Military Standards for Anaphylaxis: Aligning Allergy Science, Operational Risk, and Waiver Documentation.

PURPOSE OF THE REVIEW: Allergic disease is common in the United States, and a growing number of otherwise qualified applicants and service members present with histories of food allergy, venom allergy, medication reactions, physical urticarias, or prior anaphylaxis. The challenge for clinicians is to translate a nuanced allergy-immunology evaluation into strict military accession and retention standards designed for readiness, austere care, and deployability, rather than for diagnosis alone. RECENT FINDINGS: The current accession standard disqualifies a history of anaphylaxis other than to a

Non-Battle Injury in Military Operations Novel Treatment for Soft Tissue Injury and Infection with Logistics Implications.

BACKGROUND: Non-battle injuries (NBIs) represent a substantial proportion of morbidity during military deployments, often exceeding combat-related injuries in frequency. NBIs, including superficial wounds, burns, blisters, and soft tissue infections, remain a significant source of lost duty time and operational disruption in deployed military populations. In austere environments with limited medical support, even minor injuries often progress to cellulitis or abscess, which require substantial logistical resources including irrigation fluids, medical dressings, and antibiotics, impairing opera…

Genito-Urinary Trauma in the War Context: A Review of Two Decades of Experience From the French Armed Forces.

BACKGROUND: In recent conflicts, injuries to urogenital organs ranged from 5 to 7.2% of all combat-related injuries. Open surgery remains the main approach in combat settings, and in the French military, urgent surgical procedures are mostly performed by general surgeons with no specialization in urological care. To explore the specific needs in advanced surgery courses, we aimed to describe the epidemiology and management of genito-urinary traumas in recent conflicts in French Medical Treatment Facilities (MTFs). METHODS: Data were extracted from the OPEX® registry, which prospectively recor

Risk and protective factors for self-harm and suicide behaviours among serving and ex-serving personnel of the UK Armed Forces, Canadian Armed Forces, Australian Defence Force and New Zealand Defence Force: A systematic review.

BACKGROUND: Self-harm and suicide behaviours are a major public health concern. Several factors are associated with these behaviours among military communities. Identifying these factors may have important implications for policy and clinical services. The aim of this review was to identify the risk and protective factors associated with self-harm and suicide behaviours among serving and ex-serving personnel of the United Kingdom Armed Forces, Canadian Armed Forces, Australian Defence Force and New Zealand Defence Force. METHODS: A systematic search of seven online databases (PubMed, Web of S

Early Tympanoplasty Using a Synthetic Biomembrane for Military-Related Blast Induced Large Tympanic Membrane Perforation.

INTRODUCTION: Blast injuries in modern warfare are common, and tympanic perforation is often found. Spontaneous closures of large perforations that encompass greater than 80% of the tympanic surface are rare. Early closure of the tympanic membrane avoids the immediate infectious risk, which potentially complicates the initial management of these war-wounded patients, and allows for safe and early recovery of military activity. This study compared the outcomes of spontaneous closures and early biomembrane myringoplasty in subjects with large blast injury-induced tympanic perforation following a

Stress and mental disorders in female military personnel: comparisons between the sexes in a male dominated profession.

The proportion of women in militaries is growing; however, many studies in the area of military mental health have been conducted with majority male samples. The present study examined sex differences in trauma exposure, work stress, and mental disorders in the Canadian Community Health Survey - Canadian Forces Supplement, a representative sample of 5155 regular force personnel and 3286 reservists ages 16-54. Past-year DSM-IV mental disorders (depression, generalized anxiety disorder, panic disorder, social phobia, PTSD, and alcohol dependence), lifetime exposure to 28 traumatic events, and wo

Lifetime fluoridation exposure and dental caries experience in a military population.

UNLABELLED: While there is good evidence of caries-preventive benefits of fluoride in drinking water among children and adolescents, there is little information about effectiveness of water fluoridation among adults. OBJECTIVES: To determine whether exposure to fluoride in drinking water is associated with caries experience in Australian Defence Force (ADF) personnel. METHODS: Cross-sectional study of 876 deployable ADF personnel aged 17-56 years. At each person's mandatory annual dental examination, military dentists recorded the number of decayed, missing and filled teeth (DMFT) using visu

Long-lasting T cell responses to biological warfare vaccines in human vaccinees.

BACKGROUND: Medical countermeasures against biological warfare include the use of vaccines for anthrax and plague, which require repeated dosing and adjuvant to achieve adequate protection from threats such as inhalational anthrax and pneumonic plague. Despite the widespread use of these measures in preparation for recent military deployments, little is known about the cell-mediated immune response that is induced by these vaccines, in comparison with conventional vaccines, such as pertussis or tetanus-diphtheria vaccines. METHODS: To examine this question, we used cytokine enzyme-linked immu…

Dental Readiness Class 1 Military Personnel Are Not All the Same! A Systematic Review and Meta-Analysis.

INTRODUCTION: Dental emergencies (DEs) pose a significant threat to the readiness and operational effectiveness of the U.S. military. Caries, third molars, broken teeth/defective restorations, and pulpal problems account for a substantial 76.34% of all DEs seen at dental treatment facilities during deployments. The authors aim to investigate the effect of common dental treatments on the annual probability of experiencing a dental emergency through a literature review and meta-analysis. MATERIALS AND METHODS: Three unique searches were conducted, one for each of the dental treatments of intere

Malaria and mefloquine prophylaxis use among Japan Ground Self-Defense Force personnel deployed in East Timor.

BACKGROUND: Malaria poses a significant threat to military personnel stationed in endemic areas; therefore, it is important to examine the risks of military operations, particularly in areas where malaria-related data are scarce. The recent deployment of Japan Ground Self-Defense Force (JGSDF) for a peacekeeping operation in East Timor provided an opportunity to investigate these risks. The results of these studies may be translated into chemoprophylactic strategies for travelers. METHODS: A total of 1,876 members were deployed between April 2002 and September 2003. They consisted of three ba

Characterizing the Influence of Exposure to Military Sexual Trauma and Intimate Partner Violence on Mental Health Outcomes among Female Veterans.

Military sexual trauma (MST) has deleterious long-term psychological consequences. Among female U.S. military members, MST is associated with increased risk for future interpersonal victimization, such as experiencing intimate partner violence (IPV). Few studies have investigated the implications of the cumulative effects of IPV and MST on psychological functioning. This study examined rates of co-exposure to MST, IPV, and their cumulative impact on psychological symptoms. Data were collected from 308 female Veterans (FVets; age: M = 42, SD = 10.4) enrolled in an inpatient trauma-focused treat

Psychosocial well-being among veterans with posttraumatic stress disorder and substance use disorder.

OBJECTIVE: Concurrent posttraumatic stress disorder and substance use disorder (PTSD/SUD) in U.S. military veterans represents an urgent public health issue associated with significant clinical challenges. Although previous research has shown that veterans with PTSD/SUD endorse more psychosocial risk factors and fewer protective factors than veterans with neither or only one of these disorders, no study has applied a comprehensive framework to characterize the vocational, financial, and social well-being of veterans with PTSD/SUD. Furthermore, it is not fully known how well-being among veteran
