[The histamine-serotonin-monoamine oxidase system in patients with foot mycosis during treatment].

Examinations of 18 patients with mycoses of the foot complicated by allergic processes, have revealed elevated blood serum concentrations of histamine, serotonin, cathepsin D, and acid phosphatase, and a lowered activity of monoamine oxidase, as compared to 22 patients with mycoses not complicated with allergic manifestations. This fact reflects one of the aspects of a pathogenetic difference between these patients and necessitates combined therapy to correct the detected abnormalities.

Cutaneous manifestations of subcutaneous and systemic fungal infections in tropical regions: a retrospective study from a referral center in southern Taiwan.

BACKGROUND: Deep cutaneous fungal infections, including subcutaneous mycoses and systemic fungal infection with cutaneous involvement, cause significant morbidity and mortality in light of increasing immunocompromised patients and global warming. Although a few studies reviewed deep fungal infections in temperate regions, a relevant study in tropical regions is lacking. We evaluated features of deep cutaneous fungal infections in southern Taiwan among the tropical regions. METHODS: We retrospectively reviewed all histopathological specimens with deep cutaneous fungal infections in a single re…

[Antifungal prophylaxis: up-date and prospectives].

Author reviews the now available strategies for the prophylaxis of primary and opportunistic mycoses. Chemoprophylaxis of opportunistic mycoses that are the most frequently observed fungal infections in Italy, gives satisfactory protection in endogenous fungal infections. On the contrary, fungal exogenous infections are not easily treated. In fact, systematically acting drugs which are also easy to use, are still lacking. Strict reverse isolation seems to achieve good results in these infections.

Pulmonary fungal infections in immunocompromised children.

Pulmonary mycoses are increasingly encountered in children with underlying diseases associated with immunosuppression, including children with cancer or HIV infection. Knowledge of the epidemiology and pathogenesis of pulmonary mycoses can be applied to prevent infection in many cases and to promptly diagnose infections that do occur. Treatment is usually successful if initiated early, although pulmonary aspergillosis and zygomycosis are portentous ailments unless surgical resection or prompt immunologic recovery ensue.

Frequency of mycological diagnosis in a clinical laboratory in the city of Pelotas, south of Brazil, during the three-year period (2020-2023).

The increasing prevalence of immunosuppressive diseases, the increase in the elderly population, and globalization have led to the urgent need for research on the epidemiology and prevalence of fungi that cause mycoses in Brazil and worldwide. This is especially important, in the case of Brazil, for the South region, which currently lacks updated data on the subject. To this end, the objective was to investigate fungi diagnosed in mycoses by a laboratory in the city of Pelotas, South of Brazil. An observational and analytical survey was conducted for three years to analyze fungal diagnoses in …

The Rise and Fall of Oral Ketoconazole.

BACKGROUND: Ketoconazole was the first broad-spectrum oral antifungal agent available to treat systemic and superficial mycoses. Evidence of hepatotoxicity associated with its use emerged within the first few years of its approval. Growing evidence of serious side effects including endocrine dysregulation, several drug interactions, and death led to the review of oral ketoconazole in 2011. OBJECTIVE: This article chronicles the use of oral ketoconazole from its introduction to its near replacement in medicine. CONCLUSION: Due to its hepatotoxic side effects, oral ketoconazole was withdrawn f…

State-of-the-art review of pulmonary fungal infections.

The endemic mycoses are restricted geographically based on environmental and other factors that favor the growth of these organisms in the soil. Histoplasmosis and blastomycosis mostly afflict patients in the Mississippi and Ohio River Valleys whereas coccidioidomycosis occurs primarily in the desert southwest United States. Cryptococcosis also may present as pulmonary disease, particularly in persons with cellular immune impairment. These mycoses are increasing in importance as causes for opportunistic disease in immunocompromised patients, especially those with acquired immune deficiency syn…

Fungal Lesions of the Oral Mucosa Diagnosis and Management.

Oral fungal infections are opportunistic and due to impaired host resistance. The increasing number of immunosuppressed individuals contributes to rising numbers of mycoses worldwide, and the ease of global migration has allowed the geographic range of endemic mycoses to expand. Deep fungal infections can clinically mimic other pathologic conditions including malignancy. This review highlights the pathogenesis, clinical features, diagnosis, and treatment recommendations of eight fungal infections that can be encountered in the dental setting.

[Levuroses appearing in cases of severe burns].

The preconditions for the development of systemic mycoses, mainly levuroses, caused by opportunistic organisms are particularly favorable in the severely burned patients: on the one hand as a consequence of suppression of immunity, on the other hand by the overgrowth of these fungi in the digestive tract as a side-effect of the antibacterial chemotherapy which is necessary for the treatment of the original disease. Since such mycoses are usually very severe, mostly life endangering, their prophylaxis and early diagnosis are highly important. The aspects and measurements, most important in this…

[Mycosis of the maxillary sinus].

In the structure of morbidity and mortality has significantly increased the role of mycoses. The increase in their spread is due not only to the improvement of the quality of diagnosis, but also the emergence of new strains resistant to previously used antifungal drugs, a significant change in the immune status of the population. In addition to environmental factors, it is important to introduce modern methods of treatment that increase survival in the whole spectrum of diseases. This situation can't affect the structure of diseases of ENT organs: the number of mycoses has increased significan…

Immunotherapy against Systemic Fungal Infections Based on Monoclonal Antibodies.

: The increasing incidence in systemic fungal infections in humans has increased focus for the development of fungal vaccines and use of monoclonal antibodies. Invasive mycoses are generally difficult to treat, as most occur in vulnerable individuals, with compromised innate and adaptive immune responses. Mortality rates in the setting of our current antifungal drugs remain excessively high. Moreover, systemic mycoses require prolonged durations of antifungal treatment and side effects frequently occur, particularly drug-induced liver and/or kidney injury. The use of monoclonal antibodies with…

Medication association and immunomodulation: An approach in fungal diseases and in particular in the treatment of paracoccidioidomycosis.

Fungal infections have been increasing in recent decades, mainly affecting immunocompromised individuals, although certain mycoses, such as paracoccidioidomycosis (PCM), infect immunologically competent individuals. The major problems observed regarding fungal diseases are inadequate diagnosis, prolonged treatment time, the reduced number of drugs available for treatment, in addition to the fact that there are no vaccines for clinical use. Drug combination in order to immunomodulate the immune response is a new strategy used for the treatment of mycoses, since it is difficult to develop new an…

Therapeutic innovation for multi-resistant candidemics: Synergy of isavuconazole and caspofungin association.

Fungal infections occurring in immunocompromised patients after immunochemotherapy treatment are often difficult to eradicate and capable of even being fatal. Systemic mycoses affecting severely immunocompromised patients often manifest acutely with rapidly progressive pneumonia, fungemia, or manifestations of extrapulmonary dissemination. Opportunistic fungal infections (mycoses) include several pathogens elements, as candidiasis, aspergillosis, mucormycosis (zygomycosis) and fusariosis. Prompt diagnosis and effective therapy are needed to improve the associated morbidity and mortality, espec…

[New developments in medical mycology].

Not only have the systemic mycoses clearly increased in number but also mycoses of the skin are more common than presumed in the past. Today onychomycosis is found in up to 10% of human beings. Onychomycosis can compromise quality of life markedly. Common tinea pedis is one of the most important risk factors for erysipelas of the lower legs. The clinical presentation of oral candidosis in HIV-infected patients is changing; Candida dubliniensis has been identified as another important causative microorganism. Onychomycosis today in most cases can be cured using terbinafine or itraconazole. When…

Terbinafine: broad new spectrum of indications in several subcutaneous and systemic and parasitic diseases.

Immunocompromised patients are at risk of contracting serious fungal infections. The emergence of acquired resistance to azole treatment by opportunistic fungal organisms is increasing and poses a major therapeutic challenge. Treatment of some deep cutaneous and subcutaneous mycoses remains unresolved, relapses are frequent, lack of tolerability of the antifungal drugs becomes an obstacle, and, unfortunately, surgery is, in some cases, the last option. The development of allylamine antifungals, of which terbinafine is the most effective to date, may help to resolve this situation. In vitro, te…

Antifungal treatment of small animal veterinary patients.

Antifungal therapy has progressed significantly with the development of new drugs directed at various processes in fungal cell metabolism. Within veterinary medicine, treatment options for systemic mycoses remain limited to amphotericin B, ketoconazole, fluconazole, and itraconazole. However, newer triazoles, echinocandins, and lipid-based formulations of amphotericin B are now approved for use in humans. This article provides a comprehensive review of the antifungal medications available for veterinary patients, and includes a brief discussion of the newer, presently cost-prohibitive, antifun…

Tinea versicolor, tinea nigra, white piedra, and black piedra.

Superficial mycoses are fungal infections limited to the stratum corneum and its adnexal structures. The most frequent types are dermatophytoses or tineas. Tinea versicolor involves the skin in the form of hypochromic or hyperchromic plaques, and tinea nigra affects the skin of the palms with dark plaques. White piedra and black piedra are parasitic infections of scalp hairs in the form of concretions caused by fungal growth. Diagnosis of these mycoses is made from mycologic studies, direct examination, stains, and isolation, and identification of the fungi. Treatment includes systemic antifun…
