Development of a rapid metal oxide semiconductor-based sensory system for noninvasive neonatal sepsis detection.

A portable electronic nose system, termed cNose, is presented which combines an optimized metal oxide semiconductor gas sensor array with machine learning algorithms to determine fecal volatile organic compounds (VOCs) for noninvasive bedside screening of neonatal sepsis. The system was evaluated at a public hospital in Sleman, Yogyakarta, Indonesia, using 347 fecal samples (197 positive, 150 negative for sepsis confirmed by blood culture). This balanced dataset was used for exploratory machine learning analysis and proof-of-concept evaluation rather than for population-level inference. Featur…

A Case of Severe Neonatal Neutropenia Secondary to Late-Onset Sepsis From E. coli Omphalitis.

Severe neonatal neutropenia, defined as an absolute neutrophil count (ANC) of <500/µL, is a critical hematologic finding that signals either a severe consumptive process or a primary bone marrow disorder. We present the case of an eight-day-old, full-term female infant who presented with fever and irritability, later diagnosed with severe neutropenia (ANC: 400/µL) and elevated C-reactive protein (CRP) (150 mg/L). Omphalitis was identified as the source, with pus culture growing pan-sensitive Escherichia coli, confirming late-onset neonatal sepsis (LONS). The patient was managed with targeted i…

Artificial Intelligence for Neonatal Sepsis Prediction: A Review of Advances, Clinical Integration, and Future Prospects.

Neonatal sepsis (NS) remains a cardinal cause of morbidity and mortality worldwide, disproportionately affecting neonates in low- and middle-income countries (LMICs). Blood culture, the current diagnostic reference standard, is burdened by prolonged turnaround times and the pervasive phenomenon of culture-negative sepsis, a limitation compounded by antenatal antibiotic exposure in mothers rather than representing an intrinsically poor tool. Conventional biomarkers, including C-reactive protein (CRP), procalcitonin, and lactate, are time-dependent in their rise and require integration with clin…

From Gram-Negative Neonatal Sepsis to Neurodevelopmental Impairment: A Retrospective Cohort Study in Preterm Infants.

BACKGROUND/OBJECTIVES: Gram-negative neonatal sepsis remains a cause of morbidity and mortality in preterm infants, yet the relationship between early clinical severity and long-term neurodevelopmental outcomes is incompletely defined. This study aimed to characterize Gram-negative sepsis in preterm infants and to evaluate its short-term and 18-24-month neurodevelopmental consequences. METHODS: We conducted a retrospective observational cohort study of preterm infants admitted to a tertiary neonatal intensive care unit between 1 January 2022 and 31 December 2023. Infants with culture-proven G…

Neonatal leucocyte cell population data: reference intervals and relevance for detecting sepsis and necrotizing enterocolitis.

BACKGROUND: Timely diagnosis of neonatal sepsis is crucial but remains challenging. Cell Population Data (CPD) provide high-resolution phenotyping of leukocytes, offering potential for sepsis detection. We aimed to establish neonatal CPD reference intervals and explore their capacity to detect sepsis and necrotizing enterocolitis (NEC). METHODS: CPD from neutrophils, monocytes, and lymphocytes were analyzed in hospitalized newborns. Reference intervals (5-95th percentiles), at birth and during the first 28 days, were derived from newborns without conditions potentially impacting CPD (referenc…

Examining Transcriptomic Markers Associated With Neutrophil Extracellular Traps to Predict Mortality Risk in Neonatal Sepsis.

BACKGROUND: Neonates are highly susceptible to sepsis, which is often accompanied by fatal coagulopathy. Anticoagulant therapies have not reduced sepsis-related mortality in clinical trials, possibly due to patient heterogeneity. Neutrophil extracellular traps (NETs) enhance coagulation by activating platelets, suggesting that NET-specific biomarkers may identify patients who may benefit from targeted anticoagulant treatment. This study evaluated the association between NET gene expression and adverse outcomes in neonatal sepsis. METHODS: We analyzed whole blood transcriptomes from 123 neonat…

The diagnostic accuracy of presepsin for late-onset neonatal sepsis: a multicenter prospective cohort study.

BACKGROUND: Antibiotic overtreatment in infants is a significant problem, due to lack of accurate diagnostic tools for late-onset neonatal sepsis (LONS). We aimed to investigate the diagnostic accuracy of presepsin for LONS at initial suspicion. METHODS: In this multicenter prospective observational cohort study, we consecutively included all term and preterm infants who started on antibiotics empirically for a nosocomial LONS suspicion. Presepsin concentrations were determined at initial LONS suspicion before antibiotic initiation (t = 0), and 12 and 24 h afterwards. Diagnostic accuracy meas…

Neonatal Sepsis in Dogs Caused by Beta-Hemolytic Streptococcus Sourced From Maternal Mastitis-Case Report.

Bacterial infections are common during the neonatal period, and sepsis is one of the main causes of mortality in dogs in the first 21 days. Maternal clinical and subclinical mastitis is one of the main sources of neonatal infection, leading to the ingestion of milk with bacteria and bacterial toxins, known as toxic milk syndrome. The objective of this report was to describe a case of neonatal sepsis in puppies secondary to maternal clinical mastitis, as well as its diagnosis and treatment. Three 5-day-old Pug dogs presented signs of neonatal infection: diarrhea, body erythema, and omphalitis. …

Potential of Molecular Culture in Early Onset Neonatal Sepsis Diagnosis: A Proof of Principle Study.

Delay in the time-to-positivity of a peripheral blood culture (PBC), the gold standard for early onset neonatal sepsis (EOS) diagnosis, has resulted in excessive use of antibiotics. In this study, we evaluate the potential of the rapid Molecular Culture (MC) assay for quick EOS diagnosis. In the first part of this study, known positive and spiked blood samples were used to assess the performance of MC. In the in vivo clinical study, the second part of this study, all infants receiving antibiotics for suspicion of EOS were included. At initial EOS suspicion, a blood sample was collected for PBC…

Clinical and laboratory factors associated with neonatal sepsis mortality at a major Vietnamese children's hospital.

Sepsis is a major cause of neonatal mortality and children born in low- and middle-income countries (LMICs) are at greater risk of severe neonatal infections than those in higher-income countries. Despite this disparity, there are limited contemporaneous data linking the clinical features of neonatal sepsis with outcome in LMICs. Here, we aimed to identify factors associated with mortality from neonatal sepsis in Vietnam. We conducted a prospective, observational study to describe the clinical features, laboratory characteristics, and mortality rate of neonatal sepsis at a major children's hos…
