Trends in management and healthcare resource utilization for achalasia following adoption of per-oral endoscopic myotomy.

BACKGROUND: Per-oral endoscopic myotomy (POEM), Heller myotomy (HM), and pneumatic dilation (PD) offer definitive interventions of achalasia, but comparative analysis of long-term healthcare resource utilization is not well described. This study explores longitudinal trends and one-year post-procedure healthcare resource utilization in Indiana following local adoption of POEM in 2014. METHODS: This multicenter retrospective statewide cohort study utilized relevant ICD-9, ICD-10, and CPT codes to identify all patients with achalasia who underwent definitive index disease intervention with POEM…

Contemporary Operative Trends and Outcomes of Laparoscopic and Robotic Heller Myotomy Using a Large National Database.

BackgroundRobotic-assisted Heller myotomy (RAHM) is increasingly utilized for the surgical treatment of achalasia. While prior studies have suggested potential technical advantages of RAHM over laparoscopic Heller myotomy (LHM), the economic implications remain less well-defined. This study aimed to compare hospitalization costs and clinical outcomes between RAHM and LHM using a nationally representative cohort.MethodsThe 2016-2021 Nationwide Readmission Database was utilized to identify adult patients ((≥18 years) undergoing either LHM or RAHM for achalasia. Entropy balancing on key variables…

Posterior Peroral Endoscopic Myotomy With Versus Without Sparing of the Oblique/Sling Fibers: A Meta-analysis.

BACKGROUND AND AIMS: Several studies have hypothesized that sparing the oblique/sling fibers during posterior peroral endoscopic myotomy (POEM) may reduce the incidence of gastroesophageal reflux disease (GERD) and reflux esophagitis without compromising the established safety and efficacy of the procedure. This study compares perioperative, postoperative motility-related, and postoperative GERD-related outcomes between posterior oblique/sling fibers-sparing POEM (OFS-POEM) and conventional posterior POEM through a pairwise meta-analysis of comparative studies. METHODS: We conducted a systema…

Use of intraoperative fluoroscopy to reduce post-peroral endoscopic myotomy reflux: a proof-of-concept study.

BACKGROUND AND AIMS: Post-peroral endoscopic myotomy (POEM) gastroesophageal reflux is a common adverse event. Intraoperative fluoroscopy (IOF) can help identify the gastroesophageal junction (GEJ) during submucosal tunneling and evaluate the extent of myotomy into the stomach during POEM. In this study, we investigated the use of IOF to predict and prevent post-POEM GERD (PPG). METHODS: This was a retrospective review of all patients who underwent POEM with IOF at our institution. A blinded gastroenterologist measured the fluoroscopic angle (FA) between the endoscope tip at the GEJ, before s…

An "esophageal rosette" sign is useful for predicting favorable outcomes in peroral endoscopic myotomy for esophageal achalasia.

BACKGROUND: An "esophageal rosette" (ER) sign is one of the endoscopic findings in primary esophageal achalasia. We investigated whether ER was associated with the therapeutic efficacy of peroral endoscopic myotomy (POEM). METHODS: The clinical characteristics and short-term outcomes of POEM were retrospectively evaluated in 69 patients who underwent the procedure for esophageal achalasia. The patients were divided into two groups according to the presence of an ER sign (ER and non-ER groups). Clinical success was defined as the post-POEM Eckardt score of three or less. RESULTS: On preoperat…

Peroral Endoscopic Myotomy Is an Effective Treatment Option for Managing Jackhammer Esophagus: A Single Center Experience.

BACKGROUND AND AIMS: Jackhammer esophagus (JE) is a rare hypercontractile motility disorder often associated with dysphagia, regurgitation, and chest pain. In patients with clinically relevant symptoms, treatment options aim to decrease esophageal contractions. Medical, endoscopic and surgical therapies have limited long-term efficacy. The advent of peroral endoscopic myotomy (POEM) has evolved as a minimally invasive treatment option. Yet data regarding JE is scare. As such we aimed to investigate the clinical efficacy of JE. PATIENTS AND METHODS: This was a single center retrospective study…

The robot can break bars with the best of them: a novel approach to treating cricopharyngeal bars with myotomy.

INTRODUCTION: Failure of the cricopharyngeus to relax results in oropharyngeal dysphagia, which over time results in hypertrophy and increased risk for aspiration. Open myotomy is one definitive treatment option, however there are several drawbacks attributable to the long neck incision, ± drain placement, and invasiveness of the procedure. We aim to share our experience using the DaVinci robotic platform to perform a minimally invasive cricopharyngeal myotomy, which has never been described before in the literature. METHODS: All robotic cricopharyngeal myotomies performed in adult patients b…

Long-term outcomes of laparoscopic Heller's myotomy with angle of His accentuation in patients of achalasia cardia.

BACKGROUND: Laparoscopic Heller's myotomy (LHM) is an established treatment for achalasia cardia. Anti-reflux procedures (ARP) are recommended with LHM to reduce the post-operative reflux though the optimal anti-reflux procedure is still debatable. This study reports on the long-term outcomes of LHM with Angle-of-His accentuation (AOH) in patients of achalasia cardia. METHODS: One hundred thirty-six patients of achalasia cardia undergoing LHM with AOH between January 2010 to October 2021 with a minimum follow-up of one year were evaluated for symptomatic outcomes using Eckardt score (ES), DeM…
