great range of important functions in the organism, such as vasodilatation, relaxation of muscles, neurotransmission, neuromediation, and host defense reactions. In the upper airways, nasal cavities and paranasal sinuses are the main sources of this biological mediator. Although the exact role of NO in nasal physiology remains poorly understood, the functions are thought to be host defense, ciliary motility and improved ventilation-perfusion ratio in the lungs by auto-inhalation. Low NO concentrations were reported in certain diseases such as primary ciliary d

Development of an intelligent surgical instrument for otitis media with effusion.

To treat a worldwide common ear disease (OME), a device allowing fast grommet tube insertion has been designed in our earlier works (Gao et al., 2015 [1] and Liang et al., 2013 [2]). However, the instrument has to be manually placed as close as to the Tympanic Membrane before the insertion procedures. To realize a fully automated surgical process, the instrument shall be automatically manipulated to align to the axial direction of ear canal and proceed to complete the surgery. A vision-based servomechanism is proposed to solve the path planning problem. A fuzzy-gain-scheduled controller is pro

Evaluating treatments for Ménière's disease: controversies surrounding placebo control.

Although double-blind experimental designs are considered the gold standard for documenting treatment effectiveness, many treatments for Ménière's disease have not been evaluated using this methodology. Particularly with a disease characterized by exacerbation and remission, carefully controlled, long-term studies are required. The nature of the placebo effect is described in this article, and the concept of debonafide effect introduced. Ideally, patients should be given treatments supported by evidence-based medicine that have the lowest possible risk of side effects. However, risk minimizati

Long-term results and surgical indication of ventilating tube insertion during cleft palate surgery.

BACKGROUND: s: While ventilating tube insertion (VTI) is often performed, its necessity in cleft palate (CP) children without otitis media with effusin (OME) remains unclear. OBJECTIVE: We aim to reassess the safety of refraining from early VTI in CP children who do not have evidence of OME. METHODS: We retrospectively reviewed 1026 children with CP who underwent surgery at the Asan Medical Center (Jan 2000-Dec 2015). Among these, 69 patients (34 males, 35 females) had no history of otitis media and no middle ear effusion during preoperative endoscopic examination; they did not receive VTI.

Survey on the use of ototopical medications by New Zealand otolaryngologist/head and neck surgeons.

AIM: There is increasing awareness of the potential for ototoxicity with the administration of aminoglycoside eardrops in the presence of a non-intact tympanic membrane, whether that is due to a perforation or due to the presence of a patent ventilating tube. Expert panels in the USA, Canada, UK, and Australia have all advocated the preferential use of non-ototoxic topical antibiotics for the discharging middle ear and/or mastoid cavity. The advent of non-ototoxic fluoroquinolone eardrops provides the first real opportunity to prescribe topical therapy in accordance with such guidelines. METH
