Morphine was administered intravenously in bolus doses 6-hourly to 10 patients; 9 developed signs of sympathetic overactivity and required increased morphine dosage. The mean daily morphine dosage was 103 +/- 36 mg and the maximum daily dosage was 170 +/- 65 mg. In all cases morphine decreased the mean arterial blood pressure (mean 18%; P less than 0.01) and heart rate (mean 7%; P less than 0.01). Three histamine H1-receptor antagonists, chlorpheniramine, diphenhydramine and tripelennamine, were tested alone or in combination with morphine on locomotor activity in C57BL/6 mice. All three antihistaminics, at some dosage levels, slightly increased activity when given alone, but strongly enhanced morphine-induced hyperactivity. The results demonstrate that locomotor activity represents a useful test to evidence stimulatory effects of antihistaminic-opiate combinations.

High doses of caffeine-containing as well as decaffeinated instant coffee neither inhibited morphine-induced analgesia in mice nor the morphine-induced fall of blood pressure, heart rate and respiratory rate in rats. On the contrary, caffeine-containing coffee even enhanced the analgesic effects of morphine in mice. Coffee thus does not exhibit opiate-antagonizing activity in the whole organism in vivo. The very weak morphine-antagonistic efficacy of coffee powder in the myenteric plexus-longitudinal muscle preparation from the guinea pig ileum is of no practical importance.

A commercially available haemagglutination inhibition (HI) test (Drug Test Opiates, Boehringer Biochemia Robin) for the detection of morphine in urine has been evaluated alongside a routine TLC method and a radioimmunoassay. The use of intraspinal narcotics has been widely accepted as pain relief treatment for intractable cancer pain. Intraspinal low doses of morphine induce a potent selective long lasting analgesia. To avoid repetitive lumbar puncture, a drug delivery device was surgically implanted in 41 patients. The surgical procedure is described. In unanaesthetized cats the pronounced hyperglycaemia produced by intravenous morphine (5 mg/kg) was inhibited by naloxone. Satisfactory control of intractable pain has been achieved in 17 terminal cancer patients by injecting small doses of morphine into the lateral cerebral ventricle via an Ommaya reservoir. Pain relief together with a favorable behavioral response was obtained without interference with other sensory modalities, noticeable physical changes, or side effects annoying or severe enough for the patient to discontinue therapy. It has been established that morphine interacts with albumins, alpha 2- and beta-globulins, while promedol with all globulin fractions and to a less measure with plasma albumins of the intact rabbit. Interaction of morphine and especially that of promedol with the same protein fractions of allergizated animals proceeds more actively and is characterized by a more pronounced formation of biological complexes with alpha 1-globulins. Decreased pharmacological activity of morphine and promedol coincides with intensification of their interaction with proteins.
