A 20-year-old male sustained an inferior myocardial infarction following blunt chest trauma, after a motor vehicle accident. Though coronary arteriograms 9 months later were normal, LV angiography revealed severe hypokinesia of the inferobasal and diaphragmatic segments. The literature is reviewed with respect to mechanisms of injury, autopsy and angiographic findings and clinical outcome in this condition.

Low-dose intravenous nitroglycerin infusion can be safely administered during acute myocardial infarction to unload the left ventricle and salvage ischemic myocardium and left ventricular geometry and function. In an experimental conscious dog model, low-dose infusion titrated to decrease mean blood pressure by 10% over the first 6 hours after coronary artery ligation resulted in 51% decrease in infarct size, 54% decrease in preload, and more than 50% increase in collateral blood flow. To characterize the clinical and pathologic features of acute myocardial infarction (AMI) in cardiac transplant recipients, 22 Stanford patients who had 25 documented infarcts at a mean of 3.86 years after transplantation were reviewed. Plasma flecainide concentrations were measured in 10 patients with acute myocardial infarction. Following an oral loading dose of 300 mg plasma concentrations were highly variable and unpredictable. Prophylactic diuretic therapy in acute myocardial infarction (AMI) was evaluated in 83 consecutive patients without severe left ventricular failure (LVF) on admission. A high dose group (HDG) received 120-160 mg and a low dose group (LDG) 20-40 mg furosemide daily for six weeks. Mortality and reinfarction rates did not differ between the groups. Morphine anesthesia is often recommended in patients with reduced cardiac reserve. As the effect of morphine on the balance between myocardial oxygen supply and demand is not well understood, the effect of large doses of morphine (3 mg/kg, subcutaneously) on experimental myocardial infarction size in the rat was evaluated. The development of massive anteroseptal and high lateral myocardial infarction was observed in a 29-year-old male patient 7 months after symptoms of thyrotoxicosis had appeared. A selective coronaroangiography, performed later, revealed no significant stenosis. It was postulated that coronary embolism, coronary angiospasm and "small vessel disease" were possible causes of the myocardial infarction, while thyrotoxicosis was not thought to have played any causative role.

This dramatic and ominous complication usually occurs between the third and 14th day after acute myocardial infarction. The size of the defect varies from a pinhole to 4 cm. The sudden appearance of a loud systolic murmur, with or without a thrill, is often accompanied by rapid hemodynamic deterioration. This event must be differentiated from rupture of a papillary muscle with mitral regurgitation. If intensive medical treatment is ineffective, an attempt at surgical repair is indicated.
